Obstetrical brachial plexus injury in newborn babies delivered by caesarean section.

The hand surgeon is frequently involved in the medicolegal assessment of birth palsy cases. Although brachial plexus injury has been reported in newborns delivered by Caesarean section, it is difficult to determine if these cases are due to excessive force when delivering the infant from the uterus or whether the palsy is related to other factors. We have studied our series of 16 cases of obstetrical brachial plexus palsy with special attention to the newborn baby delivered by Caesarean section. We have reviewed the English literature over the last decade and found that birth palsy in newborns

Orthopaedic considerations in obstetric brachial plexus palsy.

Obstetric brachial plexus palsies are seen in orthopaedic practice today despite improved obstetric care. The incidence is believed to be 0.35 to 0.87 cases per 1,000 live births. The pathomechanics of these injuries are not fully understood, although numerous risk factors have been identified. Large birth weight, breech presentation, shoulder dystocia, and forceps-assisted deliveries have all been implicated. No one standard treatment modality exists. Therefore, the treating orthopaedist must be familiar with this entity and with the various accepted treatment approaches in order to formulate

[Experimental hypocalcemia as a model for the study of etiopathogenetic factors of hypocalcemic hysteroparesis due to parturition in dairy cows].

Hypocalcaemia was experimentally induced to cattle of different age groups, and the resulting model showed that intraplasmic parathormone-induced calcium rise due to activation of calcium from the apatite crystals of the skeletal depot was a function of age and a sufficiently plausible explanation for age-dependent growing morbidity rates in the context of hypocalcaemic hysteroparalysis in concomitance with parturition. High vitamine D3 doses are recommended for systematic prophylaxis.

Brachial plexus palsy involving the posterior shoulder at spontaneous vaginal delivery.

Brachial plexus injuries sustained at birth have most often been attributed to the maneuvers performed when attempting to relieve a shoulder dystocia or to deliver a breech vaginally. In the case now reported, the brachial plexus injury involved the posterior shoulder. As the delivery was spontaneous and without forceps or manual rotation, with delivery effected using only the McRobert's maneuver, it is hypothesized that maternal expulsive forces in conjunction with lodging of the posterior shoulder on the sacral promontory resulted in the injury.

Unilateral vocal cord paralysis associated with subdural haemorrhage in a newborn infant.

A term infant developed stridor, hoarse cry and respiratory distress after forceps-assisted delivery. Oral feeding resulted in aspiration. Flexible laryngoscopy showed a right-sided vocal cord paralysis (VCP). A magnetic resonance image (MRI) of the brain revealed an ovoid lesion in the posterior fossa impinging on the brainstem, which was considered to represent a subdural haematoma. Clinical signs of vocal cord palsy and the associated MRI changes resolved spontaneously by 6 weeks of age. Vocal cord palsy is a common cause of stridor in newborn infants, although in many cases it is considere

Risk factors for brachial plexus injury with and without shoulder dystocia.

OBJECTIVE: To ascertain whether brachial plexus palsy (BPP) that occurs without shoulder dystocia (SD) represents a traction injury during unrecognized SD or a natural phenomenon with a different mechanism of injury, we compared risk factors and outcomes between SD-associated and non-SD-associated BPP. STUDY DESIGN: Neonates with BPP after cephalic vaginal delivery were pooled from all deliveries at Johns Hopkins (June, 1993-December, 2004) and a dataset of litigated permanent BPP from multiple institutions (1986-2003), grouped by SD association based on clinician documentation and compared b

Outcome of surgically treated birth-related brachial plexus injuries in twenty cases.

Birth-related brachial plexus injury occurs in 0.19-2.5 per 1,000 live births, of which 70-92% improve with conservative management. With the advent of microsurgical techniques, patients who fail expectant treatment may benefit from brachial plexus exploration and reconstruction. From 1991 to 1996, 87 patients were referred to the multidisciplinary brachial plexus clinic at St. Louis Children's Hospital. Twenty patients were selected for surgical management. The average age at surgery was 10.5 months (range 3-35, median = 8), with an average follow-up of 23.9 months (range 7-45, median = 24).

