The natural course of bridging osteophyte formation on MRI-A pictorial illustration.

Diffuse skeletal hyperostosis is a common spinal disorder, but its pathophysiology is mostly unclear. The disorder can lead to a variety of symptoms, but many patients remain relatively asymptomatic. We present a case demonstrating the development of bridging osteophytes on a series of magnetic resonance images. An elderly person's spine was scanned repeatedly due to non-specific back pain during the last 4 years and the consecutive images revealed the formation of a bony bridge in the lumbar spine. Extensive bone marrow edema was seen during the formation of the osteophyte, suggestive of an o…

A Surprising Cause of Contained Aortic Rupture: Perforation from a Lumbar Osteophyte.

We describe a patient with contained aortic rupture due to perforation from a protruding lumbar osteophyte, who was treated by open surgery. This case underlines that less common aortic pathologies are possible, which require a high suspicion index to be diagnosed.

Osteophyte excision without cyst excision for a mucous cyst of the finger.

Osteophyte excision is a mainstay of treatment for mucous cyst combined with Heberden's node in a distal interphalangeal joint or in an interphalangeal joint of the thumb. The aim of this study was to evaluate the results of osteophyte excision without cyst excision for the treatment of a mucous cyst combined with Heberden's node. The medical records of 37 patients (42 cases) with a mucous cyst with Heberden's node were retrospectively reviewed. Thirty-eight of 40 cases with available pre-operative simple radiographs showed evidence of joint arthrosis. A T-shaped skin incision of the joint cap…

Osteophyte detection for hand osteoarthritis identification in x-ray images using CNNs.

In this paper we describe a class of analog algorithms based on the concept of cellular neural networks (CNNs) for detecting osteoarthritis (OA) from x-ray images. The indicator of OA that we examine is the presence of bony spurs or osteophytes in the vicinity of the weight bearing joints of the fingers. Results on a series of hand x-ray images are shown to be promising.

Association of gene polymorphisms with intervertebral disc degeneration and vertebral osteophyte formation.

STUDY DESIGN: Cross-sectional cohort study of elderly people. OBJECTIVES: To examine the factors influencing osteophyte formation without lumbar disc degeneration and to estimate the implications of osteophytes from the viewpoint of low back pain and gene polymorphisms. SUMMARY OF BACKGROUND DATA: The degenerative changes that occur in the intervertebral discs are the point of departure of osteophyte formation. Several studies on factors associated with genetic susceptibility to spinal osteophyte formation, such as VDR and TGF-beta1. However, there are no detailed studies concerning osteophy…

Multilevel cervical fusion and its effect on disc degeneration and osteophyte formation.

STUDY DESIGN: The effect of single and double cervical fusion on adjacent segments was investigated using a finite element model of the cervical spine. A healthy spine and a cervical spine with a single and double fusion at different levels were analyzed and evaluated. Disc degeneration and osteophyte formation at the endplates and joints can then be addressed. OBJECTIVES: To evaluate the biomechanical effects of cervical fusion on the cervical spine from C3-C7. The goal was to asses the increase of intervertebral disc and bone stress induced by cervical fusion, the effects of single versus d…

Traumatic pseudoaneurysm of the descending thoracic aorta inflicted by a spinal osteophyte.

This report describes a case involving a traumatic pseudoaneurysm of the descending thoracic aorta resulting from direct contusion inflicted by the tip of a spinal osteophyte at the level of T11-T12. Endovascular repair with placement of a custom-made endograft by the femoral route was performed after routine arteriography to locate the origin of the Adamkiewicz artery.

[Observations on structure of bone in human mandibular condyles with osteophyte formation].

The purpose of this study was to investigate the functional characteristics of human mandibular condyles from a morphological viewpoint. The structure of bone in the human mandibular condyles with osteophyte formation was observed macroscopically and microscopically and compared with that in the condyles which seemed to be normal. The following observations were made: 1. Generally, it was observed that remodeling seemed to occur frequently in compact bone in the joint surface area of the mandibular condyle. 2. As the distribution of lamellar bone, nonlamellar, and bundle bone in the joint surf…

Correlation of inflammatory mediators with osteophyte formation in end-stage knee osteoarthritis.

Osteoarthritis (OA) is a prevalent degenerative joint disorder with varying clinical and radiological presentations. This study aimed to investigate the relationship between pro-inflammatory mediators in serum and synovial fluid (SF) and osteophyte size in patients with end-stage knee OA. Forty-four patients (58 knees) who underwent total knee arthroplasty were included. Serum and SF levels of matrix metalloproteinase-1 (MMP-1), interleukin-8 (IL-8), and interleukin-18 (IL-18) were measured using enzyme-linked immunosorbent assay. Radiographic parameters, including osteophyte size, were assess…

Surgical Procedure for Osteophyte Impingement of the Quadriceps Tendon: A Case Report.

INTRODUCTION: Osteophytes associated with knee osteoarthritis can irritate surrounding soft tissues and cause pain and mechanical symptoms. Although impingement of the medial collateral ligament and popliteus tendon has been reported, symptomatic osteophyte impingement of the quadriceps tendon is rare, and no reports describe its surgical treatment. CASE REPORT: An 82-year-old Asian man presented with several years of left knee pain accompanied by a snapping sensation during knee flexion. The pain did not occur during gait. Physical examination revealed tenderness at the superolateral aspect …

A rare cause of esophageal stenosis: Compression due to a thoracic osteophyte.

Several cases of esophageal stenosis caused by cervical vertebral osteophytes have been reported; however, few reports of esophageal stenosis caused by thoracic osteophytes are available. We describe the case of an 86-year-old man with esophageal stenosis caused by a thoracic osteophyte near the tracheal bifurcation. An endoscopic ultrasonography examination was scheduled to determine the cause of acute pancreatitis; however, lacerations observed at the bifurcation following endoscope removal during prior esophagogastroduodenoscopy led us to cancel the ultrasonography to avoid potential esopha…

Multidisciplinary Approach to Isolated Pulmonary Vein Compression by an Enlarging Vertebral Osteophyte.

Compression of mediastinal structures by vertebral osteophytes is rare. We report a case of pulmonary vein compression by a vertebral osteophyte that failed stenting. A minimally invasive approach to osteophyte removal with subsequent re-expansion angioplasty yielded an optimal outcome, negating the need for cardiopulmonary bypass, stent removal, and pulmonary venoplasty. (Level of Difficulty: Intermediate.).

Fully endoscopic transoral resection of high cervical osteophyte. How I do it?

BACKGROUND: Fully endoscopic transoral approaches (FETOA) constitute a reasonable option for the treatment of middling compressive pathology that involve the craniocervical junction and higher cervical levels. METHODS: We describe, step by step, the FETOA for the treatment of upper cervical lesions. More specifically, the ones that are located between C1 and C3. A giant anterior C1-C2 osteophyte resection will be used as an illustration of these approaches. CONCLUSIONS: This technique represents a minimally invasive treatment option for these kinds of high cervical lesions. It offers optimal…

The characteristics of osteophyte around lumbar vertebral foramina associated with spinal stenosis.

Spinal stenosis most commonly occurs on lumbar vertebrae because of degenerative changes. This research studied the characteristics of osteophyte development in lumbar vertebrae foramina and association of osteophyte development with lumbar spinal stenosis. The total number of all levels of lumbar spines of subjects was 179 from 31 to 90 years of age. The vertebral foramen was divided into six zones. The prevalence and measurements of the length of osteophytes in the vertebral foramina were obtained. The prevalence and length of osteophytes in the posterior body zone were higher than the lamin…
