Value of Diagnostic Tools in the Diagnosis of Osteomyelitis: Pilot Study to Establish an Osteomyelitis Score.

Osteomyelitis (OM) remains one of the most feared complications in bone surgery and trauma. Its diagnosis remains a major challenge due to lack of guidelines. The aim of this study was to prospectively analyze the value of the most common and available diagnostic tools and to establish an OM score to derive treatment recommendations. All patients with suspected OM were included in a prospective pilot study. All patients underwent blood sampling for C-reactive protein and white blood cell count analysis. Magnetic resonance imaging (MRI), and microbiologic and histopathologic samples, were taken…

Osteomyelitis in a general hospital. A five-year study showing an increase in subacute osteomyelitis.

We have reviewed 60 patients with primary bone infections; 21 of these (35%) had subacute osteomyelitis, a figure which supports other recent observations that this variant of bone infection is becoming more widespread. In this group open culture and biopsy were necessary in order to exclude bony malignancy, and a raised erythrocyte sedimentation rate proved a useful diagnostic aid. All the patients with acute osteomyelitis or with vertebral infection responded to primary treatment, but five of those with subacute osteomyelitis had recurrences.

Classification and management strategies for paediatric chronic nonbacterial osteomyelitis and chronic recurrent multifocal osteomyelitis.

INTRODUCTION: Chronic non-bacterial osteomyelitis (CNO) is an autoinflammatory bone disease that most commonly affects children and adolescents causing significant pain and damage to bones. The absence of diagnostic criteria and biomarkers, an incomplete understanding of the molecular pathophysiology, and lack of evidence from randomized and controlled trials make the diagnosis and care challenging. AREAS COVERED: This review provides an overview of the clinical and epidemiological features of CNO and displays diagnostic challenges and how they can be addressed following strategies used inter…

Outcome of culture-negative pyogenic vertebral osteomyelitis: comparison with microbiologically confirmed pyogenic vertebral osteomyelitis.

OBJECTIVES: Although pyogenic vertebral osteomyelitis (PVO) with no identified microorganism is treated empirically, the clinical outcome is not well understood. METHODS: We conducted a retrospective review of patients with PVO at a tertiary-care hospital from 2000 through 2012. The study compared clinical features and outcomes of microbiologically confirmed (M-PVO) with clinically diagnosed PVO (C-PVO). RESULTS: Of 151 patients with PVO, 75 (49.7%) had M-PVO. Compared to patients with M-PVO, patients with C-PVO had fewer underlying medical conditions. In addition, they presented less freque…

Clinical heterogeneity in paediatric patients with chronic nonbacterial osteomyelitis and chronic recurrent multifocal osteomyelitis.

Chronic recurrent multifocal osteomyelitis (CRMO) and chronic nonbacterial osteomyelitis (CNO) are autoinflammatory diseases characterised by sterile bone inflammation. Patients with CNO or CRMO often complain about pain in the extremities, although several unusual manifestations have been reported. We report four paediatric cases of CNO/CRMO, demonstrating marked clinical variability in symptom onset and disease course. All patients presented with bone pain during their disease course, whereas unusual manifestations such as fever of unknown origin prior to the bone pain and cervical pain were…

MRI features of spinal chronic recurrent multifocal osteomyelitis/chronic non-bacterial osteomyelitis in children.

BACKGROUND: Spinal lesions in pediatric chronic recurrent multifocal osteomyelitis/chronic non-bacterial osteomyelitis (CRMO/CNO) can cause permanent sequelae; thus, early recognition of these is vital for management. OBJECTIVE: To characterize the MR imaging features and patterns of pediatric spinal CRMO/CNO. MATERIALS AND METHODS: This cross-section study received IRB approval. The first available MRI with documented spine involvement in children with CRMO/CNO was reviewed by a pediatric radiologist. Descriptive statistics were used to describe the characteristics of vertebral lesions, dis…

Clinical characteristics of acute epiphyseal osteomyelitis and acute metaphyseal osteomyelitis of long bones in children.

