Cryptorchidism is one of the main risk factors for infertility and testicular cancer. Orchiopexy surgery corrects cryptorchidism effects. Different models of cryptorchidism developed in the rat include surgery. A 37-year-old man was referred to our hospital with the chief complaint of infertility. Semen analysis at another clinic had demonstrated severe oligozoospermia and he had a history of bilateral orchiopexy for undescended testes. Scrotal ultrasonography revealed a hypoechoic heterogeneous mass (1.5 cm in diameter) within the left testis. With a provisional diagnosis of testicular tumor, left high orchiectomy was performed. PURPOSE: Scrotal pouch orchiopexy without suture fixation will result in strong atraumatic fixation of the testis in the management of undescended testis. We describe a similar technique that is effective in the management of testicular torsion.
MATERIALS AND METHODS: A total of 19 scrotal pouch orchiopexies was performed in 11 boys with testicular torsion. Age ranged from 10 to 17 years (mean 14). Ten boys with nonpalpable testes located in the peritoneal cavity underwent celiopelviscopy and clip ligation of the spermatic vessels with the intent of augmenting vasal collateral blood flow. After intervals in excess of 6 months, 7 patients underwent vas-based orchiopexy following division of the occluded spermatic vessels. Six boys had good results as judged by size of the relocated testes. In 1 boy the testis atrophied. He had undergone previous inguinal-only exploration before referral to us for celiopelviscopy and at stage 2 orchiopexy a silk suture was found on the vas deferens.

Extensive laboratory experiments in animals have led us to believe that autotransplantation of the testis in the human is feasible. To date, nine of our patients (aged 4-13 years) with undescended intra-abdominal testes have undergone autotransplantation. Using a microvascular surgical technique to anastomose the testicular vessels to the inferior epigastric vessels, it has been possible to place the testis in a scrotal position in every case.
