Are neuroradiological or neurophysiological characteristics associated with upper-extremity hypertonia in severe ischaemia in supratentorial stroke?

OBJECTIVE: The primary goal was to identify the neuroradiological and neurophysiological risk factors for upper-extremity hypertonia in patients with severe ischaemic supratentorial stroke. DESIGN: Inception cohort. PATIENTS: Forty-three consecutive patients with an acute ischaemic supratentorial stroke and an initial upper-extremity paralysis admitted to an academic hospital recruited over a 1.5-year period. MAIN OUTCOME MEASURES: Upper-extremity hypertonia was assessed by the Ashworth scale and clinically relevant hypertonia was defined as Ashworth scale >or=2. Any association of (clinica

Natural evolution of idiopathic carpal tunnel syndrome with respect to wrist and hand anthropometry: A prospective cohort study.

OBJECTIVE: A greater wrist depth/width ratio and wrist depth/palm length ratio are known risk factors for carpal tunnel syndrome. We hypothesized that these parameters might also predict progression in patients who were not surgically treated. METHODS: Seventy-eight patients with moderately severe idiopathic carpal tunnel syndrome of at least 10 months duration at recruitment, who declined surgical treatment and steroid injection, underwent repeated neurophysiological assessments after 3 years. A > 10% increase in median SNAP latency was taken as evidence of significant deterioration. RESULT

Neurophysiologic and ophthalmic markers of chemotherapy-related cognitive impairment in patients diagnosed with hematologic cancer: A feasibility study.

BackgroundBiomarkers of chemotherapy-related cognitive impairment (CRCI) in hematologic cancer are understudied and underdeveloped. We evaluated the feasibility of using ophthalmic and neurophysiologic markers to assess CRCI in hematologic cancer.MethodsHematologic cancer patients either receiving (Ctx+) or not receiving (Ctx-) chemotherapy were recruited from a tertiary medical center. Demographically-matched healthy controls (HC) were also recruited. Ctx+ participants completed the following study visits: (1) after diagnosis but prior to chemotherapy (baseline); (2) after one treatment cycle

Unilateral lower limb suspension affects motor unit action potential train characteristics in a size-dependent manner during high force isometric knee extension.

Unilateral lower limb suspension (ULLS) mimics spaceflight-induced decreases in voluntary muscle strength and muscle recruitment. The purpose of this study is to examine the effects of short-term ULLS on quadriceps motor unit action potential train (MUAPT) characteristics during high-intensity isometric contractions. Thirteen healthy adults (7 women and 6 men; age: 18-44 yr) completed bilateral knee extension assessments, including maximal voluntary isometric contraction (MVIC), interpolated twitch, and trapezoidal ramp contractions, before and after 13-day ULLS. Surface electromyography (sEMG

Theory-of-mind-related neural activity for one's romantic partner predicts partner well-being.

Healthy social relationships are linked to myriad positive physical and mental health outcomes, raising the question of how to enhance relationship formation and quality. Behavioral data suggest that theory of mind (ToM) may be one such process. ToM is supported by a network of brain regions including the temporo-parietal junction (TPJ), medial prefrontal cortex and precuneus (PC). However, little research has investigated how the ToM network supports healthy social relationships. Here, we investigate whether recruitment of the ToM network when thinking about the mental states of one's romanti

Arthrogenic muscle inhibition in the leg muscles of subjects exhibiting functional ankle instability.

BACKGROUND: Functional ankle instability or a subjective report of ;;giving way'' at the ankle may be present in up to 40% of patients after a lateral ankle sprain. Damage to mechanoreceptors within the lateral ankle ligaments after injury is hypothesized to interrupt neurologic feedback mechanisms resulting in functional ankle instability. The altered input can lead to weakness of muscles surrounding a joint, or arthrogenic muscle inhibition. Arthrogenic muscle inhibition may be the underlying cause of functional ankle instability. Establishing the involvement of arthrogenic muscle inhibition

[Results of surgical treatment of radio-humeral epicondylopathy].

We examined extent and affection of an assumed neuromuscular transmission disorder by performing a prospective clinical study on 75 patients with therapy-resistant radiohumeral epicondylopathy. Before operation, we electromyographically diagnosed an increased rate of polyphasic potentials of the long wrist extensors as well as a prolonged motor latency of the respective muscles. Corresponding to a hereby implied damage to the distal part of the motor neuron, disordered neuromuscular recruitment combined with a reduced maximum strength and -elasticity could be proven. Both effects were signific

Comparative study between the normally hearing child and the hard of hearing child, by acoustic impedance measurements of the ear.

In the first part, the author makes a comparative study of the tympanometric results obtained from the normally hearing child and the hard of hearing child. The frequency of serous otitis media is statistically higher in the deaf child. The author compares such results of tympanometry as obtained from the child, considers the contribution of this technique to the field of pediatric audiology and suggests a classification of tympanograms in terms of the probe-tone frequency of the impedancemeter. The second part concerns the acoustico-facial reflex. This study compares the recording of the stap

Cross Recruitment of Domain-Selective Cortical Representations Enables Flexible Semantic Knowledge.