Migration of an extradural catheter into the subdural space. A case report.

After repeated, successful extradural injections in a parturient, the extradural catheter migrated into the subdural space. The injection of 3% chloroprocaine 10 ml resulted in an extensive block, which extended to the cranial nerves and paralysed the respiratory muscles. The position of the catheter tip was confirmed by fluoroscopy. Subdural injection should always be suspected when tests for subarachnoid injection are negative, but an extensive block occurs gradually within 15-20 min. Respiration and circulation should be supported until the block regresses.

[Brachial plexus palsy associated with birth. A review of 30 cases].

OBJECTIVE: Our objective was to perform an updated analysis of the incidence and risk factors during pregnancy and labor related to obstetrical brachial palsy. PATIENTS AND METHODS: A retrospective study of all cases of brachial palsy associated with birth detected in our hospital between January 1994 and March 1998 was performed. Data recorded included age of mother, parity, gestational age at the moment of birth, type of birth, presentation, duration of delivery, sex of child, weight, Apgar test at 5 minutes, arterial pH of umbilical cord, type of brachial palsy, side affected and associati

The treatment of brachial plexus injuries.

The author describes 1068 patients with brachial plexus lesions who were referred to him during a period of 18 years. Seventy two percent of the injuries were caused by road traffic accidents. Traction or crush injuries were the usual type encountered. They may occur at five levels above, behind and below the clavicle. Of 329 patients who underwent operation, 23% had an associated injury of a major vessel and 80% had multiple injuries elsewhere. The operative approach preferred is described and the principles governing the choice of nerve repair, nerve graft and nerve transfer are discussed. T

[Virginia Apgar and her scale].

Virginia Apgar (1909-1974), born in New Jersey, managed to continue medical school despite the financial crisis of 1929, continued for a brief time in surgery and subsequently became one of the first specialists in anaesthesiology. In 1949 she was appointed to a professorship, the first woman to reach this rank at Columbia University in New York. She then dedicated herself to obstetric anaesthesiology and devised the well known scale for the initial assessment of newborn babies, according to 5 criteria. From 1959 she worked for the National Foundation for Infantile Paralysis (now March of Dime

Failed forceps.

Among 53 cases of failed forceps occurring in 6524 uncomplicated primiparous deliveries, depression at birth and encephalopathy occurred with similar frequency as when cesarean section was done for failure to progress in the second stage, and birth trauma was no more common than that with successful midforceps delivery. Factors predisposing to or associated with midforceps deliveries or second stage cesarean sections were short mothers, heavy babies, induced or prolonged labors, and fetal distress or meconium release in labor.

Pathway for the Differential Diagnosis of Obstetrical Brachial Plexus Palsy.

BACKGROUND: Obstetrical brachial plexus palsy (OBPP) is a common birth injury, generally presenting as upper limb paralysis at birth. While diagnosis is straightforward when associated with known risk factors, misdiagnoses can nevertheless occur. This study aimed to elucidate the process of differential diagnosis for OBPP by analyzing cases of suspected OBPP, ultimately diagnosed with other conditions. METHODS: We reviewed the electronic medical records of patients under 1 year of age presenting with suspected or previously diagnosed OBPP at 2 tertiary childrens' centers in Osaka between 1994

Automatic identification of lumbar level with ultrasound.

An ultrasound-based system is created to label the lumbar vertebrae for the purpose of percutaneous needle insertion. Many lumbar punctures have a preferable vertebral level for needle insertion, but the traditional method of manual palpation is known to be inaccurate for determining the level. Needle insertion for epidural anesthesia in obstetrics is preferably performed at the L3-L4 interspace and miscalculation can lead to complications such as nerve damage and paralysis. Similar risks occur for other spinal needle insertions. In this paper, an ultrasound-based system is devised that create
