Octreotide Administration Methods in Sulfonylurea Poisoning: A Retrospective Chart Review.

INTRODUCTION: Octreotide is used to treat sulfonylurea induced hypoglycemia, but the effectiveness of intravenous bolus, intravenous infusion, and subcutaneous routes of octreotide administration has not been compared. METHODS: This is a multi-center, retrospective chart review of patients with suspected sulfonylurea induced hypoglycemia who received octreotide, comparing intravenous bolus, intravenous infusion, and/or subcutaneous routes of octreotide administration. The primary outcome was number of hypoglycemic events after octreotide initiation. Secondary outcomes included total octreotid

Octreotide for long-lasting malignant bowel obstruction: two case reports.

BACKGROUND: Efficacy of the combination of octreotide and other drugs for the management of malignant bowel obstruction (MBO) has been well described. However, long-lasting stages with lack of stool emission are a challenging clinical condition of MBO that have never described. CASE DESCRIPTION: We describe two cases in which the addition of octreotide to supportive care measures, even given late after more than 3 weeks of no stool emission, resulted to be still effective in recovering the bowel transit. In the first case, a patient admitted to home palliative care had a nasogastric tube and

Incidental Finding of Benign Trichilemmal Cysts on 99mTc-Octreotide Scan.

A 63-year-old woman with neuroendocrine carcinoma of the colon was referred to our department for staging with Tc-octreotide scan. Two focal, highly intense, superficial lesions were detected in patient's scalp and paralumbar region, medial to the right kidney. Pathology correlation was recommended, revealing them both to be benign trichilemmal cysts. This unexpected finding strengthens the hypothesis of Merkel cell carcinoma and trichilemmal cysts originating from pluripotent stem cells of adnexal epithelium, making a neuroendocrine differentiation for the latter plausible.

Metastatic Adrenal Cortical Carcinoma Responding to Octreotide: A Case Report.

Advanced adrenocortical carcinoma (ACC) is an aggressive disease with poor prognosis, and the current therapeutic options, such as mitotane or platinum-based chemotherapy regimens, often offer limited efficacy. Here, we present the first report, to the author's knowledge, of metastatic ACC with positive octreoscan scintigraphy that was successfully treated with octreotide long-acting release (LAR). A patient with metastatic ACC who showed poor tolerance to mitotane received octreotide LAR because of positive octreoscan scintigraphy. She obtained major partial response to the somatostatin analo

Impact of preoperative pasireotide therapy on invasive octreotide-resistant acromegaly.

A 43-year-old woman with an 8-year history of diabetes, hypertension, and dyslipidemia presented with amenorrhea and convulsion. Her MRI scan revealed a 3.5-cm T2-hyperintense pituitary macroadenoma with suprasellar extension to the frontal lobe and bilateral cavernous sinus invasion. Her serum levels of GH and insulin-like growth factor-I (IGF-I) were elevated to 9.08 ng/mL (normal range: <2.1 ng/mL) and 1,000 ng/mL (normal range: 90-233 ng/mL, SD score +10.6), respectively. Bromocriptine insufficiently suppressed her GH levels, while octreotide paradoxically increased her GH levels. Together

Severe hyperkalaemia resulting from octreotide use in a haemodialysis patient.

A 48-year-old man, with end-stage renal disease (ESRD) on haemodialysis, presented to the emergency department with sulphonylurea-induced hypoglycaemia. His hypoglycaemia was persistent despite glucose infusion, so he was treated with octreotide. Octreotide administration reversed the hypoglycaemia but also resulted in severe hyperkalaemia. The patient was urgently dialysed, the octreotide was discontinued and his potassium normalized. We believe that the hyperkalaemia was caused by octreotide-induced insulin suppression and resultant impaired cellular potassium uptake. Although octreotide may

Peptide receptor radionuclide therapy in vitro using [111In-DTPA0]octreotide.

UNLABELLED: Peptide receptor radionuclide therapy (PRRT) using [(111)In-DTPA(0)]octreotide (where DTPA is diethylenetriaminepentaacetic acid) is feasible because, besides gamma-radiation, (111)In emits both therapeutic Auger and internal conversion electrons having a tissue penetration of 0.02-10 and 200-500 micro m, respectively. The aim of this study was to investigate the therapeutic effects of [(111)In-DTPA(0)]octreotide in a single-cell model including the effects of incubation time, radiation dose, and specific activity of [(111)In-DTPA(0)]octreotide. Finally, we discriminated between th

Octreotide in acute bleeding esophageal varices: a prospective randomized study.

BACKGROUND/AIMS: To assess the value of octreotide in the control of acute bleeding esophageal varices, in a prospective randomized study. METHODOLOGY: One hundred and ninety-seven patients admitted for variceal bleeding confirmed at endoscopy were recruited and divided into two groups: group I (n = 111) with endoscopic stigmata of recent bleeding; and group II (n = 86) with active bleeding at emergency endoscopy. Patients in group I were randomized to receive a continuous infusion of octreotide (n = 58) or emergency sclerotherapy (n = 53). Patients in group II were assigned to sclerotherapy

Cardiac pheochromocytoma: resection after diagnosis by 111-indium octreotide scan.

