Sclerosing mesenteritis and mesenteric panniculitis - clinical experience and radiological features.

BACKGROUND: Sclerosing mesenteritis (SM) is sometimes used as an umbrella-term for idiopathic inflammatory conditions in the mesentery. Mesenteric panniculitis (MP) is a radiological finding and its relation to clinical SM is not fully understood. The aims of this study were to determine whether any correlation could be found between the radiological findings and the clinical disease course. METHODS: Patients observed due to idiopathic inflammation of the mesentery were identified. If SM could be verified histologically or MP radiologically, the patients were included in this descriptive retr

Mesenteric liposarcoma or lipodystrophy: an elusive diagnosis.

Mass lesions of the mesentery may be fortuitously encountered on computerized tomographic (CT) scans, posing a diagnostic challenge. Despite CT, magnetic resonance (MR) imaging and a surgical biopsy, a patient with mesenteric lipodystrophy was misdiagnosed as having a low-grade mesenteric liposarcoma. Spontaneous regression of the mass on control CT scan and review of the pathological material prompted us to reconsider the diagnosis of malignancy. Because a wide variety of tumors and pseudotumors produce alterations in the density and volume of mesenteric fat on CT scan, a surgical biopsy is u

Panniculitis-Like Presentation of Extracavitary Primary Effusion Lymphoma.

Primary effusion lymphoma (PEL) is defined as a HHV-8-associated large B-cell lymphoma, which favors HIV-infected young adults, typically presenting as a serous (pleural, pericardial, or peritoneal) effusion with no identifiable tumor mass. Uncommon instances of lymphoid proliferations with the same morphology, immunophenotype, and molecular features as PEL, but occurring as a solid tumor mass without serous cavities involvement, have been termed extracavitary (or solid) variant of PEL. We hereby report the exceptional case of a HIV-associated extracavitary PEL primarily localized to the skin

Fat embolism secondary to yellow fat disease in an Appaloosa horse.

A 4-year-old female Appaloosa horse was referred to the Department of Comparative Pathology, Veterinary Faculty, with a history of colic and acute respiratory distress. At necropsy, gross lesions consisted of extensive firm, multinodular, hemorrhagic foci in fat tissues with yellow-brown discoloration. The most affected areas were peritoneal fat and perirenal, epicardial, and subcutaneous adipose tissues. Other findings were hepatic lipidosis and multiple 1-1.5 cm hemorrhagic foci scattered in both lungs. Histopathological examination revealed severe degeneration and necrosis of adipose tissue

New technique and management guidelines for percutaneous nonendoscopic tube gastrostomy.

A percutaneous nonendoscopic gastrostomy tube placement device and a new technique for nonendoscopic gastric feeding tube placement were developed for use in animals weighing < 15 kg. After a successful experimental trial in 7 cats, the tube placement device was used in 8 clinical animals (7 cats and 1 dog). All gastrostomy feeding tubes were placed on the first attempt within 10 minutes. Position of the tube was evaluated by endoscopy in 14 of 15, by radiography in 4 of 15, and by celiotomy and necropsy in 2 of 15 animals, respectively. Placement was comparable to the percutaneous endoscopic

Mesenteric Panniculitis as a Side Effect of Nivolumab in a Patient with Larnyngeal Cancer.

Mesenteric panniculitis is rare, usually idiopathic, caused by inflammation of the fatty tissue of the mesentery, especially in the small intestine. The relation between cancer and mesenteric panniculitis is unclear. In some studies, mesenteric pannicullitis precedes cancer diagnosis; on the other hand, some studies suggest no correlations. Immunotherapeutics have a wide range of side effects; virtually, every system and organ in the body can be affected. Herein, we presented a rare case of mesenteric panniculitis in a patient with larnyngeal cancer as a side effect of nivolumab treatment. The

A case of immunoglobulin G4-related sclerosing mesenteritis without other organ involvement.

A 64-year-old man presented to our hospital with abdominal pain and 4-5 episodes of watery diarrhea per day for 2 months. Abdominal ultrasound examination revealed a mass in the peritoneal cavity, and computed tomography showed a 13.4 cm mass in the mesentery and a 3 cm mass in the mesocolon. The patient underwent laparoscopic partial resection for diagnosis. Microscopically, abundant fibrosis and numerous immunoglobulin (Ig) G4-positive plasma cells were observed. The serum level of IgG4 was 665 mg/dl postoperatively. These findings suggested that the lesion was consistent with IgG4-relate

A rare case report of immunoglobulin G4-related sclerosing mesenteritis and review of the literature.

