Sensory nerve conduction velocities of median, ulnar and radial nerves in patients with vibration syndrome.

OBJECTIVE: The present study aimed to clarify the range of involvement for hand-arm vibration syndrome (VS) in the median, ulnar and radial nerves of the hand. METHODS: Sensory nerve conduction velocities (SCVs) for 3 nerves in the hands and arms were examined for 34 patients with VS and 23 age-matched controls. Neuropathy types were classified by possible carpal tunnel syndrome (CTS), Guyon's syndrome and digital neuropathy in three nerves. RESULTS: SCV in the median nerve (middle finger, wrist-elbow) and ulnar nerve (little finger, wrist-elbow), and amplitudes of the median nerve (wrist-pr

Posterior Interosseous Nerve Compression in the Forearm, AKA Radial Tunnel Syndrome: A Clinical Diagnosis.

BACKGROUND: Posterior interosseous nerve (PIN) compression in the forearm without motor paralysis is a challenging clinical diagnosis. This retrospective study evaluated the clinical assessment, diagnostic studies, and outcomes following surgical decompression of the PIN in the forearm. METHODS: This study reviewed 182 patients' medical charts following PIN decompression between 2000 and 2020 by a single surgeon. After exclusion of combined nerve entrapments, polyneuropathy, motor palsy, or lateral epicondylitis, the study included 14 patients. Data collected included: clinical presentation a

Late-Onset Radial Nerve Palsy in the Setting of Humeral Shaft Hypertrophic Nonunion: A Case Report.

CASE: A 67-year-old woman presented 9 months after a closed midshaft humerus fracture with a new onset radial nerve palsy. Radiographs demonstrated a hypertrophic nonunion. Upon exploration, the radial nerve was in continuity and entrapped in fracture callus. It was extricated from the callus, and an open reduction and plate fixation was performed. Full radial nerve function returned by 3 months. CONCLUSION: We recommend that delayed onset radial nerve palsies be treated on a semiurgent basis with radial nerve exploration and decompression followed by internal fixation to achieve primary bone

Complications of total elbow arthroplasty.

Modifications in implant design and improvements in surgical technique have expanded the applications of total elbow arthroplasty. Complications associated with reconstructive elbow surgery persist, however, often leading to profound and sometimes nonsalvageable disability. The most recognized complications include implant loosening, periprosthetic fracture, implant failure, infection, triceps insufficiency, and nerve palsy. Although far fewer elbow arthroplasties than lower extremity arthroplasties are performed, the proportion of complications is greater with elbow arthroplasty, and the outc

Radial nerve palsy following humeral shaft fracture: a theoretical PNF rehabilitation approach for tendon and nerve transfers.

BACKGROUND: Humerus fracture-induced radial nerve injury can create severe and permanent disabilities. PURPOSE: Surgical management often relies on either tendon or nerve transfer. Regardless of which procedure is selected, physical therapists are challenged to restore functional outcomes without jeopardizing repair healing. Through synergistic, multi planar upper extremity movement patterns, neuromuscular irradiation, or overflow, and neuroplasticity, proprioceptive neuromuscular facilitation (PNF) may improve strength, range of motion and tone. METHODS: After reviewing the literature, a fi

The Thrower's Fracture A Case Report and Literature Review.

There is a paucity of literature regarding atraumatic distalthird humeral shaft fractures, or "thrower's fracture." We report on a case of a healthy adult male, amateur athlete, who suffered an atraumatic fracture of the humerus while throwing a softball with his dominant arm. He underwent operative repair and developed a radial nerve palsy postoperatively that recovered 4/5 strength by 6 weeks and 5/5 strength by 12 weeks. Through this case report, we hope to increase the awareness of this unique injury and highlight the presentation, treatment, and complications associated with the thrower's

Complicated Traumatic Nerve Injury Treated with Tendon Transfers in a Patient with Myotonic Dystrophy: First Case Report.

CASE: We report a right-handed 37-year-old woman, with myotonic dystrophy type 1 (MD1), presenting with a posterior interosseus nerve injury because of a penetrating trauma in the right forearm. The tendon transfer technique was chosen based on tendon response and functionality of the arms during the surgery. The patient has been able return to her daily life activities with proper fine and gross motor control. CONCLUSION: Despite tendon transfer surgery being a common technique for radial nerve palsy reconstruction, its use has not been extensively described in the literature in patients wit

Combined radial and median nerve injury in diaphyseal fracture of humerus: a case report.

