[A case of neurosarcoidosis with swelling and gadolinium enhancement of spinal nerve roots on magnetic resonance imaging].

An 80-year-old woman was admitted to our hospital because of developed sense of constriction in the trunk and gradually progressive numbness … abnormalities on MRI. It is important to consider neurosarcoidosis in the differential diagnosis of polyradiculopathy with swelling and gadolinium enhancement of spinal nerve roots.

Detection of herpesvirus DNA by nested polymerase chain reaction in cerebrospinal fluid of human immunodeficiency virus-infected persons with neurologic disease: a prospective evaluation.

A nested polymerase chain reaction-based method was used prospectively to detect herpesvirus DNA in cerebrospinal fluid (CSF) from 111 patients with AIDS, 39 of whom had a suspected diagnosis of cytomegalovirus (CMV)-associated neurologic disease (patients with encephalopathy, polyradiculopathy, or peripheral neuropathy) and 72 who had alternative diagnoses. CSF from 24 (62%) of the patients with suspected CMV-associated disease had detectable CMV DNA compared with only 8 (11%) of the patients with other diagnoses. Varicella-zoster virus DNA was detected in CSF from 3 patients (2 with myelitis…

Nature and incidence of peripheral nerve syndromes in HIV infection.

Fifty four patients with peripheral nerve syndromes were seen during a 15 month period in a population of about 1500 HIV infected patients a … infection at other sites. The 13 non-painful neuropathies seen were more heterogeneous. Lumbosacral polyradiculopathy associated with cytomegalovirus and lymphomatous mononeuritis multiplex occurred in fewer than 1% of AIDS patients. Mononeuropathies were seen in 3% of AIDS patients. No patients with acute or chronic inflammatory demyelinating polyradiculoneuropath …

Postpartum lumbosacral radiculopathy: a neuraxial anaesthesia complication or an intrinsic obstetric palsy?

Postpartum peripheral nerve disorders are an uncommon obstetrical complication, with most cases resulting from intrinsic obstetric palsies. … lowing 2 months. Although a definitive aetiological diagnosis could not be confirmed, a lumbosacral polyradiculopathy due to intrinsic obstetric complication was considered. The patient presented several risk factors known to be associated with intrinsic obstetric palsies. Obstetricians and anaesthesiologists have an important role in preventing and diagnosing post …

Myelopathy: chameleons and mimics.

The diagnosis of spinal cord disease may be delayed or missed if the presentation does not conform to the expected pattern of a symmetrical … ronic setting with some high cervical cord lesions. A second pathology, such as a polyneuropathy or polyradiculopathy, can mask the presence of a myelopathy. The converse situation, of non-myelopathic disease mimicking a cord lesion, arises typically when symptoms and/or signs approximate bilateral symmetry. This may happen with certain diseases of the brain, or of …

Acute nutritional axonal neuropathy and Wernicke's encephalopathy status post Roux-en-Y gastric bypass.

We describe an unusual case of a woman in her 30s who had recently undergone Roux-en-Y gastric bypass and presented to the emergency departm … howed no albuminocytological dissociation. Electrodiagnostic studies showed evidence of lumbosacral polyradiculopathy. The patient was eventually diagnosed with thiamine deficiency-related acute nutritional axonal neuropathy and Wernicke's encephalopathy. After aggressive thiamine supplementation and 8 weeks of physical rehabilitation, her leg strength improved, bu …

Hepatitis E virus infection as a direct cause of neuralgic amyotrophy.

INTRODUCTION: We describe a patient who developed neuralgic amyotrophy (NA) related to hepatitis E virus (HEV) infection. METHODS: The patient underwent neurological and electrodiagnostic examinations, high-resolution analysis of serological changes, and HEV load profile, and was treated with intravenous immunoglobulin. RESULTS: There was evidence of bilateral, asymmetric acute inflammatory cervical polyradiculopathy and possible brachial plexopathy. Positive serum anti-HEV IgM was followed by seroconversion to anti-HEV IgG positivity. A calculated anti-HEV antibody index was compatible with…

Polyneuropathy: a poetic diagnosis.

A 60-year-old woman presented with subacute progressive lower extremity weakness. This was associated with a 40lb weight loss and fevers. She was previously healthy, aside from renal transplantation 9 years earlier for end stage renal disease resulting from IgA nephropathy. On examination, she had clubbing, a single lymph node in the right cervical chain, and objective findings of lower extremity weakness with lower motor neuron findings. Investigations revealed a thrombocytosis and elevated CSF protein. Electromyographic evidence of diffuse polyradiculopathy with demyelination was documented.…

Mononeuritis multiplex. A harbinger of acute leukemia in relapse.

OBJECTIVE: To report the findings in a patient who developed severe mononeuritis multiplex in the setting of hematologic remission from acut … nt of the cauda equina or many of the proximal nerves. CONCLUSION: Mononeuritis multiplex, without polyradiculopathy or diffuse peripheral neuropathy, can be a presenting feature of leukemia.

British Association of Spine Surgeons standards of care for cauda equina syndrome.

This group of articles looks at the BASS guidelines for CES. TG and AC gave us the background on the long journey taken in publishing this, SA summarized the forum discussion on the BASS Web site, and NT gave us a medicolegal comment. The guidelines are concise, highlighting the need for prompt MRI scanning and as a consequence emergency surgery in appropriate cases. This has resource implication in terms of MRI availability and a comprehensive spinal on-call system. The question of whether operating "in the small hours" carries increased risk or whether we are using this as an excuse not to g

Primary Epstein-Barr virus infection with polyradiculitis: a case report.

BACKGROUND: Nervous system complications of primary Epstein-Barr virus (EBV) infection in adults are rare, but may occur with encephalitis, meningitis, myelitis, cranial and peripheral neuropathies, or radiculitis. CASE PRESENTATION: We describe an immune competent adult with a primary EBV infection complicated by lumbosacral polyradiculitis with pure radicular pain. Prior to the onset of radicular pain the 35-year-old woman had been suffering from infectious mononucleosis misdiagnosed for streptococcal tonsillitis. The diagnosis of primary EBV infection associated polyradiculitis was proven

Facial myokymia in the Guillain-Barré syndrome.

Facial myokymia, a unique involuntary movement of facial muscles, is described in a patient with Guillain-Barré syndrome. Although this involuntary movement is most often described with intramedullary lesions of the brain stem, it may also appear with lesions external to the neuraxis. We review the literature and propose a more widespread distribution of potential lesions along the facial nerve pathway, which may produce facial myokymia.
