Characteristics of visual field progression in patients with normal-tension glaucoma with optic disk hemorrhages.

PURPOSE: To study the characteristics of visual field progression in patients with normal-tension glaucoma (NTG) with optic disk hemorrhages. DESIGN: Observational study. METHODS: Fifty-eight eyes of 58 untreated patients with NTG who had at least five reliable visual fields of the Humphrey Field Analyzer (central 30-2) within the follow-up period of more than 24 months were enrolled. Of these, 27 eyes had optic disk hemorrhages in their clinical courses. Pointwise linear regression analysis was done using total deviation values of the fields at each of 74 test locations in each patient. Pro…

[Relationship of changes in visual functions and optic disk in patients with glaucoma concurrent with myopia].

Forty-nine myopic patients (70 eyes) aged 2 to 12 years (mean 7 +/- 4.2 years) with primary open-angle glaucoma (POAG) were examined. In patients with glaucoma concurrent with myopia, enlarged peripapillary retinal atrophy is one of the first signs of a progressive process, long optical axis in particular. Reduced visual functions were detectable in fewer cases than were changes in the optic disk and peripapillary retina. The most sensitive methods are in the following order: distant contrast discrimination (DCD), critical flicker fusion frequency (CFFF), Bjerrum's zones, statitic perimetry wi…

[Bilateral optic disk edema in polyneuropathy, organomegaly, endocrinopathy, monoclonal proteins and skin changes (POEMS syndrome)].

BACKGROUND: POEMS occurs only in about 1% of plasmocytoma cases, but in more than 50% of the rare osteosclerotic subtypes and plasma cell dyscrasias. The estimated frequency is 20 cases per year in Germany. Swelling of the optic disks is an early sign of the syndrome. CASE REPORT: An osteosclerotic plasmocytoma type IgA lambda was known in a 63-year-old farmer for 4 years. The primary lesion in the left femur was irradiated at diagnosis. Half a year prior to our examination the patient experienced edema of the legs and recurrent diarrhea. A staging confirmed the earlier diagnosis of a POEMS s…

Optical Coherence Tomography Angiography Demonstrates Strain and Volume Effects on Optic Disk and Peripapillary Vasculature Caused by Horizontal Duction.

PURPOSE: The optic nerve mechanically loads the eye during ocular rotation, thus altering the configuration of the disk and peripapillary tissues. We used optical coherence tomography (OCT) angiography (OCTA) to investigate mechanical strains and volume changes in disk and peripapillary blood vessels during horizontal duction. METHODS: Structural OCT and OCTA were performed centered on optic disks; imaging was repeated in central gaze, and in 30° ab- and adduction. By an algorithm employing point-set registration of 3 D features, we developed a novel approach for measuring disk strains, and s…

Sonography in optic disk drusen: imaging findings and role in diagnosis when funduscopic findings are normal.

OBJECTIVE: Optic disk drusen are relatively benign lesions of the eye but can cause visual symptoms and may occasionally mimic papilledema on funduscopy. We studied the range of sonographic findings and determined if sonography can be used to diagnose this condition in the absence of typical funduscopic findings. SUBJECTS AND METHODS: Sonography of the eyes was performed in 12 patients with known or suspected optic disk drusen by using a 10-MHz transducer. Optic disk drusen were diagnosed clinically in 16 of the 24 eyes. The funduscopic appearance was typical in 11 eyes and atypical in five e…
