The Involvement of the Endocannabinoid, Glutamatergic, and GABAergic Systems in PTSD.

Post-traumatic stress disorder (PTSD) is a debilitating mental health condition that develops in response to traumatic events. The endocannabinoid, glutamatergic, and GABAergic systems play crucial roles in the neurobiological mechanisms of PTSD. Both the endocannabinoid, glutamatergic, and GABAergic systems are involved in synaptic remodeling and neuronal differentiation, ensuring efficient information transmission in the brain. Their interplay influences motivation, behavior, sensory perception, pain regulation, and visual processing. Additionally, these systems regulate processes such as ce

NAMING CONVENTIONS FOR UNIDENTIFIED PATIENTS IN EMERGENCY AND TRAUMA SETTINGS: A NARRATIVE REVIEW.

Unidentified patients pose unique challenges in emergency and trauma care, where rapid registration and treatment initiation are essential despite the absence of formal identification. Naming these patients is critical for safe clinical care, communication, and legal documentation. This narrative review synthesizes the literature on who unidentified patients are, the issues associated with naming them, and the naming systems currently used. Studies consistently describe these patients as predominantly young males presenting with trauma, substance misuse, or altered consciousness, often from so

A Comprehensive Review of Strategies to Enhance Quality and Safety in Infertility Laboratories.

Assisted reproductive technology (ART) laboratories are complex environments in which human gametes and embryos are handled through highly sensitive procedures. Because of procedural complexity, manual documentation, high workload and the need for precise identification at ev-ery step, these laboratories are inherently vulnerable to er-rors that may compromise patient safety, clinical outcomes and confidence in ART. Potential hazards may arise from equipment malfunction, human error, environmental insta-bility or deviation from validated procedures. Therefore, a comprehensive framework for ris

Care Needs of Burn Patients: A Meta-Synthesis Guided by Orem's Self-Care Deficit Nursing Theory.

INTRODUCTION: A burn patient requiring admission to a Burn Unit undergoes a challenging treatment process whose physical, emotional and social consequences can result in self-care deficits. In this regard, Orem's Self-Care Deficit Nursing Theory (SCDNT) is utilized to frame and systematize nursing interventions according to the identification of the care needs of the hospitalized burn patient. AIM: This study aimed to identify and synthesize the care needs of burn patients during hospitalization, using Orem's Self-Care Deficit Nursing Theory as a theoretical framework. METHODOLOGY: A meta-sy

Patient access and 1-year outcomes of percutaneous coronary intervention facilities with and without on-site cardiothoracic surgery: insights from the Veterans Affairs (VA) Clinical Assessment, Reporting, and Tracking (CART) program.

BACKGROUND: The safety of percutaneous coronary intervention (PCI) at medical facilities without on-site cardiothoracic (CT) surgery has been established in clinical trials. However, the comparative effectiveness of this strategy in real-world practice, including impact on patient access and outcomes, is uncertain. The Veterans Affairs (VA) health care system has used this strategy, with strict quality oversight, since 2005, and can provide insight into this question. METHODS AND RESULTS: Among 24,387 patients receiving PCI at VA facilities between October 2007 and September 2010, 6616 (27.1%

[Blindness after nitrous oxide anesthesia and internal gas tamponade].

The authors describe the case of a patient with bilateral retinal detachment. Immediately after surgery for the second eye under general anesthesia with nitrous oxide, the patient reported severe visual loss in the first eye successfully treated surgically 2 weeks before, with a residual gas bubble (C3F8) of 50%. In this case, nitrous oxide had rapidly entered the gas bubble and induced a transient expansion of the gas tamponade with a dramatic increase in intraocular pressure. The consequence was a central retinal artery occlusion, which resulted in irreparable ischemic retinal damage and bli

How good is the coverage and how accurate are exposure data in the Swedish Biologics Register (ARTIS)?

OBJECTIVES: To assess the coverage of the Swedish Biologics Register (Anti-Rheumatic Therapy in Sweden, ARTIS) across indications, and the accuracy of the registered information on treatment with biologics. METHOD: Through cross-reference of ARTIS to almost complete national health registers on prescriptions (adalimumab and etanercept), outpatient visits, and death/residency during 2008-2010, we assessed: the treatment coverage of ARTIS for each treatment indication, the validity of the registered start and stop dates, ARTIS treatments with no corresponding drug dispensations, and the accurac

Phlebotomy issues and quality improvement in results of laboratory testing.

Laboratory testing is an integral part of the decision-making process, and results of laboratory testing often strongly influence medical diagnoses and therapies. There is a long history of quality requirements in laboratory medicine, which have mainly concerned the analytic phase of this process. Owing to the substantial advances in technology, laboratory automation and analytic quality, there is increasing evidence that further quality improvements should be targeted to extra-analytic phases of laboratory testing. Objective difficulties to monitor most of the preanalytic variables which lie

High-Sensitivity In Situ Hybridization DNAscope for Detection of Papillomavirus or Epstein-Barr Virus Genomes Combined with Immunofluorescent Staining in Adherent Cells.

