Traumatic vertebral artery dissection associated with cervical neck traction devices.

Cervical arterial dissection (CAD) is a common cause of stroke in young people which can be classified as either spontaneous or traumatic. The primary initial symptoms are headache, neck pain, and dizziness. Recently, a 57-year-old woman experienced a severe headache after using a cervical neck traction device. Radiological examination of the head and neck revealed right vertebral artery dissection, which emphasizes the importance of recognizing that using cervical neck traction devices increases the risk of traumatic vertebral artery dissection.

Dual spinal accessory nerve: an anatomical anomaly during neck dissection.

The spinal accessory nerve (SAN) is an important cranial nerve encountered during neck dissection. Preservation of this nerve from iatrogenic damage is crucial to avoid debilitating sequalae, which can be made challenging due to variation of its anatomical course. In this case report, we present a patient who underwent supraomohyoid neck dissection, where a rare variation of a dual SAN, traversing the internal jugular vein midway, was encountered. In this case report, we study this anatomical finding, which is undoubtedly a valuable addition to the existing knowledge of the SAN. Ultimately, al…

Analysis of Surgical Outcomes in Endoscopic-Assisted Lateral Neck Dissection.

BACKGROUND: Currently, endoscopic-assisted lateral neck dissection (EALND) is performed to reduce postoperative scarring of the anterior neck. This study aimed to compare surgical outcomes between EALND and conventional open lateral neck dissection (COLND) for papillary thyroid carcinoma (PTC) with lateral lymph node metastases (LNM). METHODS: The study reviewed 103 PTC patients who were classified on the basis of surgical type and statistically compared using propensity score matching analysis. RESULTS: The mean operation time is longer, and the cost of surgery is higher in the EALND group …

Clavicle fracture following neck dissection: imaging features and natural course.

OBJECTIVE: This study aimed to assess the imaging features and natural course of clavicle fracture following neck dissection (ND). METHODS AND MATERIALS: Eight patients with clavicle fracture following ND were included. Because bilateral clavicle fractures occurred in one patient, a total of nine clavicle fractures following ND were assessed using CT images. Pathological fracture due to bone metastasis or local recurrence was carefully ruled out. RESULTS: The time interval from ND to clavicle fracture ranged from 2 to 8 months (median, 4 months). On CT images, all the nine fractures occurred…

Robotic-assisted neck dissection in submandibular gland cancer: preliminary report.

PURPOSE: Robot-assisted neck dissection (ND) in patients with head and neck cancer has been developed as a plausible substitute for conventional surgery and it provides an excellent cosmetic outcome. The authors hypothesized that surgery for submandibular gland (SMG) cancer could be achieved with a gasless retroauricular (RA) approach using the robotic system. This study evaluated the feasibility of robot-assisted ND using an RA approach for SMG cancer. PATIENTS AND METHODS: Primary tumor resection with robot-assisted ND using an RA incision was performed in 6 patients with SMG cancer. RESUL…

[The painflu stiff shoulder following radical neck dissection (author's transl)].

72 patients following radical neck dissection were studied. The accessory nerve was sacrificed in 60 patients and preserved in the remaining patients (for a control group). The most important clinical findings in all patients are reported. Most patients with loss of the accessory nerve complained more of postoperative pain and limitation of shoulder movement than of other clinical difficulties or cosmetic defects.

Managing regional metastasis in patients with cutaneous head and neck melanoma - is selective neck dissection appropriate?

BACKGROUND: Neck dissection is recommended for patients with head and neck cutaneous melanoma and nodal metastasis. However, there appears to be no clear evidence to guide the extent of nodal resection. METHODS: Loco-regional recurrence (LR), overall survival (OS) and progression free survival (PFS) was retrospectively compared between patients who had Comprehensive neck dissection (CND) and Selective neck dissection (SND). RESULTS: There was no difference in LR, OS and PFS between CND (n=18) and SND groups (n=79). Extra capsular extension (ECE), frontal disease and increasing number of invo…

Scapular muscle exercises following neck dissection surgery for head and neck cancer: a comparative electromyographic study.

BACKGROUND: Shoulder pain and dysfunction can occur following neck dissection surgery for cancer. These conditions often are due to accessory nerve injury. Such an injury leads to trapezius muscle weakness, which, in turn, alters scapular biomechanics. OBJECTIVE: The aim of this study was to assess which strengthening exercises incur the highest dynamic activity of affected trapezius and accessory scapular muscles in patients with accessory nerve dysfunction compared with their unaffected side. DESIGN: A comparative design was utilized for this study. METHODS: The study was conducted in a p…

Combined radiotherapy with planned neck dissection for small head and neck cancers with advanced cervical metastases.

BACKGROUND: We have previously described our treatment algorithm for patients with small head and neck cancers with advanced cervical metastases (stage N2 or greater). Primary radiotherapy is given to the primary site and neck, followed 6 weeks later with endoscopy and biopsy of the primary site. If biopsy of the primary site is negative by frozen section, an immediate neck dissection is performed even when no clinical residual neck disease is present. Our initial review found that 36% of patients with a complete clinical response to radiotherapy had positive nodes on histological examination.…

Recurrence of laryngeal or hypopharyngeal primary tumor after radical neck dissection for postradiotherapy neck nodal metastases.

BACKGROUND: When the primary tumor in the larynx or hypopharynx responds well to radiotherapy, nodal metastasis in the neck may persist or recur. The primary site is usually left alone when no lesion is evident, but its long-term control is uncertain. METHODS: Radical neck dissection was performed in 23 patients who had nodal metastasis, while the primary tumor in the larynx or hypopharynx was controlled after radiotherapy. The primary site was left alone and kept under surveillance. RESULTS: Seven patients (30%) had recurrence at the primary site on follow-up. Two significant risk factors f…

Intermediate Grade Salivary Gland Mucoepidermoid Carcinoma: Is Neck Dissection Indicated?

OBJECTIVE: NCCN guidelines recommend a neck dissection addressing at least levels II-IV for high-grade mucoepidermoid carcinoma (MEC) and close observation of the lymphatic basins for low-grade MEC. However, no guidelines exist for intermediate-grade MEC with clinically and radiologically uninvolved cervical lymph nodes. STUDY DESIGN: Retrospective analysis. SETTING: Patients with intermediate-grade MEC with a clinically N0 neck from our tertiary academic institution from 2015 to 2023. METHODS: Evaluation for histologic lymphatic metastases was performed when surgeons elected to perform nec…
