Propofol is a lipid-based sedative that provides 1.1 kcal/mL. Because propofol has rapid onset and quick recovery, it is becoming used widely in critical care units. A 15-year-old critically ill pregnant patient received specialized nutrition support concomitantly with propofol infusion for sedation. A serum triglyceride concentration obtained on day 6 of the propofol infusion was 1100 mg/dL with no previous history of hyperlipidemia. Caloric intakes from propofol averaged 1275 kcal/d (range 445 to 2354 kcal/d) over a 5-day period. Infusion of propofol or any other lipid-based drug must be mon

This study was performed to identify and document the benefits associated with a multidisciplinary consult-based nutritional support team. The adequacy of nutritional support and incidence of metabolic complications were prospectively studied in 28 adult patients receiving total parenteral nutrition in a large private tertiary care institution during a 2-week period. Estimated daily caloric and protein requirements were met significantly more often in patients followed by our NST than in control group patients. Chloride and bilirubin abnormalities occurred significantly less often in the NST g…

BACKGROUND: Gastrointestinal surgery is a complicated process used to treat many gastrointestinal diseases, and it is associated with a large trauma: Most patients often have different degrees of malnutrition and immune dysfunction before surgery and are prone to various infectious complications during postoperative recovery, thus affecting the efficacy of surgical treatment. Therefore, early postoperative nutritional support can provide essential nutritional supply, restore the intestinal barrier and reduce complication occurrence. However, different studies have shown different conclusions.

Oral nutritional support (ONS) is a form of supportive intervention in patients' diet in response to insufficient oral intake, malabsorption, or functional insufficient food intake during the disease process. This study aimed to evaluate the nutritional status, morbidity, re-admissions, functional status, QoL of patients who had undergone elective colorectal cancer surgery and initiated home ONS. A total of 144 patients who had undergone colorectal cancer surgery and given home ONS were analyzed with regard to demographics, diagnosis, weight-BMI changes, re-admissions, morbidity, daily caloric…

Nutritional problems often result from malignancies and aggressive multimodal treatment. Early identification of reliable risk factors associated with malnutrition and need for nutritional support is necessary for development of preventative approaches. Nutritional and treatment-related characteristics were examined for 173 pediatric oncology patients referred for nutritional support and a comparison sample of 43 patients matched on treatment protocol and/or diagnosis who had never been referred for nutritional support. Abnormally low serum albumin levels, poor oral intake, mucositis, prior ra…

PURPOSE OF REVIEW: Several observational studies have pointed out the negative effects of an energy deficit in long-staying ICU patients. Morbidity as well as mortality seem to be linked to such an energy deficit, which is often built up during the first week of stay. A combination of enteral and parenteral nutrition may be a strategy to avoid the development of an energy deficit.
RECENT FINDINGS: A cumulated energy deficit is demonstrated to be connected to an increased infectious morbidity. The question is whether or not addition of parenteral nutrition to the enteral nutrition may prevent

BACKGROUND: The effectiveness of Nutrition Support Services in optimizing parenteral nutrition has not been evaluated since the 1980s.
METHODS: We prospectively monitored medical and surgical patients on total parenteral nutrition (TPN) in a university hospital who did not receive Nutrition Support Service recommendations to compare the incidence of metabolic complications in 1979 (group 1, n = 100) with that in 1992 (group 2, n = 106). The Service provided automatic recommendations on a subsequent group of medical service patients (group 3, n = 128) and compared them with the patients in gro

Most of the studies that have examined the effect of nutrition support teams (NST) on the quality of total parenteral nutrition (TPN) have found reductions in the frequency of metabolic and central venous catheter related complications. Previous studies have not examined the patterns of nutrient delivery (eg, calories and protein) when TPN is provided either by a primary physician or by a NST. We compared the quality of TPN care provided by our NST or by primary physicians utilizing nonspecialized hospital personnel and resources. As expected, catheter complications were significantly less fre

AIMS: To report on peri-operative nutritional status in gastric cancer patients undergoing total gastrectomy, and to examine the role of post-operative parenteral nutrition.
METHODS: Retrospective study of prospectively collected data on 90 consecutive patients who underwent total gastrectomy for malignancy.
RESULTS: At diagnosis 46% of patients reported clinically severe weight loss, and dietary intake was inadequate in 72% of patients. Post-operatively 42% were given total parenteral nutrition (TPN) and 53% were given intravenous fluids (IVF) alone. TPN patients spent a mean of 13.6 days o

The gastrointestinal system alongside nutritional support, the endocrine system, and the hematological system has integral roles in sepsis. Understanding these systems can help clinicians better manage patients with sepsis and potentially improve outcomes. This chapter details these 3 important organ systems and, in particular, the clinical impact on each organ system in sepsis.

Malnutrition, sarcopenia, and obesity can negatively impact the course of gastrointestinal surgery, increasing the risk of postoperative com … ne surgery. This article discusses the importance of nutritional status screening and perioperative nutritional support, emphasizing the need for a comprehensive approach to improve patients' quality of life and reduce postoperative complications.

Sarcopenia is a specific disease that commonly occurs in the elderly and patients with consumptive diseases. Sarcopenia can cause a severe decline in strength and movement, and even lead to prolonged bed rest. Nutritional support is an emerging approach to mitigate sarcopenia and is currently the gold standard for clinical management of sarcopenia. However, complex nutritional composition may affect the effect of treatment. Herein, the pathogenesis of sarcopenia is first introduced. Then, the diagnostic methods of sarcopenia are described in detail, including grip strength, chair stand test, d…

Intestinal failure is a syndrome characterized by a diminished intestinal function that is inadequate to maintain normal digestion and absorption, leading to systemic metabolic disorder and requiring long-term nutritional supplementation to sustain health and growth. Short bowel syndrome (SBS) is one of the primary causes of intestinal failure. Given the significant differences among SBS patients, nutritional treatment strategies should emphasize individualization. This review focuses on SBS, combining its anatomical and pathological characteristics, to introduce nutritional support treatment …

OBJECTIVE: To evaluate the clinical and rehabilitation effectiveness of the protein module (Fresubin Protein) in therapeutic nutrition in pa … atory process parameters (CRP, white blood cell count, r=-0.79, p=0.001). Against the background of nutritional support with the inclusion of a protein module in the diet, by the 21st day, patients of the main group showed a reliable (p<0.05) normalization of protein metabolism parameters, an increase in lean body mass.
CONCLUSION: The results of the studies indicat …

Patients in chronic critical illness after brain injury may experience various conditions that limit nutritional support. The aim of this work is to analyze the factors limiting or hindering the provision of nutritional support (in particular, enteral clinical nutrition) in patients in chronic critical illness after brain damage. Material and methods. This study is a prospective observational cohort study. In the course of the study, 47 patients (27 men and 20 women, mean age 52±18.4 years) in a chronic critical illness who were undergoing rehabilitation after brain damage and who had certain …

Hepatobiliary complications of hypoalimentation and parenteral nutrition (PN) are widely recognised. Hypoalimentation includes conditions such as anorexia nervosa (AN), obesity malnutrition and liver disease following bariatric surgery. Treatment of the underlying condition causing hypoalimentation can result in an improvement in liver dysfunction. Liver function test abnormalities are also commonly found in patients on PN, with the three main complications being steatosis, cholestasis and biliary system sludge/stones. Patients with intestinal failure receiving PN often have multiple possible
