Physical Examination Discovered Prostate Cancer Metastasis to the Testis: A Case Report.

BACKGROUND Prostate cancer is the most common non-cutaneous cancer in men. While approximately three-quarters of all cases present as localized disease, the rate of metastatic disease has been increasing. Common sites of metastatic prostate cancer include regional lymph nodes, bones, and lungs. In this case report, we discuss a man with a history of low-risk prostate cancer who developed a testicular mass, which was ultimately diagnosed as a solitary testicular metastasis. CASE REPORT An abnormal nodule on the left apex area was identified on a digital rectal exam of an otherwise healthy 67-ye

Fifteen-minute consultation: Cardiac murmurs in the Newborn Infant Physical Examination (NIPE).

The finding of a cardiac murmur on the initial newborn examination is common but may be a source of anxiety for practitioners due to worries about missing critical congenital heart defects (CHDs). This article aims to provide an approach to this common finding, in particular, reviewing the evidence base behind features of the history, examination and subsequent non-specialist investigations which may increase the likelihood of CHDs. The aim of this structured approach is to give clinicians confidence in dealing with this common clinical finding, to be able to pick out those infants most at ris

A superior vena cava aneurysm discovered by chance at regular physical examination.

A superior vena cava (SVC) aneurysm is a rare disease that can be confused with upper mediastinal tumor. A 57-year-old female visited our hospital regarding an abnormal shadow in her mediastinum on a chest X-ray. Upon closer examinations, which included three-dimensional computed tomography, we diagnosed it as a SVC aneurysm. Since her SVC aneurysm was regarded as fusiform type at low risk of rupture and thrombosis, she has been managed conservatively and is free from any complications to date. Thus, it is important to keep SVC aneurysms in mind during routine examinations.

Association between Patient History and Physical Examination and Osteoarthritis after Ankle Sprain.

UNLABELLED: Structural abnormalities on MRI are frequent after an ankle sprain. To determine the association between patient history, physical examination and early osteoarthritis (OA) in patients after a previous ankle sprain, 98 patients with persistent complaints were selected from a cross-sectional study. Patient history taking and physical examination were applied and MRI was taken. Univariate and multivariable analyses were used to test possible associations. Signs of OA (cartilage loss, osteophytes and bone marrow edema) were seen in the talocrural joint (TCJ) in 40% and the talonavicul…

Phenotyping chronic pelvic pain based on latent class modeling of physical examination.

Introduction. Defining clinical phenotypes based on physical examination is required for clarifying heterogeneous disorders such as chronic pelvic pain (CPP). The objective of this study was to determine the number of classes within 4 examinable regions and then establish threshold and optimal exam criteria for the classes discovered. Methods. A total of 476 patients meeting the criteria for CPP were examined using pain pressure threshold (PPT) algometry and standardized numeric scale (NRS) pain ratings at 30 distinct sites over 4 pelvic regions. Exploratory factor analysis, latent profile ana…

Does patient history and physical examination predict MRI proven cauda equina syndrome?

STUDY DESIGN: Systematic review. STUDY RATIONALE: While magnetic resonance imaging (MRI) is used as the diagnostic gold standard for cauda equina syndrome (CES), many MRI scans obtained from patients presenting with signs and/or symptoms of CES do not reveal concordant pathology. As a result, the role of the history and physical examination remains unclear when determining which patients require emergent MRI.Objective or clinical question: Are there elements from the history or physical examination that are associated with CES as established by MRI? METHODS: A systematic review of the litera…

Identifying the patient with sleep apnea: upper airway assessment and physical examination.

Since the final common pathway for obstructive sleep apnea is obstruction of the upper airway during nocturnal respiration, examination and assessment of the anatomy of the upper airway plays a central role in patient evaluation. Since the upper airway begins at the nose and lips and ends at the larynx, a complete assessment of the upper airway evaluates this entire length of this anatomic region including the bony framework and soft tissue. Though office assessment of these structures does not necessarily mimic the appearance of behavior of these structures during physiologic sleep, the offic

Examining the Relationship Between Physical Function and Anxiety/Depression in Parkinson's.

BACKGROUND: Parkinson's disease (PD) is a complex neurological disorder characterized by both motor and nonmotor symptoms, including tremor, muscle stiffness, anxiety, and depression. OBJECTIVES: The primary aim of this study was to examine the relationship between physical function and psychological symptoms, specifically anxiety and depression, in people with Parkinson's (PwP). The secondary aim was to explore whether any discrepancies between participant-reported and clinician-rated measures of physical function exist. METHODS: This study utilized the Parkinson's Progression Markers Initi

Physical examinations for older adults with shoulder pain: a narrative review.

Shoulder pain is a common complaint in primary care settings. The prevalence of shoulder pain is on the rise, especially in societies with aging populations. Like other joint-related conditions, shoulder pain is predominantly caused by degenerative diseases. These degenerative changes typically affect bones, tendons, and cartilage, with common conditions including degenerative rotator cuff tears, impingement syndrome, and osteoarthritis. Diagnosing these degenerative diseases in older adults requires a thorough understanding of basic anatomy, general physical examination techniques, and specif…

Routine history and physical examinations prior to cataract surgery are not beneficial.

OBJECTIVE: To determine the safety of a policy change eliminating the requirement for preoperative history and physical examination before cataract surgery. DESIGN: A retrospective population-based study. METHODS: Because preoperative history and physical examination were no longer required in Alberta after April 1, 2017, a retrospective review of provincial health claims data was conducted to determine the safety of removing this requirement. Data from Albertans who underwent cataract surgery and had a preoperative medical examination between April 2014 and March 2017 were compared with dat…

Regularly scheduled physical examinations and the detection of breast cancer recurrences.

PURPOSE: Follow-up care of early breast cancer (EBC) patients usually includes routinely scheduled physical examinations. While ASCO guidelines recommend a physical exam every three to six months for the first three years, little evidence supports this schedule. We evaluated recurrence detection of patients transferred into a single centre survivorship program that follows ASCO recommendations. METHODS: Patients with EBC referred to the Wellness Beyond Cancer Program (WBCP) who had breast cancer recurrence between February 1, 2013, and January 1, 2019 were reviewed. Descriptive analyses were …

Early Diagnosis of Anal Canal Duplication: The Importance of a Physical Examination.

Anal canal duplication (ACD) is an extremely rare congenital anomaly of the intestinal tract that presents as an extra opening of the anal canal without communication with the anorectum. We present the case of a five-year-old male presenting to the pediatrician without symptoms and upon physical examination, a duplicated anal canal along the midline was discovered. The patient was admitted for surgery and the canal was removed via mucosal stripping. Postoperatively, the patient recovered well. The present study aims to expand on our knowledge of a very rare pathological entity and emphasize th…

Role of patient history and physical examination in the diagnosis of trigeminal neuralgia.

The diagnosis of facial pain has been a source of confusion for neuroscientists and primary care givers alike. The profusion of various subtypes, differential syndromes, and confusing nomenclature is silent testimony to this dilemma. The author presents a simple scheme with which to arrive at the diagnosis. The use of the patient's history, confirmed by the physical examination, can be supplemented with some of the tests described herein.
