In the past, patients with injuries of the cervical spine and spinal cord have been diagnosed by means of myelography and polytomography. In an attempt to improve the radiographic evaluation of patients with cervical spinal cord injuries the authors performed computerized tomography (CT) scanning of the cervical spine following injection of metrizamide into the spinal subarachnoid space. In 23 patients with cervical spinal cord injuries, metrizamide myelography was performed via a C1-2 puncture. Myelography was used only for localization of the lesion and to determine the site of CT scanning. …

The authors describe five cases of an uncommon complication after Dimer X Myelography consisting in a more or less complete lesion of the caudia equina. Up to the present only seven similar cases have been reported in the literature. The evolution and the clinical symptoms of these cases were similar to those reported by others. While in the cases presented the clinical outcome was favourable, with complete recovery in all cases, less favourable evolutions with definitive neurological symptoms are described by others.

A case is presented of convulsions, subarachnoid haemorrhage and death following the use of meglumine iothalamate 280 for myelography. The patient was suffering from sciatica but had no pre-existing symptoms of other neurological disease. This case dramatically illustrates the dangers inherent in the use of meglumine iothalamate 280 for myelography.

The use of water soluble contrast medium for myelography in lumbar spinal stenosis is illustrated in 4 typical cases. It is particularly useful in demonstrating lateral types of nerve root entrapment.

Iatrogenic dural tear is usually recognized during the surgery. We describe a rare case of unrecognized dural tear caused by percutaneous endoscopic lumbar surgery at another hospital clearly confirmed with dynamic myelography. Although magnetic resonance imaging of the lumbar spine showed no obvious fluid collection suggesting dural tear, dynamic myelography revealed leakage of intradural subarachnoid contrast medium along root sleeve into the intervertebral disc space. In the setting of endoscopic spine surgery, incidental dural tear might be overlooked due to the narrow and fluid-filled sur…

Dynamic CT myelography is used to precisely localize fast spinal CSF leaks. The procedure is most commonly performed in the prone position, which successfully localizes most fast ventral leaks. We have recently encountered a small subset of patients in whom prone dynamic CT myelography is unsuccessful in localizing leaks. We sought to determine the added value of lateral decubitus dynamic CT myelography, which is occasionally attempted in our practice, in localizing the leak after failed prone dynamic CT myelography. We retrospectively identified 6 patients who underwent lateral decubitus dyna…

Hypertrophic pachymeningitis (HP) is a rare inflammatory disease that causes the thickening of the dura mater. Its etiology is mainly classified as idiopathic or secondary, and autoimmune disease is one of the main causes of secondary HP. Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis and IgG4-related disease are common among autoimmune diseases. Here we present a case of spinal HP in which the patient showed spinal shock and neurological symptoms deteriorated after myelography. Since the patient was sedated without caution of the neck posture, the HP itself compressed the sp…

INTRODUCTION: The objective of this study was to demonstrate that functional, quantitative magnetic resonance myelography (MRM) allows standardized diagnosis of symptomatic lumbar spinal stenoses which show at least equal detectability compared to functional myelography and postmyelographic CT (pmCT) based on intra- and postoperative findings.
METHODS: We investigated 43 volunteers and 47 patients with symptomatic lumbar spinal stenoses using MRM in normal position as well as in flexion and extension in a standard whole-body MR scanner. Twenty volunteers were additionally examined under axial…

BACKGROUND AND PURPOSE: Recently, the performance of C1-2 punctures for cervical myelography was challenged in a medicolegal proceeding as being below the standard of care. We sought to examine current neuroradiologic practices and opinions on the technique.
MATERIALS AND METHODS: An 11-question survey was sent to 120 program directors of neuroradiology via e-mail links regarding cervical myelography using a C1-2 puncture. Reminders were sent during a 2-month period before data were finalized.
RESULTS: Eighty-five of 120 (71%) surveys were returned. In the previous year, 14.3% (12/85) of ins…

Magnetic resonance imaging has many advantages compared with myelography and/or computed tomography in evaluating the lumbar spine for herniated nucleus pulposus. The authors have included a series of three patients whose histories and physical examinations were clinically suggestive of herniated nucleus pulposus but whose magnetic resonance imaging scans were interpreted by a radiologist as a disc bulge without nerve root compression. Because all patients had not responded to a conservative care treatment program and surgical intervention was to be considered, subsequent testing with lumbar m…

Metrizamide myelography and postmyelographic computed tomography (CT) were evaluated for relative efficacy when correlated with operative findings in a series of 30 patients. Fifty-seven levels were operated on in the 30 patients with 27 patients diagnosed as having diffuse cervical stenosis, hyperostotic spondylosis, or herniated nucleus pulposus. Metrizamide myelography and CT metrizamide myelography were equally useful in providing preoperative diagnostic information at 44 of 57 levels. Both radiographic techniques agreed with the degree of canal and neural forman stenosis found operatively…

The side effects in myelography are well known and frequently observed. The most common are headache, nausea, and vomiting. In this study, a rather new compound, Thiorphan, was examined, which displays an antinociceptive activity by inhibiting enkephalinase activity. Forty-two patients received intravenous infusions of Thiorphan before myelography. Another 42 patients were in a control group, and Thiorphan was not administered. In the treated group, postmyelographic headache was found in 24% (versus 52% in the control group). Nausea and vomiting were never seen. Low back pain or sciatica was d…

To assess the diagnostic accuracy of CT scanning in lumbar stenosis, twenty-nine patients in whom water-soluble myelography showed degenerative bony encroachment of suggested developmental narrowing of the spinal canal were scanned at all lumbar levels, and CT images were compared with myelographic findings. In developmental stenosis computerised tomography was found to be less accurate than water-soluble myelography, whereas in most cases of degenerative stenosis CT findings correlated well with myelographic patterns. An intermediate accuracy index was found in combined stenosis. In central d…

Computed tomographic metrizamide myelography (CTMM) was performed on 25 infants and children and 2 adults with normal spinal cords. Both the cord and the cauda equina were precisely outlined. The most detailed information was obtained with a small window setting, with the image subsequently magnified and color-reversed. Hounsfield-unit measurements alone were inaccurate. Advantages of CTMM include: high accuracy in demonstrating the intrathecal contents of the spine; less need for general anesthesia; and the need for a smaller amount of water-soluble contrast material than in conventional myel…

Computed tomographic metrizamide myelography (CTMM) has developed into a highly sophisticated and accurate neuroradiologic technique in the diagnosis of childhood spinal dysrhaptic states. Of the 84 patients studied by this technique, 31 with the dysrhaphic spine also had lesions such as a tethered and low conus, diastematomyelia, or neuroenteric cysts. We believe that the exquisite pathologic anatomy obtained by CTMM obviates all neuroradiologic techniques other than a preliminary anteroposterior and lateral spinal roentgenograph.
