[Radiation-induced multiple tumors].

Multiple primary tumors have been detected in 192 (5.0%) cases out of 3,812 patients with primary cervical malignancies, 100 (2.6%) cases having been traced to radiation-related origin. The peculiarities of the syndrome of radiation-induced multiple tumors of the rectum, vagina and corpus uteri, duration of their development, histological patterns, degree of cell differentiation, depth of invasion and patterns of metastasis spreading have been identified. Relative cumulative risk of secondary tumor development and causal factors have been established.

[Radiation exposure during defecography].

The introduction of a new radiological method requires the assessment of the advantage-risk ratio. On measuring the radiation doses during defecography, skin entrance and gonadal exposure values were found to be lower than during well-known and well-established examinations of the small pelvis, such as colon enema and CT of the lower abdomen. Defecography, as a radiological tool for diagnosing the anorectal function is, therefore, also suitable for young persons.

Radiation therapy of acromegaly.

Conventional megavoltage irradiation of GH-secreting tumors has predictable effects on tumor mass, GH, and pituitary function. 1. Further growth of the tumor is prevented in more than 99% of patients, with only a fraction of a percent of patients requiring subsequent surgery for tumor mass effects. 2. GH falls predictably with time. By 2 years GH falls by about 50% from the baseline level, and by 5 years by about 75% from the baseline level. The initial GH elevation and the size and erosive features of the sella turcica do not affect the percent decrease in GH from the baseline elevation. 3. W…

Radiation, diagnosis, and management.

The biomedical effects of ionizing and nonionizing radiations include responses that are known to have thresholds ("nonstochastic" effects) as well as responses that are presumed to have no thresholds ("stochastic" effects). The latter include mutagenic, carcinogenic, and teratogenic effects. Present radiation risk management strategies seek to protect completely against nonstochastic effects by preventing anyone from accumulating a dose of radiation in excess of the relevant threshold. Protection against stochastic effects, on the other hand, is sought by limiting the level of radiation expos…

Radiation-induced lung injury.

The use of radiation therapy is limited by the occurrence of the potentially fatal clinical syndromes of radiation pneumonitis and fibrosis. Radiation pneumonitis usually becomes clinically apparent from 2 to 6 months after completion of radiation therapy. It is characterized by fever, cough, dyspnea, and alveolar infiltrates on chest roentgenogram and may be difficult to differentiate from infection or recurrent malignancy. The pathogenesis is uncertain, but appears to involve both direct lung tissue toxicity and an inflammatory response. The syndrome may resolve spontaneously or may progress…

[Radiation-induced brain atrophy].

Clinico-pathologic characteristics of radiation-induced brain atrophy were reviewed. Radiation-induced brain atrophy becomes obvious 2 to 3 months after radiation therapy in more than half of the patients who have been irradiated to the brain. Mental and neurologic deterioration were well correlated with the severeness of brain atrophy. Pathologically, demyelinating process secondary to direct neurotoxicity of irradiation seemed to be responsible for the entity. A large target field or whole brain irradiation and the senility of patients were major risk factors. The most important differential…

[Radiation-induced vaginal necrosis].

A report is given on three cases of total vaginal necrosis observed between 1980 and 1983 in 156 evaluable patients submitted to primary or postoperative irradiation. Characteristic signs of this disease are the incidence of sudden pains (increased in sitting position), necrosis and ulceration of the complete vaginal area, and a tendency to spontaneous recovery. All patients had been treated by hysterectomy or Wertheim operation, suffered from associated cardiovascular troubles, and had been exposed to a large surface irradiation of the vagina with 75 to 90 Gy. The disease was reversible in tw…

Infrared radiation and cataract.

The study was undertaken to determine if occupational exposure to infrared (IR) radiation increases the risk of developing cataract. The study includes epidemiologic investigations of two groups of workers exposed to IR radiation and two groups of non IR-exposed controls. The first investigation included 208 iron and steel workers and 208 controls. For each of the workers, the lifetime IR-exposure was calculated with the help of occupational interviews and measurements of the IR-exposure to their eyes in their jobs. Eye examinations including a detailed slit lamp examination of the lens were m…

Radiation-induced arterial injuries.

In a 20-year period from 1961 to 1981 at the University of Kentucky Medical Center, there were 20 patients with vascular lesions directly attributable to irradiation. Two distinct patterns of arterial injury attributable to radiotherapy were identified--arterial disruption and occlusion. Arterial disruption occurred in 12 patients--11 carotid blowouts and 1 iliac artery rupture. Two patients underwent prophylactic carotid artery ligation for impending rupture. In the 11 carotid artery ruptures, ligation of the artery in nine patients resulted in stroke or death in five patients. Iliac artery d…

Methotrexate and ultraviolet radiation.

Reactivation of a sunburn has been reported after the administration of methotrexate for cancer chemotherapy. A similar reaction is described in a patient with psoriasis who was receiving both coal tar with ultraviolet radiation for cutaneous lesions and methotrexate for arthritis. Knowledge of this poorly understood side effect of methotrexate is particularly important for physicians administering phototherapy and, perhaps, photochemotherapy.

Radiation safety for children.

In recent years there has been much public and professional concern about reducing patient exposure to radiation. This concern is especially appropriate for children because of their greater sensitivity to it. Exposure risks to children may be reduced by using radiography only when there is a specific clinical indication. Bitewing radiographs are indicated for detection of approximal caries in the child when the molar contacts are closed. The frequency of this examination should be determined on an individual basis. Other radiographs should be obtained only when there are clinical signs or sym…

Reactions to ionizing radiation.

This review deals with essential physical and biologic aspects of ionizing radiation as related to dermatology. The discussion of major physical factors and basic biophysics includes the production of x rays, dose definitions and recent changes in terminology, various factors affecting dose and tissue damage and current radiotherapy methods. The biologic effects and potential risks of ionizing radiation are reviewed in detail, particularly the major nonstochastic and stochastic somatic effects, such as radiation-induced skin cancer, thyroid carcinoma, breast cancer, and other radiogenic neopla…

Colonoscopy in radiation colitis.

Colonoscopy in 13 patients with stenosis or hemorrhage after radiation therapy is correlated with clinical and radiographic features. The authors found colonoscopy helpful in delineating the diagnosis and in guiding treatment.

The radiation-injured bowel.

Radiation disease of the intestine is usually iatrogenic and frequently unavoidable. The disease, its treatment, and the disability produced are formidable. There is hope that means may be found to increase the resistance of the intestine to radiation damage. Radiation enteropathy is an insidious, progressive disease that is seen with increasing frequency. Serious disabilities may develop after years of gestation. Those patients who require surgery are treated by control of sepsis, correction of metabolic abnormalities, and reversal of protein/calorie malnutrition prior to definitive surgery. …
