The neuroma map: A systematic review of the anatomic distribution, etiologies, and surgical treatment of painful traumatic neuromas.

BACKGROUND: This study analyzed all reported cases of painful traumatic neuromas to better understand their anatomic distribution, etiologies, and surgical treatment. METHODS: PubMed, Embase, Cochrane, and Web of Science were searched in October 2023 for articles describing painful traumatic neuromas. RESULTS: In total, 414 articles reporting 5,562 neuromas were included and categorized into head/neck, trunk, upper extremity, lower extremity, and autonomic nerves. Distribution was as follows: Head/neck: 83 articles reported on 393 neuromas (93.2% iatrogenic) most frequently involving the lin…

[The surgical treatment of peripheral neuromas].

Peripheral neuromas are a prevalent problem following nerve injury or certain surgical interventions like limb amputation. It is important to consider a peripheral neuroma when a patient experiences pain in the innervation area of a peripheral sensory or mixed nerve (branch), especially following trauma or amputation. Adequate recognition of a painful neuroma is crucial to treat patients satisfactorily for their invalidating and chronic symptoms. We want to emphasize that surgical intervention can be an effective and permanent treatment for symptomatic neuromas. The standard surgical treatment…

[Acoustic neuromas : Differential diagnoses].

CLINICAL ISSUE: Acoustic neuromas in adults are the most common infratentorial tumors that originate from the Schwann's cells of the vestibular part of the eighth cranial nerve and are clinically noticeable through neurological deficits, such as unilateral hypoacusis, tinnitus, dizziness and unilateral facial nerve palsy. Due to the widespread use of computed tomography (CT) and magnetic resonance imaging (MRI), acoustic neuromas can now be diagnosed with a high rate of sensitivity and specificity; however, there are a number of possible differential diagnoses, such as meningioma, epidermoid a…

Neuromas at the castration site in geldings.

BACKGROUND: Inguinal pain, unexplained hind limb lameness, back pain or behavioural problems in geldings could be attributable to painful neuromas that develop as a consequence of crushing and severing the testicular nerves during castration. The presence of neuroma in this anatomical location has never been reported, hence the knowledge of possible clinical relevance is limited. The aim of this study was to histologically investigate the testicular nerves at the castration site in geldings for the presence of neuromas. Proximal spermatic cord remnants were collected from 20 geldings admitted …

[Treatment of giant acoustic neuromas].

INTRODUCTION AND OBJECTIVES: To analyze the treatment modality and outcome of a series of patients with giant acoustic neuromas, a particular type of tumour characterised by their size (extracanalicular diameter of 4cm or more) and high morbidity and mortality. MATERIALS AND METHODS: This was a retrospective unicentre study of patients with acoustic neuromas treated in a period of 12 years. In our institutional series of 108 acoustic neuromas operated on during that period, we found 13 (12%) cases of giant acoustic neuromas. We reviewed the available data of these cases, including presentatio…

The Diagnosis and Treatment of Joplin's Neuroma.

Joplin's neuroma is a rare perineurial fibrosis of the medial plantar digital proper nerve arising from various etiologies but most commonly after bunion surgery. Treatment for this painful great toe problem remains controversial. It is our purpose to describe our experience with this chronic pain problem, considering it to be a neuroma requiring resection. A retrospective medical record review of 8 patients with medial hallux pain related to the digital nerve was performed. Each patient had failed to respond to >6 months of nonoperative therapy. At surgery, the medial digital nerve to the hal…

The painful neuroma and the use of conduits.

Treatment of neuromas in the foot and ankle is evolving. A paucity of studies deals with neuromas in this region; most knowledge comes from hand surgery. A trend toward reconstructive surgery using nerve grafts and conduits for nerves with critical function is being seen. For noncritical nerves, generally accepted treatment is neuroma resection and burial into a tissue bed. A clear knowledge of neural anatomy is paramount, together with correct identification of all the nerves involved in the pain-generation process. More studies dealing with neuromas in this area are needed for evidence-based…

Neuromas of the hand and upper extremity.

The painful neuroma is an often debilitating sequela of nerve injury about the hand. The exact pathophysiology of this condition is poorly understood. After sharp trauma to a peripheral nerve, as nerve ends try to connect with their end organs and "find" the distal nerve stump, fascicular escape and scarring can lead to the development of a painful neuroma. Painful neuromas can even be associated with blunt trauma or retraction of a nerve when the nerve is not actually divided. Green's definition of a neuroma is "the inevitable, unavoidable, and biologic response of the proximal stump after it…

[Neuromas of the extrahepatic bile ducts].

Authors collected three cases of neuromas situated in the wall of extrahepatic bile ducts, which occluded lumen and caused jaundice. In two cases without any evidence of previous surgery or cholelithiasis, neuroma was localized in ductus choledochus. After excision of ductus choledochus and cholecystectomy, we performed hepaticojejunostomy Roux--en Y. In the last patient, 7 years after cholecystectomy, neuroma localized in hepatic duct was treated by T-tube. In all patients with mechanical jaundice, before operation we had never correctly diagnosed this very rare cause of obstruction of extrah…

Incidental discovery of acoustic neuromas.

Our objective is to report 4 cases of incidentally discovered acoustic neuromas (ANs) and to determine the incidence of asymptomatic ANs. A prospective study of 161 consecutive patients undergoing gadolinium-enhanced MRI (Gd-MRI) at a tertiary-care university-affiliated medical center was carried out from September 1994 to April 1995. The Gd-MRI scans were performed for tentative diagnoses other than AN or sensorineural hearing loss. In 161 consecutive patients examined, no ANs were found incidentally on Gd-MRI scans. Previous studies have suggested that the incidence of occult ANs is as high …

Radiologic surveillance of acoustic neuromas.

OBJECTIVE: This study aimed to define the incidence and rate of tumor growth of acoustic neuromas (ANs) in patients who have undergone radiologic surveillance. DATA SOURCES: MEDLINE literature searches covering the period of January 1966-June 1997 were performed as well as a review of the bibliographies of the studies that were found. STUDY SELECTION: Criteria for inclusion of a study in this metaanalysis were: a defined group of patients diagnosed with an AN for whom radiologic surveillance was the selected strategy of management, limited inclusion of patients with neurofibromatosis type II…

Delays in the diagnosis of acoustic neuromas.

From the medical files of 164 consecutive patients who underwent surgical treatment for a unilateral acoustic neuroma between 1980 and 1992, we collected data on the delay until the diagnosis was made. A distinction was made between the patient's and general practitioner's delay (delay 1) and the delay after the specialist's first visit until the radiologic diagnosis (delay 2). The average delay was 35.7 months (SD, 62.2) for delay 1 and 15.2 months (SD, 36.3) for delay 2. Specialist's delay (otolaryngologist or neurologist) was divided into a delay of a maximum of 12 months (134 patients) and…

Neuroma of the vulva. A case report.

Even in the definitive textbook on vulvar conditions edited by Ridley no description of neuroma of the vulva exists, although neural tumors and neurofibroma are considered. The medical literature covering the past 25 years records only one case of terminal fiber neuroma in an episiotomy scar and one neuroma of the clitoris. The latter occurred in a patient with neurofibromatosis. This report documents a symptomatic neuroma of the labia minora considered to be the result of a vulval injury. It is postulated that dyspareunia related to episiotomy may be associated with disorganized proliferation…
