OBJECTIVE: To illustrate that Acremonium and Penicillium species are being increasingly recognised in periprosthetic joint infections (PJIs).
CASE REPORT: A 47-year-old male with liver cirrhosis and bilateral hip hemiarthroplasties complained of persistent right hip pain. Bone scan was suspicious for right hip septic arthritis. Laboratory studies revealed an elevated erythrocyte sedimentation rate (ESR; 127 mm/h) and C-reactive protein (CRP; 16.171 mg/L). At surgery caseous necrosis with a yellowish appearance around the hip was noted. Intraoperative cultures were positive for Acremonium and

Lactococcus garvieae is usually an animal pathogen. Only a few cases of infections in humans have been described. We describe a case of an elderly patient with prosthetic heart valves with a septicaemia without infective endocarditis, and with a favourable clinical course.

Actinomyces neuii has until now not been described as a pathogen associated with periprosthetic infection in total joint replacement. The case presented here suggests that A. neuii subsp. neuii is a causative pathogen. The discussion and review of the literature indicate the impact that detection of Actinomyces species could have.

In patients with osteoarthritis or arthritis, prosthetic joint replacement is increasingly used to alleviate pain and to improve mobility. The most important risk factors are comorbidity and prior joint replacement (revision surgery). Diagnosis of prosthetic-joint-associated infection is difficult, because the infecting agent may be missed in synovial fluid due to its exclusive presence as a device-associated biofilm. Implant-associated infections are difficult to treat because of their resistance to natural host defence mechanisms and to most antibiotics. In staphylococcal implant-associated

Over the last three decades total hip replacement became one of the most common surgical procedures in orthopaedic surgery. According to the number of large joint endoprosthesis, hip replacement is on the first place. Lately, the number of revisional and special tumoral endoprosthesis is increasing, with more severe complications. Dislocation is a leading early complication of total hip arthroplasty. Dislocations after primary total hip arthroplasties occur at an overall incidence of 1-3%, and at 15-20% in revision and tumoral procedures. Closed reduction and eventually immobilization is the m

Late infections with Actinomyces israelii have been described for prosthetic hip joints but not in association with intravenous drug use. We present a case of a 43-year-old intravenous drug user who developed A. israelii infection in connection with a hip prosthesis 11 years after implantation, and we review four previously reported cases of Actinomyces prosthetic joint infections.

Six thousand four hundred eighty-nine knee replacements were done in 6120 patients at the authors' institution between 1993 and 1999. Operations were done in a theater with vertical laminar flow and with the surgical team using body exhaust suits. Of these knee replacements, 116 knees became infected and 113 were available for followup. One hundred of the infections occurred in patients undergoing primary knee replacement, whereas the remaining infections occurred in patients undergoing revision knee replacement. Ninety-seven of these knees (86%) had deep periprosthetic infections and the rema

A case of Listeria monocytogenes septicaemia and infection of a prosthetic hip joint is presented in a patient with non-insulin-dependent diabetes mellitus. The patient developed the infection five and a half years after primary alloplastic operation. The prosthesis was removed and a temporary prosthesis with gentamicin was placed ("spacer"). The antibiotic treatment included ampicillin, pivampicillin and sulfamethoxazole-trimetroprim. Six weeks after removal of the prosthesis an un-cemented prosthesis was placed, and the patient had a full functional recovery.

Despite major advances in cardiovascular surgical techniques and routine use of prophylactic antimicrobial agents, prosthetic valve endocarditis (PVE) continues to complicate the course of a small percentage of patients after cardiac valve replacement. Using actuarial methods to describe the risk of PVE after valve implementation, several studies have shown that its incidence peaked at around 5 weeks and levelled off to a stable rate by 12 months, for a cumulative risk of 3% at that time. The microbial aetiology of early PVE is dominated by staphylococcal species, S. epidermis and S. aureus ac

Prosthetic vascular graft infection in the thoracic aortic area is a rare but serious complication. Adequate management of the complication is essential to increase the chance of success of open surgery. While surgical site infection is suggested as the root cause of the complication, it is also related to decreased host tolerance, especially as found in elderly patients. The handling of prosthetic vascular graft infection has been widely discussed to date. This paper mainly provides a summary of literature reports published within the past 5 years to discuss issues related to multidisciplinar

Infection after a total hip arthroplasty is a severe complication and is associated with a high incidence of morbidity. We describe a case of late infection, 5 years after the implantation of a cemented hip prosthesis. The infection was caused by gram-negative Brucella melitensis and occurred in a 63-year-old man who owned cattle. As far as we know, such a complication has never been published before.

BACKGROUND: This video article describes the use of a bone-anchored prosthesis in patients with high above-the-knee amputations resulting in short residual limbs, most typically from trauma, cancer, infections, or dysvascular disease. The use of a socket prosthesis is usually unsuccessful in patients with a high transfemoral amputation because such prostheses have an unstable connection and often require additional waist belts for better attachment to the short residual limb. In most cases, a bone-anchored prosthesis results in substantial improvements in wear time, mobility, and quality of li

BACKGROUND: Periprosthetic joint infection (PJI) is one of the most devastating complications of total joint arthroplasty. Given the mortality and morbidity associated with PJI and the challenges in treating it, there has been increased interest in risk factors that can be modified before surgery. In this study, we used a novel mouse model to consider the role of the gut microbiome as a risk factor for PJI.
QUESTIONS/PURPOSES: (1) Does the state of the gut microbiota before surgery influence the likelihood of developing an established infection in a mouse model of PJI? (2) How does the state

BACKGROUND: The purpose of this study was to evaluate the usefulness of sonication technique for microbiological diagnosis and the sterility of the recycled autoclaved femoral components from infected total knee arthroplasty (TKA) using a sonication method.
METHODS: Nineteen femoral implants explanted from patients with infected TKA were sterilized with a standard autoclave method. Standard culture of the fluid before and after sonication of the sterilized implants was performed to detect pathogenic microorganisms. Additional experiments were performed to evaluate the sterility of the recycle

Infection of a total knee arthroplasty with Streptococcus bovis in a 76-year-old man that led to the diagnosis of a bowel carcinoma is reported. Investigation revealed a malignancy in the ascending colon with extension into the adrenal gland. S bovis in conjunction with colonic neoplasia has been reported in several orthopedic conditions: vertebral osteomyelitis, discitis, lateral neck abscess, and osteomyelitis of the ileum. The relationship of S bovis to endocarditis, meningitis, brain abscesses, and peritonitis has also been well described. However, S bovis is a rare pathogen infecting join

Serratia marcescens is a rare cause of musculoskeletal infections and osteomyelitis occurring most frequently in nosocomial infections, debilitated patients, drug addicts, and traumatic open wounds. It is rarely a cause of postoperative infection in elective surgery with only a few such cases being reported in the United States and European literature. The authors present a case of Serratia marcescens infection of a total joint implant from an apparent perioperative source and review the literature concerning this organism as a cause of infection in man.
