Effect of Transosseous Tunnels on Patella Fracture Risk After Medial Patellofemoral Ligament Reconstruction: A Cadaveric Study.

PURPOSE: To determine whether (1) tunnels that breach the anterior cortex of the patella result in increased fracture risk and (2) transosseous tunnels drilled across the patella significantly reduce the tensile force needed to fracture the patella. METHODS: Twenty-six fresh-frozen cadaveric human patellas were randomized to 1 of 3 groups: a control group with unmodified patellas, a group with 2 transverse tunnels (TT) that did not breach the anterior cortex, and a group with 2 TT that breached the anterior cortex of the patella (PA). Patellas were connected in series to a load cell via freez

Single-Stage Fixation of Bilateral Open Medial Hoffa Fractures Associated with Bilateral Patella Fracture and Bilateral Distal Radius Fractures: A Case Report.

INTRODUCTION: Posterior cruciate ligament (PCL) avulsion fractures, lateral collateral ligament (LCL)-biceps femoris complex avulsion injuries are relatively uncommon injuries, and associated diaphyseal tibia fracture further is the rarest combination. It is challenging to diagnose such complex injuries, and missed injuries, particularly at the knee can lead to significant instability, increasing patients' morbidity. CASE REPORT: A 25-year-old male patient was diagnosed with PCL avulsion fracture, LCL - biceps femoris complex injury at the fibular side with Type I open mid-shaft both-bone leg

Lower Extremity Salvage in a Diabetic Patient With Cutaneous Mucormycosis and COVID-19 After Open Patella Fracture.

Background: Cutaneous mucormycosis, while less common than sinonasal or pulmonary infections, can cause widespread tissue necrosis after seemingly innocuous encounters. The most common location of cutaneous mucormycosis is the extremities, and extensive infection has been reported after trauma or orthopedic procedures. Case Report: A 60-year-old female with poorly controlled type 2 diabetes mellitus sustained an open patella fracture after a fall. She underwent washout and internal fixation with cannulated screws and cable tension band wiring. The patient's recovery was complicated by asymptom…

Finite Element Analysis of Patellofemoral Contact Pressure with Varying Fixation of Transverse Patella Fractures.

BACKGROUND: Understanding patellofemoral contact pressure is crucial for knee biomechanics, as abnormalities can lead to joint issues. This study investigates the changes in contact pressure after surgical fixation of patellar fractures. METHODS: Finite element model of the knee was created using cadaveric data on transverse patellar fractures. Models were analyzed and evaluated at 0°, 45°, and 90° flexion under quadriceps force to assess peak pressure, contact pattern, and pressure irregularity. RESULTS: The plate and wire fixation models exhibited distinct stress distribution patterns at 4

Reconstruction of patella fractures with the tension band technique: A review on clinical results and tips and tricks.

INTRODUCTION: The goals of surgical treatment of patellar fractures are a biomechanically stable joint and congruent restoration of the retro patellar joint surface. Surgical treatment strategies for patellar fractures have evolved from tension band in combination with wire cerclages to new devices. METHODS: The modified anterior tension band (MATB) technique for fixation of patellar fractures consists of two longitudinal 1.8 mm Kirschner wires (K-wires) and an 18-gauge stainless steel wire looped in a figure-of-8 pattern over the anterior aspect of the patella. The K-wires should be inserted

Biomechanical Evaluation of Suture Augmentation of Dorsal Locking Plate Fixation in Transverse Patella Fractures.

INTRODUCTION: Adjunctive suture augmentation of patellar plate fixation has yet to be investigated. Our biomechanical study sought to evaluate whether suture augmentation improves dorsal patellar locking plate fixation. Our hypothesis was that suture augmentation would improve fixation of this construct. METHODS: A transverse patella fracture in six matched cadaveric pairs was stabilized using a patellar locking plate with or without suture augmentation. Specimens were tested at 60° knee flexion with load placed through quadriceps. Cyclic loading followed by load to failure was done. Stiffnes…

An innovative compression-clamp for open contaminated fractures of patella: a short report of five cases.

Five patients with open contaminated fractures of the patella were managed using an indigenously designed external clamp cum compressor. All had an infected lacerated wound at the fracture site, and one had open exposed knee joint (Gustilo grade IIIB open fracture). These patients reported after 3-7 days of injury. The wounds were thoroughly washed and débrided and the clamp cum compressor was applied. They were encouraged to do active knee movements immediately post-operatively. The wounds healed within 3-4 weeks, and the fractures united within 10-16 weeks. Good functional results were achie

Functional Outcome of Extensor Mechanism Salvage in Patella Fracture Non-union Using Krackow Sutures: A Case Report.

Patellar fractures can lead to extensor mechanism dysfunction if not repaired properly, impacting knee function and mobility, and this complication can be challenging to manage, especially in cases where previous surgical interventions have failed. The aim of this study was to evaluate the functional outcome of the salvaged extensor mechanism in patella fracture non-union using Krakow sutures in a 61-year-old female patient who presented with knee pain, reduced flexion, and a 40-degree extension lag after two previous patellar fracture surgeries using tension band wiring resulted in non-union.…

Biomechanical comparison of a novel tensioned cable construct versus tension band wiring for transverse patella fracture fixation.

PURPOSE: Tension band wiring (TBW) is the most widely accepted method for patella fracture fixation. The purpose of our study was to compare the biomechanical efficacy of a novel cable construct to TBW for the fixation of transverse patella fractures. The tensioned cable construct was hypothesized to have less fracture gapping after cyclic flexion-extension loading and greater ultimate load to failure as compared to TBW. METHODS: Transverse patellar osteotomies (AO/OTA 34C1.1) were performed on nine pairs of fresh-frozen human cadaveric whole legs (mean age 82.2 years, range 71-101). Treatmen…