Cardiac pheochromocytoma is an exceedingly rare and unusual clinical entity. Only 37 previous surgically treated adult patients were found in review of the surgical literature. We report the case of a 13-year-old boy who had a cardiac pheochromocytoma that was localized by the 111-indium diethylenetriamine pentaacetic acid octreotide scintigraphy scan and confirmed by magnetic resonance imaging after computed tomographic and B1-iodine-metaiodobenzylguanidine scans had failed. At operation, a 6-cm pheochromocytoma of the left atrium was found and successfully resected with reconstruction of the

Subcutaneous cavernous hemangioma visualized on an indium-111-octreotide scan.

Indium-111-octreotide, a somatostatin analog is the first commercially available receptor-based agent. It shares the binding site of naturally occurring somatostatin and is suitable for imaging somatostatin receptor-bearing tumors. An 111In-octreotide scan performed on a 6-wk old girl showed increased uptake in a subcutaneous cavernous hemangioma, which should be added to the list of lesions that can be visualized on an octreotide scan.

Secretory symptoms from metastatic adrenal cortical carcinoma responding to octreotide.

The prognosis of patients with metastatic adrenal cortical carcinoma is poor, and their disabling symptoms are usually unresponsive to conventional therapy. A patient with Cushing's syndrome secondary to a secretory adrenal cortical carcinoma was treated with octreotide, endocrine therapy and chemotherapy having failed. Treatment led to a dramatic relief of her symptoms with a fall in corticosteroid secretion. Somatostatin analogue therapy for this tumour should be encouraged in view of the lack of alternative palliative treatment.

Sigmoid volvulus presenting as chronic secretory diarrhea responsive to octreotide.

Secretory diarrhea can be seen in a variety of pathologic states; however, intermittent colonic obstruction usually is not considered as a possible cause. We report a 68-yr-old patient with chronic secretory diarrhea and hypokalemia due to intermittent sigmoid volvulus. Because the volvulus was not originally diagnosed, the patient was treated with the long-acting somatostatin analogue octreotide for 1 yr, with marked clinical improvement. Surgical resection of the redundant sigmoid responsible for the volvulus resulted in prompt and complete resolution of all signs and symptoms. Detailed macr

[Somatostatin analog (octreotide) in clinical use: current and potential indications].

The long-acting somatostatin analogue octreotide is a synthetic cyclic peptide consisting of 8 amino acids. Depending on the organ, it acts either as a hormone or as a neurotransmitter. The effect on various physiological functions in the brain and the gastrointestinal tract is mainly inhibitory. Due to its inhibitory actions, the possibility of intravenous and subcutaneous administration and the lack of serious side-effects, octreotide offers a broad spectrum of possible indications. Today octreotide is recommended in acromegaly patients and for the treatment of hormone dependent symptoms in

Potential gastrointestinal uses of somatostain and its synthetic analogue octreotide.

Octreotide is a long-acting analogue of somatostatin which has been FDA approved for symptomatic carcinoid syndrome and for vasoactive intestinal peptide-producing tumors. Somatostatin and octreotide have significant effects on gastrointestinal physiology which may be beneficial for a variety of disorders. This paper will review the currently available literature on the use of somatostatin and octreotide in non-neoplastic disorders of the gastrointestinal tract.

Paltusotine versus octreotide: different effects on radioligand uptake in neuroendocrine tumours.

OBJECTIVE: Somatostatin receptor analogues are well-established in the treatment of metastatic gastro-enteropancreatic neuroendocrine tumours (GEP-NETs), especially for symptom control in patients with the carcinoid syndrome, and to control tumour growth. However, they need to be discontinued before peptide receptor radionuclide therapy (PRRT) as they may saturate the somatostatin receptor 2 (SSTR2) and prevent binding of the radioactive ligand. DESIGN: We evaluated the effects of the novel somatostatin analogue paltusotine on 18F-SiTATE radioligand uptake and on GEP-NET cell viability in com

Ménétrier's disease : a case of successful treatment using long-acting octreotide.

The authors describe a 31 years old male, admitted for hematemesis, epigastric pain and lower limb edema. Laboratorial data showed haemoglobin 18.4g/dl, total proteins 2.8g/dl, albumin 1.6g/dl and hipogammaglobulinaemia. 24h urinary proteins were normal. HIV and CMV serology were negative. Upper GI endoscopy revealed markedly enlarged gastric folds covered by abundant exudative fluid. Endoscopic ultrasound showed ascites, pleural effusion and gastric wall thickening with mucosa expansion and intact submucosa. In gastric biopsies foveolar hyperplastic and regenerative mucosa were observed being

Octreotide therapy and restricted fetal growth: pregnancy in familial hyperinsulinemic hypoglycemia.

SUMMARY: Hypoglycemia during pregnancy can have serious health implications for both mother and fetus. Although not generally recommended in pregnancy, synthetic somatostatin analogues are used for the management of blood glucose levels in expectant hyperinsulinemic mothers. Recent reports suggest that octreotide treatment in pregnancy, as well as hypoglycemia in itself, may pose a risk of fetal growth restriction. During pregnancy, management of blood glucose levels in familial hyperinsulinemic hypoglycemia thus forms a medical dilemma. We report on pregnancy outcomes in a woman with symptoma
