OBJECTIVE: To assess the function of the pylorus after pylorus-preserving pancreaticoduodenectomy (PPPD) done for periampullary or pancreatic cancer.
DESIGN: Prospective, observational controlled clinical study.
SETTING: Teaching hospital, Italy.
SUBJECTS: 17 patients who had undergone PPPD, and 15 healthy control subjects.

From 1984 to the end of 1993, we performed 51 duodenopancreatectomies with pylorus preservation (27 for chronic pancreatitis and 24 for tumours). We have a complete follow up for 96% of our patients with a mean range of 76 months in the first group and 33 months in the second. There was no per- or postoperative mortality and 15% immediate postoperative morbidity. We observed 5 cases (10%) of anastomotic ulceration. DESIGN: Retrospective review of the clinical records of patients undergoing PPPD for malignant diseases of the periampullary region. The motility index (MI) and contractile frequency were determined for the gastric antrum, pylorus, and duodenum during digestive and interdigestive states in eight dogs before and after resection of the ramus pyloricus nervi vagi (NR) and after selective proximal vagotomy (SPV) subsequent to NR. We report here a case of double pylorus diagnosed endoscopically in a patient with adrenal adenoma. A 44-yr-old man was found to have double pylorus during examinations for recurrent epigastric pain and obesity. We think that the formation of double pylorus in this case resulted from recurrent peptic ulcer which, in turn, probably was induced by the hypersecretion of endogenous corticosteroids accompanying adrenal adenoma.

Intravenous histamine causes high amplitude repetitive phasic contractions of the in vivo cat pylorus but has little effect on the antrum and duodenum. The genesis of this phasic response was studied using a pinned perfused catheter with openings at the pylorus, antrum, and duodenum. 2-Pyridylethylamine, an H1 agonist, produced phasic contractions similar to histamine whereas dimaprit, an H2 agonist, did not.
