Using a Latent Variable Method to Develop a Composite, Multidimensional Measure of Structural Racism at the City Level.

INTRODUCTION: Although structural racism is strongly related to racial health disparities, we are not aware of any composite, multidimensional measure of structural racism at the city level in the United States. However, many of the policies, programs, and institutions that create and maintain structural racism are located at the city level. To expand upon previous research, this paper uses a novel measure to measure structural racism at the city level for the non-Hispanic Black population. METHODS: We used confirmatory factor analysis to model the latent construct of structural racism for 77…

Structural Racism and Triple-Negative Breast Cancer Among Black and White Women in the United States.

Purpose: To determine the associations between state-level indicators of structural racism and incidence of triple-negative breast cancer (TNBC) among black and white women diagnosed with breast cancer. Methods: Black and white women diagnosed with breast cancer between 2010 and 2016 were identified from 12 states represented in the Surveillance, Epidemiology, and End Results (SEER18) program. State-level disparities were measured by black to white rate ratios in educational attainment, political participation, incarceration, and unemployment; and dichotomized to "high" or "low" structural rac…

Complex effects of racism and discrimination on African Americans' health and well-being: Navigating the status quo.

Most research on the effects of racism and discrimination on the health and well-being of African Americans utilize a deficit perspective, one that homogeneously paints African Americans as disadvantaged victims. Such approaches do little to highlight the variability in the effects of racism and discrimination on relevant outcomes, and the resources that African Americans have drawn upon to navigate an environment characterized by varying levels of racialized hostility. The goal of this special issue is to inspire more refined conceptualizations of how African Americans navigate an often-hosti…

Structural Racism and Oral Health Inequities of Black vs. non-Hispanic White Adults in the U.S.

Structural racism negatively affects the health of Black populations in the U.S. Black populations experience a higher burden of oral diseases, such as tooth decay, periodontal disease, and oral and pharyngeal cancers than other racial groups experience. Oral health literature refers to racial inequities in the context of social disadvantage. However, structural racism perpetuates those contributory social disadvantages, such as inadequate access to affordable housing, education, and employment. In addition, in states where nearly 50% of U.S. Black populations reside, there is an inequitable d…

Blindness, deafness, silence and invisibility that shields racism in nursing education-practice in multicultural hubs of immigration.

BACKGROUND: This paper relies on data from the multilevel, sectoral discussions held among professional nursing and sociology individuals. PURPOSE: To present the outcomes of a reflective process on racism in nursing education and practice in the cities of Toronto and Paris. METHOD: The method used was a reflection on research conducted by eight individuals dealing with racism at distinct stages of their professional career. The reflections are organized as a systematic description of facts, noted feelings, appraisal of issues, analysis of learned lessons, and lead to recommendations for nur…

The Role of Nurses as Allies Against Racism and Discrimination: An Analysis of Key Resistance Movements of Our Time.

The remnants of colonialism manifesting as structural violence, racism, and oppression continue to plague our society as evidenced by the persistence of health inequities, particularly for minority populations in the United States. As a profession bound by moral and ethical mandates, nursing must resist and deconstruct oppression in all its forms. Nurses, informed by critical race theory, intersectionality, and historical trauma, can become formidable allies with marginalized populations in the fight for social justice and health equity.

The Relationship Between Structural Racism and Black-White Disparities in Fatal Police Shootings at the State Level.

OBJECTIVE: The objective of this study was to discern the relationship between state-level structural racism and Black-White disparities in police shootings of victims not known to be armed. METHODS: Using a Poisson regression, we evaluated the effect of structural racism on differences between states in Black-White disparities in fatal police shootings involving victims not known to be armed during the period from January 1, 2013 through June 30, 2017. We created a state racism index, which was comprised of five dimensions: (1) residential segregation; and gaps in (2) incarceration rates; (3…

A hierarchical bayesian analysis of parasite prevalence and sociocultural outcomes: The role of structural racism and sanitation infrastructure.

OBJECTIVES: We conduct a revaluation of the Thornhill and Fincher research project on parasites using finely-resolved geographic data on parasite prevalence, individual-level sociocultural data, and multilevel Bayesian modeling. In contrast to the evolutionary psychological mechanisms linking parasites to human behavior and cultural characteristics proposed by Thornhill and Fincher, we offer an alternative hypothesis that structural racism and differential access to sanitation systems drive both variation in parasite prevalence and differential behaviors and cultural characteristics. METHODS:…

Experiences of discrimination: validity and reliability of a self-report measure for population health research on racism and health.

