[Wernekink Commissure Syndrome With Hallucinations and Involuntary Groping:Report of One Case].

Wernekink commissure syndrome is a rare midbrain syndrome with bilateral cerebellar dysfunction, eye movement disorder, and palatal myoclonus.Few cases of this syndrome have been reported in China, let alone those combined with hallucinations and involuntary groping. This paper reports the diagnosis and treatment of a case of Wernekink commissure syndrome with hallucinations and involuntary groping, aiming to enrich the knowledge about this disease for clinicians.

Infranuclear Eye Movement Disorders.

Infranuclear motility disorders are such of the cranial nerves, the extraocular muscles or changes in the orbit but definitely peripheral to the nuclei of the cranial nerves. Characteristic are movement deficits, a compensatory head posture and the pattern of incomitancy. The secondary angle of deviation is usually larger than the primary. Combined pareses suggest a lesion in the cavernous sinus, orbital apex or a multilocular event. It is essential to rule out supranuclear disorders, especially if the motility deficit is atypical. For clarification, an individual risk assessment is recommende

Near Point of Convergence Deficits and Treatment Following Concussion: A Systematic Review.

CONTEXT: Convergence dysfunction following concussion is common. Near point of convergence (NPC) is a quick and easy assessment that may detect oculomotor dysfunction such as convergence insufficiency (CI), but NPC measurements are rarely reported. Convergence dysfunction is treatable in otherwise healthy patients; the effectiveness of oculomotor therapy following concussion is unclear. OBJECTIVES: The purpose of this article was to systematically review the literature and answer the following clinical questions: (1) Is performance on NPC negatively affected in patients diagnosed with a concu

Mathematical models and human disease.

Mathematical models of brain function are built from data covering anatomy, physiology, biophysics and behavior. In almost all cases, many possible models could fit the available data. Theoreticians make assumptions that allow them to constrain the number of possible model structures. However, a model that was more useful clinically would result if the constraints came from lesion studies in animals or clinical disorders. Here, we show a few examples of how clinical disorders have led to improvements in models. We also show a few examples of how models could lead to neural prostheses for patie

Skew deviation: clinical updates for ophthalmologists.

PURPOSE OF REVIEW: This article discusses the current approach in diagnosing skew deviation, as well as recent findings in the lesion localization. RECENT FINDINGS: Skew deviation can be defined as vertical misalignment of the eyes that does not map to any of cyclovertical muscles, in association with neurologic symptoms and signs and with posterior fossa lesion. It can be differentiated from trochlear nerve palsy by the direction of ocular torsion and the change in the degree of vertical deviation with upright and supine head position. It is commonly caused by ischemia of the posterior param

[Diagnosis of vertigo: keep an eye on central eye movement disorders].

With specialist knowledge ophthalmologists can make a valuable contribution to the interdisciplinary work-up of patients with vertigo as the leading symptom. The neuro-ophthalmological examination of eye movements by an ophthalmologist and/or orthoptist is an important contribution because the various vertigo syndromes can only be correctly evaluated by a combined examination of the vestibular and ocular motor systems. If the ophthalmologist is the first doctor to examine a patient suspected disorders from other specialist fields can be indicated, in particular neurology and otorhinolaryngolog

Inferior oblique muscle palsy with "paradoxical" v-pattern strabismus.

BACKGROUND AND PURPOSE: A- and V-patterns are commonly encountered with cyclovertical muscle palsies. Inferior oblique (IO) muscle palsy produces and A-pattern caused mainly by the decreasing abducting action of the IO in upgaze and an increasing abduction action of the superior oblique in downgaze. V-pattern association with an IO palsy has not been, to our knowledge, reported before. The purpose of our paper is to report on a patient iwth IO palsy and a paradoxical V-pattern and explain the proposed pathophysiology behind the V-pattern. CASE REPORT: We report a 67 year old female with a 3 y

Superior oblique muscle palsy after frontal sinus mini-trephine.

OBJECTIVE: The aim of this study is to present a case of superior oblique muscle dysfunction after a frontal sinus mini-trephine. METHODS: This is a case report of an 18-year-old woman where a mini-trephination approach and endoscope were used to open and marsupialize a symptomatic, opacified type IV cell within the left frontal sinus. After surgery, the patient developed a persisting diplopia; a left superior oblique muscle palsy was diagnosed. Nine cadaveric dissections of the trochlea were undertaken to clarify mechanisms for potential trochlear damage. RESULTS: Cadaveric dissection revea

Acute oculogyric crisis in a patient taking clebopride.

We present a 21-year-old woman with an oculogyric crisis following the administration of clebopride. Based on the temporal relationship of the administration of clebopride to the occurrence of the event and the absence of other possible etiologies, clebopride is implicated as the cause of this acute oculogyric crisis, despite the fact that a cause-and-effect relationship cannot be confirmed.

[Saccadic eye movements in children with reading disorders].

Several reports raised the possibility that dysfunction of saccadic eye movements produces reading disorder (RD), although opposite conclusions have also been reported. In this study, saccadic eye movements were investigated in 13 forth graders with RD and 20 age-matched controls during reading and non-reading tasks. Compared to the control group, children with RD showed significantly higher incidences of both forward and backward saccadic eye movements in reading tasks, while they revealed a significantly lower number of saccadic eye movements in response to sequentially moving targets in two

Internuclear ophthalmoplegia associated with transient torsional nystagmus.

The association of internuclear ophthalmoplegia (INO) with torsional nystagmus is rare. We report the case of a 63-year-old man presenting with sudden onset of diplopia. On ocular examination, he had left INO and a torsional nystagmus with counter-clockwise quick-phases from the patient's point of view in the primary position, ipsiversive to the side of the medical longitudinal fasciculus (MLF) lesion at the pontomesencephalic junction on magnetic resonance images. Neither skew deviation nor head tilting was noted at that time. Two days later, the torsional nystagmus disappeared, although the

[Neurology of eye movements].

Eye movements are necessary for clear and stable vision, for which images of the world should be brought to the fovea and be held steady on the retina. The main types of eye movements consist of saccadic eye movements, vergence, vestibular eye movements, smooth pursuit eye movements and eye movements for visual stabilization. Saccades are fast eye movements and have consistent relationship between their peak velocity and the size of the movements. The burst neurons in the brainstem are the generator of saccades and receive projections from the frontal eye field, supplementary eye field, pariet