Knowledge about objects encompasses not only their prototypical features but also complex, atypical, semantic knowledge (e.g., "Pizza was invented in Naples"). This fMRI study of male and female human participants combines univariate and multivariate analyses to consider the cortical representation of this more complex semantic knowledge. Using the categories of food, people, and places, this study investigates whether access to spatially related geographic semantic knowledge (1) involves the same domain-selective neural representations involved in access to prototypical taste knowledge about

Muscle fiber conduction velocity in the vastus lateralis and medialis muscles of soccer players after ACL reconstruction.

The neural factors underlying the persistency of quadriceps weakness after anterior cruciate ligament reconstruction (ACLR) have been only partially explained. This study examined muscle fiber conduction velocity (MFCV) as an indirect parameter of motor unit recruitment strategies in the vastus lateralis (VL) and medialis (VM) muscles of soccer players with ACLR. High-density surface electromyography (HDsEMG) was acquired from VL and VM in nine soccer players (22.7 ± 2.9 years; BMI: 22.08 ± 1.72 kg·m-2 ; 7.7 ± 2.2 months post-surgery). Voluntary muscle force and the relative myoelectrical acti

Visual change detection recruits auditory cortices in early deafness.

Although cross-modal recruitment of early sensory areas in deafness and blindness is well established, the constraints and limits of these plastic changes remain to be understood. In the case of human deafness, for instance, it is known that visual, tactile or visuo-tactile stimuli can elicit a response within the auditory cortices. Nonetheless, both the timing of these evoked responses and the functional contribution of cross-modally recruited areas remain to be ascertained. In the present study, we examined to what extent auditory cortices of deaf humans participate in high-order visual proc

Discharge characteristics of laryngeal single motor units during phonation in young and older adults and in persons with parkinson disease.

Discharge characteristics of laryngeal single motor units during phonation in young and older adults, and in persons with Parkinson disease. The rate and variability of the firing of single motor units in the laryngeal muscles of young and older nondisordered humans and people with idiopathic Parkinson disease (IPD) were determined during steady phonation and other laryngeal behaviors. Typical firing rates during phonation were approximately 24 s/s. The highest rate observed, during a cough, was 50 s/s. Decreases in the rate and increases in the variability of motor unit firing were observed i

Pre and post-glycerol special audiometric tests battery results in endolymphatic hydrops.

The glycerol test for endolymphatic hydrops was administered to 17 patients exhibiting classical symptoms of Meniere's disease. A complete battery of audiometric tests including the SISI, TTD (Rosenberg variation). ABLB, Sweep Frequency Bekesy, two tests of speech discrimination (Rush Hughes and Northwestern), and air and bone conduction thresholds were administered pre and post-glycerol ingestion in order to determine the effect of osmotic diuresis. Improvement in pure-tone sensitivity and speech discrimination scores were most often observed. Among the special audiometric tests only the SISI

Preoperative donor nerve electromyography as a predictor of nerve transfer outcomes.

PURPOSE: We hypothesized that health of the donor nerve and corresponding muscle, as assessed by electromyography (EMG), could predict the outcome of nerve transfer surgery. METHODS: A retrospective review was performed to investigate outcomes of nerve transfers for elbow flexion and shoulder abduction. Motor strength was graded preoperatively and after a minimum 1-year follow-up. Preoperative EMG results were classified as functionally normal or affected based on motor unit recruitment pattern and correlated with follow-up motor strength and range of motion. RESULTS: Forty nerve transfers w

Fat tissue alters quadriceps response to femoral nerve magnetic stimulation.

OBJECTIVE: We investigate the influence that inguinal fat has on maximal quadriceps response to femoral nerve magnetic stimulation. METHODS: Vastus lateralis M-wave amplitude and quadriceps twitch peak torque were measured (i) in lean and overweight subjects and (ii) when slices of pig fat ranging from 2 to 18 mm in thickness were placed between the coil and the nerve. RESULTS: In overweight group, the maximal response could not be elicited when intensity was ≤90% and ≤85% of maximal power for twitch torque and M-wave amplitude, respectively. The maximal response was obtained at 80% of maxim

Predicting optimal electrical stimulation for repetitive human muscle activation.

Functional electrical stimulation is the use of electrical currents to activate paralyzed muscles to produce functional movements. Muscle force output must meet or exceed the external load to maintain a posture or produce movements. A mathematical force-fatigue modeling system that predicts muscle force responses during repetitive electrical stimulation has been developed in our laboratory to help identify stimulation patterns that optimize force output for individual subjects. This study tests how well this model predicts the number of contractions that can be maintained above a required forc

Chronic measurement of the stimulation selectivity of the flat interface nerve electrode.

The flat interface nerve electrode (FINE) is an attempt to improve the stimulation selectivity of extraneural electrodes. By reshaping peripheral nerves into elliptical cylinders, central fibers are moved closer to the nerve-electrode interface, and additional surface area is created for contact placement. The goals of this study were to test the hypothesis that greater nerve reshaping leads to improved selectivity and to examine the chronic recruitment properties of the FINE. Three FINEs were developed to reshape peripheral nerves to different degrees. Four electrodes of each type were implan