Radial nerve palsy is the most common neurological involvement in humeral shaft fractures. But combined radial and median nerve injury in a closed diaphyseal fracture of the humerus is rare. Combined injury to both radial and median nerve can cause significant disability. A detailed clinical examination is therefore necessary following humeral shaft fractures. We report a patient with closed diaphyseal humeral fracture (AO 12A-2.3) together with radial and median nerve palsy, its management and review of the literature. As the patient had two nerves involved, surgical exploration was planned.

Radial nerve palsy in the newborn: a case series.

BACKGROUND: Peripheral nerve palsies of the upper extremities presenting at birth can be distressing for families and care providers. It is therefore important to be able to identify patients whose diagnosis is compatible with full recovery so that their families can be reassured. METHODS: We conducted a retrospective review of all infants presenting with weakness of the upper extremity to our clinic between July 1995 and September 2009. We also conducted a review of the current literature. RESULTS: During the study period, 953 infants presented to our clinic. Of these patients, 25 were iden

Postoperative radial nerve palsy in humeral shaft nonunion reconstruction: Can the lateral paratricipital approach prevent this common complication?

BACKGROUND: Postoperative radial nerve palsy (RNP) is a well-known complication of nonunion reconstruction of the humerus. The purpose of the current study is to determine if the surgical approach for nonunion reconstruction of the humerus influences the rate of postoperative radial nerve palsy. METHODS: A retrospective case-control study of all humeral shaft and extraarticular distal humerus nonunion reconstructions performed between January 1, 2004, and August 31, 2021, was conducted. Patients included were over 18 years of age, had a non-pathologic humerus fracture nonunion and had intact

Analyzing the functional effects of dynamic and static splints after radial nerve injury.

The radial nerve is a commonly injured upper extremity peripheral nerve. The inability to extend the wrist results in a loss of hand function and dexterity that affects patients' ability to perform their activities of daily living. There is no strong evidence to support a particular splint design for improving dexterity. This cohort study compared whether a static or dynamic splint can improve hand dexterity when assessed with the 9-hole peg test (9-HPT) after radial nerve injury. Thirty-four subjects with radial nerve palsy participated in the study. The test was repeated three times for each

Radial Nerve Injury Caused by Compression Garment for Lymphedema: A Case Report.

Lymphedema is a condition characterized by localized fluid retention and tissue swelling caused by a compromised lymphatic system. To minimize fluid buildup and stimulate the flow of fluid through the lymphatic system, compression garments are usually applied to patients with lymphedema. There are few studies to report complications of compression garments to treat breast cancer-related lymphedema. To our knowledge, this is the first report of radial nerve compression neuropathy associated with wearing a compression garment to treat lymphedema. Level of Evidence: V.

Reliable techniques to avoid damaging the superficial radial nerve due to percutaneous Kirschner wire fixation of the distal radius fracture through the radial styloid process.

PURPOSE: The use of percutaneous Kirschner wires for fixation of unstable fractures of the distal radius has been widely accepted as the least invasive procedure. However, the injury to the superficial branch of the radial nerve (SBRN) is common. Our purpose in this study was to develop a reliable technique to avoid damaging the SBRN. METHODS: Twenty cadaver forearms were dissected to identify the SBRN distribution, and 18 forearms were used to undergo placement of three Kirschner wires (KW-A, KW-B, and KW-C). The KW-A, KW-B, and KW-C were driven in the frontal plane into the distal radius, a

Primary non-Hodgkin's lymphoma of the radial nerve: case report.

OBJECTIVE: Primary lymphomas of peripheral nerves are extremely rare, with the bulk of the literature being case reports. The nerve most commonly affected is the sciatic nerve, with 9 cases reported. To date, there are no reports in the English literature of isolated involvement of the radial nerve by a primary lymphoma. CLINICAL PRESENTATION: A 69-year-old woman with a history of osteoporosis, irritable bowel disease, asthma, and Graves' disease presented with a 6-month history of paresthesia in her left superficial sensory radial nerve territory, weakness of thumb extension, and localized p