Hybridization methods have been broadly used for the observation of DNA of different origins. Herein, we describe the highly effective and specific in situ DNA hybridization protocol DNAscope for the identification of viral genomes in cells growing in monolayer cultures. The procedure utilizes the Z-probes and hybridization-based fluorescent detection systems available from Advanced Cell Diagnostics, ACD (Newark, CA, USA). We developed our protocol for the identification of episomal and integrated human papillomavirus (HPV) or Epstein-Barr virus (EBV) DNA in human keratinocytes. This procedure

A review of Prader-Willi syndrome.

Prader-Willi syndrome is a rare and complex genetic disorder with multiple physical and behavioral characteristics, affecting endocrine, metabolic, and neurologic systems and producing a plethora of medical complications. Early identification and diagnosis are paramount to providing timely and appropriate interventions to improve patient outcomes. Treatment should focus on neonatal feeding and growth, followed by hormonal therapy for hypothalamic dysfunction, and should then be directed at the prevention and treatment of obesity and obesity-related complications. Effective treatment requires a

The functional anatomy of the female pelvic floor and stress continence control system.

This paper provides an overview of the functional anatomy of the structures responsible for controlling urinary continence under stress. The stress continence control system can be divided into two parts: the system responsible for bladder neck support, and the system responsible for sphincteric closure. Age- and injury-related changes in each of these systems are discussed. Understanding the pathophysiology of incontinence on the anatomical level will help to lead to identification of specific defects, thereby allowing better individualized treatment for the incontinent patient.

Arrhythmias in the office.

The incidence of patients who present to the office with arrhythmia and hemodynamic instability is unknown. Emergency medical systems data, based on ambulance runs, are available only for patients who have had a cardiac arrest. When faced with an unstable or potentially unstable patient, however, we must be prepared to act quickly, safely, and accurately. This article addresses the general approach to such a patient; provides necessary information on office emergency preparation, including training, rapid response team protocol, and the use of automated external defibrillators; and addresses t

An Innovative Patient Stratification Tool Integrating Clinical and Economic Data for Benchmarking Oncology and Hematology Care: The PATONCOS System.

Background: The growing complexity and cost of oncohematological treatments has created an urgent need for standardized methodologies capable of enabling inter-institutional comparisons of healthcare expenditure within homogeneous patient groups. Cancer-related pharmaceutical costs vary substantially depending on tumour type, disease stage, and therapeutic approach, making cross-institutional benchmarking challenging due to heterogeneity in patient populations and clinical practice patterns. Therefore, integrating cost analysis with clinically meaningful patient stratification is essential to

Performance of common primary and chromogenic culture media for MALDI-TOF MS identification of clinically relevant yeasts.

Timely and accurate identification of yeasts is essential for adequate treatment, considering the increase in antifungal resistance of some species, particularly for C. auris. Current matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) manufacturer's protocol for identification of yeasts requires 24- to 72-h cultivation on Sabouraud dextrose media (SAB), but not some of the mainstay primary culture media used in mycology such as inhibitory mold agar (IMA), Mycosel, CHROMagar Candida Plus, and CHROMagar Candida. As culture media can influence MALDI-TOF MS

Security infrastructure requirements for electronic health cards communication.

Communication and co-operation processes in the healthcare and welfare domain require a security infrastructure based on services describing status and relation of communicating principals as well as corresponding keys and attributes. Additional services provide trustworthy information on dynamic issues of communication and co-operation such as time and location of processes, workflow relations, integrity of archives and record systems, and system behaviour. To provide this communication and co-operation in a shared care environment, smart cards are widely used. Serving as storage media and po

Implementation of Fingerprint Technology for Unique Patient Matching and Identification at an HIV Care and Treatment Facility in Western Kenya: Cross-sectional Study.

BACKGROUND: Unique patient identification remains a challenge in many health care settings in low- and middle-income countries (LMICs). Without national-level unique identifiers for whole populations, countries rely on demographic-based approaches that have proven suboptimal. Affordable biometrics-based approaches, implemented with consideration of contextual ethical, legal, and social implications, have the potential to address this challenge and improve patient safety and reporting accuracy. However, limited studies exist to evaluate the actual performance of biometric approaches and percept

Advances in Biomechanical Parameters for Screening of Refractive Surgery Candidates: A Review of the Literature, Part III.

Corneal biomechanical properties have garnered significant interest in their relation to the development of ectatic corneal disease. Alongside the advent of corneal tomography and Scheimpflug imaging such as Pentacam and Galilei, there have been advances in assessing the cornea based on its biomechanical characteristics. Though the aforementioned imaging systems are highly capable of identifying morphologic abnormalities, they cannot assess mechanical stability of the cornea. This article, in contrast to Parts I and II of this article series, will focus on in vivo corneal biomechanical imaging