Population health research on racial discrimination is hampered by a paucity of psychometrically validated instruments that can be feasibly used in large-scale studies. We therefore sought to investigate the validity and reliability of a short self-report instrument, the "Experiences of Discrimination" (EOD) measure, based on a prior instrument used in the Coronary Artery Risk Development in Young Adults (CARDIA) study. Study participants were drawn from a cohort of working class adults, age 25-64, based in the Greater Boston area, Massachusetts (USA). The main study analytic sample included 1

Internalized racism, racial collective self-esteem, and Asian American adults' disordered eating: Psychological distress as mediator.

Research emphasizes the importance of considering how sociocultural factors affect the disordered eating behaviors of Asian American adults. The present study thus investigated how internalized racism and racial collective self-esteem are associated with disordered eating among Asian American adults, as well as the potential mediating role of psychological distress. A final sample of 796 Asian American adults completed a cross-sectional survey that contained the study's questionnaires. Regression analyses revealed that internalized racism was positively associated with disordered eating, where…

Social media use, online racism, and mental health among Black Emerging Adults in the United States.

Social media use is a key risk factor for mental health symptomatology among emerging adults. Black emerging adults use social media frequently, where they are exposed to online racism, which may contribute to worse mental health. The goal of the current study was to examine the frequency of exposure to online racism as a mediator in the association between the frequency of social media use and depression and anxiety symptoms. Using a non-probability sample of 1005 monoracial Black emerging adults (Mage = 24.07, 50.6% women) from a larger investigation, participants completed an online survey,…

Racism and mental health within humanitarian and development organisations: perspectives from in-house mental health professionals.

INTRODUCTION: Racism is recognised as a concern within humanitarian and development (HD) organisations, yet little research has examined its implications for staff mental health. Notably, the perspectives of in-house mental health professionals (MHPs)-a key but understudied group-have remained absent from the literature. This study addresses this gap by asking three questions: First, how do MHPs perceive and personally experience racism within HD organisations? Second, in what ways does racism affect the mental health of staff, and how do MHPs navigate their role in addressing these impacts? T…

United against racism: enhancing access and patient experience for black and Hispanic patients in Neuro-oncological care.

Structural inequities in the U.S. healthcare system contribute to disparities in access and outcomes for racial and ethnic minority patients. This study examined the experiences of Black, Hispanic, and Non-Hispanic White (NHW) patients receiving neurosurgical care within a large academic health system. A validated survey was distributed to 400 patients treated for brain and spinal cord tumors between October 2022 and March 2023. The survey assessed barriers to access, patient-provider interactions, and overall satisfaction. Chi-squared tests were used for categorical variables, and Kruskal-Wal

The epidemiology of periodontal disease demonstra … es that racial inequities in periodontal disease have been observed, many have argued that systemic racism is the fundamental driver of racial health inequity. This paper interrogates the roles of systemic racism, dental education, clinical treatment, and patient behavior in periodontal disease. We describe how, together, these mechanisms contribute to r …

Addressing anti-black racism in an academic preterm birth initiative: perspectives from a mixed methods case study.

BACKGROUND: Growing recognition of racism perpetuated within academic institutions has given rise to anti-racism efforts in these settings. In June 2020, the university-based California Preterm Birth Initiative (PTBi) committed to an Anti-Racism Action Plan outlining an approach to address anti-Blackness. This case study assessed perspectives on PTBi's anti-racism efforts to support continued growth toward racial equity within the initiative. METHODS: This mixed methods case study included an online survey with multiple choice and open-ended survey items (n = 27) and key informant interviews …

Association of COVID-19 and Endemic Systemic Racism With Postpartum Anxiety and Depression Among Black Birthing Individuals.

IMPORTANCE: The intersection of endemic structural racism and the global health crisis secondary to the COVID-19 pandemic represents a syndemic, defined as the aggregation of 2 or more endemic and epidemic conditions leading to adverse repercussions for health. Long-standing inequities have placed Black individuals at disproportionate risk for negative postpartum mental health outcomes. Studies are urgently needed to understand how the COVID-19 pandemic has added to this risk (eg, syndemic associations). OBJECTIVE: To examine the association between the syndemic and the postpartum mental heal…

Black Physicians' Experiences with Anti-Black Racism in Healthcare Systems Explored Through An Attraction-Selection-Attrition Lens.

Anti-Black racism is a specific form of racism directed at Black people. In healthcare, there are poignant examples of anti-Black racism in the recruitment, selection, and retention stages of the job cycle. Research shows that anti-Black racism is associated with inequitable work outcomes and the under-representation of Black physicians. However, empirical findings are scattered with no organizing framework to consolidate these findings. To add to the literature, in this paper we present the attraction-selection-attrition (ASA) model (Schneider, 1987) as an organizing framework to discuss Blac…
