Stapes surgery: the diameter of the long process of the incus.

HYPOTHESIS: The aim of this study was to examine the long process of the incus in respect of its shape and its dimensions at the site of the attachment of a stapes prosthesis. BACKGROUND: One of the complications in stapes surgery is the erosion of the long process of the incus at the site of the attachment of the prosthesis, resulting in a fluctuating conductive hearing loss. Knowing the dimensions of the attachment site of the prosthesis at the long process of the incus will make it possible to optimize the size of the prosthesis loop. METHODS: The incus was obtained from 11 patients who h

Chronic inflammatory ear disease and cholesteatoma: creation of auxiliary attic aeration pathways by microdissection.

HYPOTHESIS: The attic compartments, except for Prussak's space, are aerated through the tympanic isthmus. The aim of this study was to develop aeration pathways that would bypass the isthmus in surgery for chronic inflammatory ear disease and cholesteatoma. BACKGROUND: Microdissection of the epitympanum has shown that the anterior attic and the supratubal recess are separated by the tensor fold, the excision of which creates a large new aeration pathway. METHODS: Earlier surgical experience was reexamined as to the access to the tensor fold. Twenty temporal bones were dissected to create cli

Comparison of Surgical Outcomes Between Ossicle-Cartilage Composite Graft and Synthetic Ossicular Prosthesis for Ossicular Reconstruction.

OBJECTIVE: To propose a modified technique of ossiculoplasty using an ossicle-cartilage composite graft (OCCG) and compare its hearing outcome and protrusion rate with those of a synthetic prosthesis. METHODS: Autologous tragal or homologous septal cartilage was combined with an ossicle to create an umbrella shape OCCG. A total of 302 ossiculoplasty surgeries performed in a single university hospital between 1997 and 2006 were retrospectively reviewed based on the prosthesis type-OCCG or polymaleinate ionomeric prosthesis (IONOS®). Data includes demographic profiles, audiometric outcomes, int

A novel computed tomography guided, transcutaneous approach to treat refractory autophony in patients with a patulous Eustachian tube - a case series.

BACKGROUND: Patulous Eustachian tube is a benign but notoriously difficult condition to treat successfully. Symptoms include autophony of voice and breathing, and aural fullness. METHODS: This paper presents a series of 8 patients (12 ears) for whom a novel computed tomography guided injection of silicone elastomer suspension implant (Vox) was used to treat patulous Eustachian tube. This is the largest and only series in the current literature using this technique. RESULTS: The combined complete and partial symptom resolution rate was 91 per cent. Complications related to the procedure are d

Osseointegrated implants in the management of childhood ear abnormalities: with particular emphasis on complications.

Little has been published about the difficulties encountered during the insertion of osseointegrated implants for the attachment of bone anchored hearing aids (BAHA) and auricular prostheses in children. This study examines this issue in the first 51 children implanted at our centre. During surgery, the most common problem encountered was the presence of thin bone resulting in incomplete insertion of fixtures. Five fixtures had failed to integrate and six fixtures were lost in the long-term, however, only five children required revision surgery. The reason why few patients require revision was

Auditory outcomes and predictors following ossiculoplasty in cholesteatoma surgery: a retrospective analysis.

INTRODUCTION: Middle ear cholesteatoma causes bone/ossicular erosion. This study aimed to analyze auditory outcomes after ossiculoplasty in cholesteatoma surgery and to identify predictors of hearing outcomes related to middle ear or surgery. METHODS: A retrospective study was conducted on patients who underwent ossiculoplasty during cholesteatoma surgery (2019-2024). Preoperative, short-term (2-month), and midterm (8-month) postoperative audiograms were analyzed. Potential pre-/postoperative parameters influencing hearing were sought. RESULTS: Eighty-eight cases were included (20 pediatric,

Congenital conductive hearing loss.