INTRODUCTION: Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is a rare autoimmune disorder involving 1 or multiple organs, most commonly the pancreas, lacrimal glands, and salivary glands. However, IgG4-related sclerosing mesenteritis (SM) involving the small-bowel mesentery is rare. Given that IgG4-related SM usually mimics the imaging characteristics of mesenteric malignancies, its preoperative diagnosis remains challenging. In addition, no specific consensus has been reached regarding the treatment of IgG4-related SM. Therefore, a better understanding of the characteristics, treatment,

Anaplastic lymphoma masquerading as sclerosing mesenteritis: a case report.

INTRODUCTION: Sclerosing mesenteritis is a rare fibroinflammatory disease of unknown pathogenesis and etiology. Although more commonly benign in its clinical course, sclerosing mesenteritis can mimic certain malignant disorders, such as lymphoma. CASE REPORT: This report describes the case of a previously healthy young woman who develops an acute illness thought to be related to sclerosing mesenteritis following an extensive diagnostic work-up including biopsy sampling, which was negative for malignancy. However, despite aggressive medical care, the patient passed away. Autopsy revealed exten

A case of sclerosing mesenteritis with rheumatoid arthritis.

Sclerosing mesenteritis (SM) is an uncommon disorder characterized by chronic nonspecific inflammation involving the adipose tissue of the mesentery. The etiology remains unclear. It has been reported in association with and as an initial presentation in some autoimmune diseases. Its clinical presentation and laboratory findings are typically nonspecific and definitive diagnosis usually requires biopsy or surgical excision. We report a patient with a history of rheumatoid arthritis who was found to have an intra-abdominal mass suspicious for malignancy. A biopsy revealed the diagnosis of SM.

Transudative chylothorax associated with sclerosing mesenteritis.

Transudative chylothorax is an uncommon type of chylous pleural effusion, typically secondary to chyle leak and a coexisting disorder such as heart failure or liver cirrhosis. Sclerosing mesenteritis is a rare inflammatory disease of the small bowel mesentery, and has once previously been reported as a cause of chylothorax. We present the case of an 81-year-old man with a right-side transudative chylothorax associated with congestive heart failure and sclerosing mesenteritis. We discuss potential mechanisms.

Sclerosing mesenteritis successfully treated with a TNF antagonist.

A 29-year-old female presented with intermittent nausea, vomiting, fevers, abdominal pain and fatigue. CT scans of the abdomen revealed inflammatory changes within the mesentery and small bowel. Histopathology of the mesentery and omentum showed chronic inflammation and fibrosis, supporting a diagnosis of sclerosing mesenteritis. Over the past 2 years, the patient suffered debilitating paroxysmal abdominal pain despite treatment with prednisone, azathioprine, sulfasalazine and narcotics. Additionally, she developed sacroiliitis diagnosed clinically and on radiographs. Intravenous infliximab (5

Occasional finding of mesenteric lipodystrophy during laparoscopy: a difficult diagnosis.

Mesenteric lipodystrophy is a rare pathological condition affecting the mesentery. Its initial presentation is typically asymptomatic. Pathological characteristics are unspecific, and generally attributed to inflammation, unless the diagnosis is suspected. Laparoscopy done for other reasons has been, as in this case, unsuccessful in providing evidence for the correct diagnosis, thus requiring laparotomy due to lack of diagnostic tissue. After 6 mo no further medical therapy is required, as the patient remains asymptomatic. Discussion of this case and a brief review of the literature are presen

Sclerosing mesenteritis with occult ileal perforation: report of a case simulating extensive intra-abdominal malignancy.

Sclerosing mesenteritis is a rare condition that is characterized by fibrosis affecting mainly small-bowel mesentery, which in extensive cases may mimic advanced intra-abdominal malignancy. Establishing the diagnosis in such cases is a clinical and histopathologic challenge. We report the successful management of a case of extensive sclerosing mesenteritis with occult ileal perforation, which was possibly the triggering cause. Severe complications occurred as a result of both the disease itself and its surgical treatment. Despite the complex course and life-threatening complications, a good pr

Mesenteric panniculitis associated with abdominal tuberculous lymphadenitis: a case report and review of the literature.

Mesenteric panniculitis is a rare disease characterized by chronic non-specific inflammation of the mesenteric adipose tissue. The specific aetiology of the disease is previously unknown. A case diagnosed as mesenteric panniculitis is presented. The cause was biopsy-proved abdominal tuberculous lymphadenitis. To our knowledge, mesenteric panniculitis associated with tuberculosis infection has not been reported previously in the literature. Thus, we would like to present the first case and describe CT features of the disease.
