Autoimmune and paraneoplastic movement disorders: An update.

Movement disorders (MDs) are common in patients with autoimmune disorders affecting the central and peripheral nervous system. They may be observed in autoimmune disorders triggered by an infectious agent, such as streptococcus in Sydenham's chorea, or in basal ganglia encephalitis with antibodies against the dopamine-D2 receptors. In these patients chorea or dystonia are usually the most prominent hyperkinetic MDs. MDs are also observed in patients with diffuse or limbic encephalitis with antibodies directed against neuronal cell-surface antigens. Anti-NMDA receptor encephalitis is one of the

Paraneoplastic neurological syndromes.

Paraneoplastic neurological syndromes (PNS) can be defined as remote effects of cancer that are not caused by the tumor and its metastasis, or by infection, ischemia or metabolic disruptions. PNS are rare, affecting less than 1/10,000 patients with cancer. Only the Lambert-Eaton myasthenic syndrome is relatively frequent, occurring in about 1% of patients with small cell lung cancer. PNS can affect any part of the central and peripheral nervous system, the neuromuscular junction, and muscle. They can be isolated or occur in association. In most patients, the neurological disorder develops befo

Anti-LGI1-antibody autoimmune encephalitis as a neurological paraneoplastic syndrome associated with gastrointestinal stromal tumour: a case report.

PURPOSE: The aim of the case is to present the significance of a thorough analysis of the cases of patients with newly developed memory disorders accompanied by epileptic seizures. CASE DESCRIPTION: We present a case of a 61-year-old male patient who was admitted to the hospital due to unexplained memory disorders and seizures. Further examination showed the presence of anti-LGI1 (leucine-rich glioma-inactivated 1) antibodies. Computed tomography of the abdominal cavity revealed a mass located between the stomach and the spleen. Histopathological examination identified the mass as a gastroint

Panhypopituitarism as the first sign of paraneoplastic limbic encephalitis in a patient with cured testicular cancer: a case report.

BACKGROUND: Paraneoplastic limbic encephalitis is a rare neurologic syndrome that affects patients with cancer and commonly presents with symptoms of personality changes, visual disturbances, seizures, vertigo, and memory loss. It has an immune-mediated pathophysiology and is associated with antibodies directed against both the tumor and limbic structures in the nervous system. Here, we report a case of paraneoplastic limbic encephalitis with an unusual presentation with initial symptoms of panhypopituitarism. CASE PRESENTATION: A 28-year-old Caucasian man with history of testicular cancer in

Current trends in autoimmunity and the nervous system.

In the broad field of autoimmunity and clinical immunology, experimental evidence over the past few years have demonstrated several connections between the immune system and the nervous system, both central and peripheral, leading to the definition of neuroimmunology and of an immune-brain axis. Indeed, the central nervous system as an immune-privileged site, thanks to the blood-brain barrier, is no longer a dogma as the barrier may be altered during chronic inflammation with disruptive changes of endothelial cells and tight junctions, largely mediated by adenosine receptors and the expression

[Analysis of Small Cell Lung Cancer with Paraneoplastic Limbic Encephalitis].

BACKGROUND: The aim of this study was to investigate the clinical features of paraneoplastic limbic encephalitis (PLE) with small cell lung cancer (SCLC) and improve clinical diagnosis and treatment. METHODS: The clinical data of 15 patients with SCLC combined with PLE from January 1980 to May 2017 were collected from Beijing Union Hospital. Their symptoms and laboratory data were analyzed and the prognosis of the patients was followed. RESULTS: PLE is a rare disease, the incidence rate in SCLC is about 0.842%. The data may be underestimated because of misdiagnose or missed diagnosis; High i

[Paraneoplastic neurologic syndromes. Diagnostic and pathogenetic significance of autoantibodies].

Paraneoplastic neurologic syndromes (PNS) are rare disorders of the nervous system that cannot be ascribed to metastases or to destruction of vital systemic organs by the tumor or its treatment. Most frequently, PNS occur in association with small-cell lung, breast or ovarian carcinomas. The most frequent PNS is Lambert-Eaton myasthenic syndrome, which at the same time has the highest predictive value concerning an underlying carcinoma. PNS have been classified mostly under anatomical aspects but now are increasingly categorized according to specific antibodies. In certain neurologic syndromes

[Lung cancer and paraneoplastic syndromes].