Congenital conductive hearing loss due to ossicular deformities can be treated by either rehabilitation with a hearing aid or surgical reconstruction. We present the results of exploratory tympanotomy performed in a large paediatric otolaryngology centre in 67 patients with non-serous congenital conductive hearing loss. Forty-two children had malformation of one or more ossicles without fixation of the stapes, and 19 had fixed stapes. In 26 cases, the surgeon decided not to perform surgical correction. Seven operated patients were lost to follow-up. As a group, 47 per cent of the patients who

Pediatric ossiculoplasty and implications in cholesteatoma surgery.

PURPOSE OF REVIEW: This review explores the current understanding and clinical considerations of ossiculoplasty in pediatric patients, focusing on ossicular chain reconstruction associated with congenital or acquired cholesteatoma. The goals of this review are to review the current literature on diagnostic approaches, surgical timing and techniques of pediatric ossiculoplasty. RECENT FINDINGS: Current literature suggests that pediatric cholesteatoma tends to be more aggressive than in adults, often necessitating staged ossiculoplasty for better long-term outcomes. Diffusion-weighted MRI, part

Titanium Biohybrid Middle Ear Prostheses: A Preliminary In Vitro Study.

Ossiculoplasty is a surgical operation performed to restore auditory transmission through the reconstruction of the ossicular chain using prosthetics. Tissue bioengineering has assumed a pivotal role in implementing alternatives to conventional ossicular middle ear replacement prostheses, to overcome extrusion while preserving acoustic properties. This in vitro study aims to explore, for the first time in current literature, the feasibility of a biohybrid middle ear prosthesis, composed of titanium surrounded by a bone extracellular matrix as bio-coating. We have hereby studied the adhesion an

Transposition of the facial nerve in congenital aural atresia.

It is generally recognized that surgery for congenital aural atresia is difficult. The success or failure of the operation is often directly related to the degree of development of the middle ear. In poorly developed middle ears, the facial nerve may overhang and conceal the oval window niche, making this area inaccessible to inspection, let alone manipulation. The criteria for transposing the facial nerve are; (1) the atresia must be bilateral, (2) there must be preoperative imaging evidence of a stapes and/or patent oval window, (3) there must be no large blood vessels feeding or draining th

[Surgical treatment possibilities of middle ear hearing loss].

Otological surgery is performed under the operating microscope with special microinstruments. The introduction of new technical means such as the argon laser, equipped with a fiberoptic handpiece, and the sceeter microdrill have improved the results according to the statistic of Berne. An important aspect is the approach to the structures in the temporal bone. The classical retroauricular and endaural incisions are replaced whenever possible by the less traumatic transmeatal approach directly through the external ear canal without external skin lesion. Ear drum perforations and cholesteatomas

Vibroplasty combined with tympanic membrane reconstruction in middle ear ventilation disorders.

Although the Vibrant Soundbridge is one of the most frequently used active middle ear implants, data regarding how middle ear ventilation disorders may affect the transmission behavior of its floating mass transducer are still insufficient. Studies involving coupling the floating mass transducer to the stapes head are particularly lacking. This temporal bone study evaluated the influence of simulated middle ear ventilation disorders on the middle ear transfer function in the reconstructed middle ear. The middle ear transfer function was measured using Laser Doppler Vibrometry after vibroplasty

Vibrant Soundbridge Implantation: Floating Mass Transducer Coupled with the Stapes Head and Embedded in Fat.

Subtotal petrosectomy may be performed for refractory chronic middle ear diseases, such as massive cholesteatoma or recurrent otitis media. It involves permanent obliteration of the operative cavity, thus precluding the chance to restore conductive hearing via traditional inertial ossicular prostheses. The Vibrant Soundbridge (VSB) is an alternative option for hearing rehabilitation. Vibrant energy is delivered into the inner ear via a floating mass transducer (FMT), which can be coupled with any part of the middle ear acoustic transmission structure. To restore the hearing of a young woman wi