This study aimed to summarize the clinical characteristics of acute epiphyseal osteomyelitis and acute metaphyseal osteomyelitis of long bones in children. Data of 43 children with acute osteomyelitis of long bones diagnosed and treated from November 2017 to January 2021 were retrospectively analyzed. Medical records, laboratory results, and MRI were reviewed. Surgical drainage of lesions was done to retrieve infective fluid and tissue for examination. There were 12 cases of metaphyseal and epiphysis involvement, including five boys and seven girls, aged from 1 to 12 years old, and the length …

Pyogenic osteomyelitis versus pseudo-osteomyelitis in Gaucher's disease. Report of a case and review of the literature.

Presented is a young girl with Gaucher's disease who developed acute bone pain accompanied by signs of inflammation and who was felt to have possible pyogenic osteomyelitis. The lack of significant pathogenic bacterial growth on culture and the findings at orthopedic surgery led the authors to conclude that this child probably represented a case of pseudo-osteomyelitis, but the isolation of an anaerobe from the operative culture of the involved bone leaves the exact diagnosis unclear. Since this child underwent an open surgical procedure, she was treated with antibiotics to prevent the possibl…

Applicability of histopathological osteomyelitis evaluation score (HOES) in chronic osteomyelitis of the foot - A feasibility study.

BACKGROUND: Diagnosis and classification of chronic osteomyelitis is not based on any objective criteria. None of the available methods for this is completely reliable. Tienmann et al. (2014) has described the so called histopathological osteomyelitis evaluation score (HOES) to overcome this limitation. But this has not been externally validated or tested in cases with foot osteomyelitis. METHODS: We prospectively reviewed the histopathological samples of 30 consecutive patients (30 feet) with foot osteomyelitis managed operatively. There were 19 males and 11 females with an average age of 41…

Histopathological Osteomyelitis Evaluation Score (HOES) - an innovative approach to histopathological diagnostics and scoring of osteomyelitis.

BACKGROUND: Treatment and diagnosis of osteomyelitis are still a challenging problem for surgeons, microbiologists and histopathologists. A direct microbiological detection of bacteria in tissues is still gold standard, but it is not always successful for example in chronic osteomyelitis and/or when an antibiotic treatment has already been started or in cases of low virulent bacteria. The goal of this study was to define diagnostic criteria of osteomyelitis, the inflammatory regression of osteomyelitis ("osteomyelitis score") under specific therapy by the correlation of histopathological and m…

Reconstruction of Severe Forearm Osteomyelitis.

This article reviews the surgical management of severe chronic forearm osteomyelitis using free vascularized fibular flaps (VFFs), with or without the one-bone forearm (OBF) technique. Based on a 22 patient retrospective series, outcomes show that VFF enables reliable reconstruction of extensive bone and soft tissue defects following radical debridement. In selected cases, OBF offers a salvage solution for both radius and ulna bone loss. The approach led to high infection control rates, bone healing, and functional recovery. Microsurgical reconstruction, particularly using VFF, remains the gol…

Chronic Osteomyelitis - Pathophysiology and Treatment.

Chronic osteomyelitis remains a significant challenge in orthopaedic and trauma surgery, despite substantial advancements in surgical techniques, antimicrobial therapy, and microbiological diagnostics. The progression from acute to chronic infection is characterised by the formation of bacterial biofilms, which result in profoundly reduced antibiotic susceptibility and pathogen persistence. The Cierny-Mader classification provides a critical clinical framework, by integrating the anatomical extent of the infection with the host's physiological condition. This review summarises current understa…

Vertebral Osteomyelitis and Epidural Abscess.

Severe spinal infections including vertebral osteomyelitis and spinal epidural abscess pose challenges in diagnosis and management. The evolution of epidemiologic risks for these processes includes the aging of the population, increase in spinal instrumentation procedures and health care-associated device utilization, and increased interconnection of the global community. Clinical presentation may be nonspecific, so clinicians must maintain a high index of suspicion to ensure prompt diagnosis and treatment to prevent progression to long-standing morbidity. Collaboration with specialists in sur…

[Staphylococcus aureus osteomyelitis in children].

Osteomyelitis is an infectious disease of the bone that primarily affects children. Here we discuss three patients, aged 11-13 years that were diagnosed with osteomyelitis. Despite the fact that in all three patients the infection was caused by the Staphylococcus Aureus bacteria, the clinical presentation and diagnostic results in all three was different. This case report presents various courses of the same disease, each with a slightly different treatment and outcome. Although the incidence of osteomyelitis is decreasing, it is of great importance to consider osteomyelitis as a diagnosis in …