Paraneoplastic syndromes (PNS) are a relatively common manifestation of cancer, and in some cases they may be the first symptom. Lung cancer has the highest incidence of paraneoplastic syndrome. This fact is important considering a non explained endocrinological and neurological syndrome, it may facilitate a prompt diagnosis, and in some cases an adequate treatment. PNS evolution seems to be parallel to the subjacent cancer. PNS management requires specific measures, because in some cases, it may compromise the patient life. Neurological and endocrinological PNS associated to lung cancer are r

Modulation of autonomic balance by tumors and viruses.

Autonomic balance, a function generally under host control, is subject to modulation by other signalers. In some cases, modulation of host autonomic function through behavioral and physical stressors exerted by another individual may have negative consequences for the stress recipient by inducing sympathetic bias. Modulation of autonomic function may sometimes benefit one party at the expense of another. Tumors and HIV are examples of illegitimate signalers who may induce host sympathetic bias to promote their own growth and evade host immune surveillance. Paraneoplastic and paraviral syndrome

Certain notable clinical attributes of the histiocytic sarcomas of the central nervous system.

By the term reticulum cell sarcoma we denote any tumor composed predominantly of undifferentiated cells, some of which have the qualities of histiocytes. The origin of such CNS tumors may be traced to circulating monocytes, perithelial ro meningeal histiocytes or microgliocytes. The ubiquity of cells of the monocyte-histiocyte series allows six possibilities of CNS involvement: a) primary in the brain: b) secondarily involve the brain or spinal cord by extending from a cranial bone or vertebra to the epidural space c) rarely to involve intraneuronal tissues (lymph nodes, bone, viscera) and the

Rare association of anti-hu antibody positive paraneoplastic neurological syndrome and transitional cell bladder carcinoma.

Introduction. Paraneoplastic encephalomyelitis (PEM) and subacute sensory neuronopathy (SSN) are remote effects of cancer, usually associated with small-cell lung carcinoma and positive anti-Hu antibody. We describe the rare association of bladder transitional cell carcinoma (TCC) with anti-Hu antibody positivity resulting in this paraneoplastic neurological syndrome. Patient. A 76-year-old female presented with bilateral muscle weakness and paraesthesia of the upper and lower limbs in a length-dependent "glove and stocking" distribution. Central nervous system symptoms included cognitive prob

Relative Frequency and Distinctive Features of Anti-Ma2 Nonparaneoplastic Neurologic Disorders: A French Cohort Study.

BACKGROUND AND OBJECTIVES: Although anti-Ma2 antibodies (Ma2-Abs) typically associate with paraneoplastic encephalitis, a subset of patients with Ma2-Abs does not have any detectable tumor. The clinical specificities of these idiopathic cases are unknown. The aim of this study was to describe clinical phenotypes and outcomes of patients with idiopathic Ma2-Abs (I-Ma2) compared with patients with paraneoplastic Ma2-Abs (PNS-Ma2). METHODS: A retrospective review of the French Reference Center of Paraneoplastic Neurologic Syndromes and Autoimmune Encephalitis database was conducted to identify c

Paraneoplastic dermatomyositis related to a chondrosarcoma involving the cavernous sinus.

Approximately one third of all cases of dermatomyositis may be associated with malignancy. We describe a patient with unexplained rash, joint pain, and muscle weakness, who subsequently developed a cavernous sinus syndrome due to a central nervous system chondrosarcoma. Discovery of this tumor and further dermatologic evaluation, including skin biopsy, resulted in diagnosis of paraneoplastic dermatomyositis due to cavernous sinus chondrosarcoma.

Hodgkin's Lymphoma: A Review of Neurologic Complications.

Hodgkin's lymphoma is a hematolymphoid neoplasm, primarily of B cell lineage, that has unique histologic, immunophenotypic, and clinical features. Neurologic complications of Hodgkin's Lymphoma can be separated into those that result directly from the disease, indirectly from the disease, or from its treatment. Direct neurologic dysfunction from Hodgkin's Lymphoma results from metastatic intracranial spinal disease, epidural metastases causing spinal cord/cauda equina compression, leptomeningeal metastases, or intradural intramedullary spinal cord metastases. Indirect neurologic dysfunction ma
