Non-participation in chlamydia screening in The Netherlands: determinants associated with young people's intention to participate in chlamydia screening.

BACKGROUND: In The Netherlands, a national chlamydia screening program started in 2008, but the participation was low and the screening was not cost-effective. This study aimed to explore unconscious and conscious associations with chlamydia screening (16-29 year-olds). In addition, we examined whether information presented in chlamydia screening invitation letters had an effect on the evaluation of these determinants compared to a no-letter group. METHODS: An Internet survey was conducted that included self-report measures of attitude, susceptibility, severity, unrealistic optimism, subjecti

Who wants to join preventive trials?--Experience from the Estonian Postmenopausal Hormone Therapy Trial [ISRCTN35338757].

BACKGROUND: The interest of patients in participating in randomized clinical trials involving treatments has been widely studied, but there has been much less research on interest in preventive trials. The objective of this study was to find out how many women would be interested in a trial involving postmenopausal hormone therapy (PHT) and how the women's background characteristics and opinions correlated to their interest. METHODS: The data come from recruitment questionnaires (n = 2000) sent to women in Estonia in 1998. A random sample of women aged 45 to 64 was drawn from the Population R

Augmenting Cognitive Behavior Therapy for School Refusal with Fluoxetine: A Randomized Controlled Trial.

This study investigates whether the augmentation of cognitive behavior therapy (CBT) with fluoxetine improves outcomes in anxious school refusing adolescents (11-16.5 years). Sixty-two participants were randomly allocated to CBT alone, CBT + fluoxetine or CBT + placebo. All treatments were well tolerated; with one suicide-attempt in the CBT + placebo group. All groups improved significantly on primary (school attendance) and secondary outcome measures (anxiety, depression, self-efficacy and clinician-rated global functioning); with gains largely maintained at 6-months and 1-year. Few participa

An empirically based typology of temporary alcohol abstinence challenge participants using latent class analysis.

INTRODUCTION: Identifying subgroups of Temporary (alcohol) Abstinence Challenge (TAC) participants may offer opportunities to enhance intervention effectiveness. However, knowledge about such subgroups is missing. This study aimed to (i) describe a TAC population; (ii) identify subgroups of participants based on determinants of changes in drinking behaviour; and (iii) characterise subgroups in terms of sociodemographic and other characteristics. METHODS: Data from 3803 Dutch TAC participants were analysed to identify subgroups using three-step Latent Class Analysis. Classes were based on dete

Acceptability of urine-based screening for Chlamydia trachomatis to asymptomatic young men and their providers.

OBJECTIVE: The objective of this study was to describe acceptability of urine chlamydia testing among asymptomatic men and providers' attitudes toward testing. STUDY DESIGN: Asymptomatic men (no urethral discharge/dysuria) were offered free testing and characteristics of men who accepted were compared with those who declined. Acceptability logs tallied the proportion who accepted, and a standardized survey was administered to providers at study's end. RESULTS: Median acceptance was 64% (range, 8-100%). Men accepting were younger and more likely to be in adolescent primary care or detention,

Psychological and physical activity training for older persons: who does not attend?

BACKGROUND: Interventions to promote successful aging include psychological and physical activity programs. Identification of determinants of attendance of older persons may be useful to develop strategies to improve attendance. For physical activity programs determinants of attendance have been investigated extensively. For psychological programs hardly any knowledge is available. Determinants of attendance at psychological and physical activity programs have never been investigated simultaneously. OBJECTIVE: To identify demographic, physical and psychological determinants of attendance of o

Implementing a Patient-Centered Education Bundle to Improve Venous Thromboembolism Prevention.

IMPORTANCE: Venous thromboembolism (VTE) is one of the most common causes of preventable harm in hospitals. While numerous efforts have been made to increase prescription of evidence-based VTE prophylaxis for hospitalized patients, many prescribed doses are either refused or missed for other reasons. Successful interventions to decrease these missed doses have been developed and tested in academic hospitals, but it remains unclear if these interventions can be scaled and implemented in community hospitals. OBJECTIVE: To disseminate and implement an evidence-based, comprehensive strategy to re

Use of pre-enrollment randomization and delayed consent to maximize participation in a clinical trial of opt-in versus opt-out tobacco treatment.

Background: Enrollment in smoking cessation trials remain sub-optimal. The aim of this analysis was to determine the effectiveness of a modified Zelen's design in engaging hospitalized patients who smoke in a pragmatic OPT-IN versus OPT-OUT tobacco treatment trial. Methods: At bedside, clinical staff screened smokers for eligibility, randomized eligible into study arms, and delivered the appropriate treatment approach. Study staff called randomized patients at one-month post-discharge, debriefed patients on the study design, and collected consent to participate. We used frequencies and percent

In California, voluntary mass prenatal screening.

A statewide program in California to detect neural tube and other birth defects may revive enthusiasm for mass prenatal screening. Participation in the program is voluntary, but all expectant mothers are asked to sign a statement of "informed consent/refusal." So far California's program seems to be working well, but questions for the future include the level of participation, the possibility that normal fetuses will be aborted, the kinds of information given to women, and the elusive nature of free choice.

The first steps in facing your fears: The acceptability of virtual reality and in vivo exposure treatment for specific fears.

Phobic individuals are often reluctant to engage in exposure in vivo (IVE). We examined whether providing exposure in virtual reality (VRE) can increase the acceptability. In Study 1, we provided 186 fearful participants with (hypothetical) VRE and IVE treatment offers and examined their willingness to engage in treatment, estimated success, negative beliefs, acceptance/refusal and their preference. Almost 70 % preferred VRE over IVE and acceptance rates were higher for the VRE offer (58 %) than for IVE (35 %). Although participants held fewer negative beliefs about VRE, they did rate IVE as s

Factors that influence vaccination decision-making by parents who visit an anthroposophical child welfare center: a focus group study.

In recent years, parents have become more disparaging towards childhood vaccination. One group that is critical about the National Immunization Program (NIP) and participates less comprises parents with an anthroposophical worldview. Despite the fact that various studies have identified anthroposophists as critical parents with lower vaccination coverage, no research has been done to explore the beliefs underlying their childhood vaccination decision-making. We conducted a qualitative study using three focus groups (n = 16) of parents who visit an anthroposophical child welfare center. Our fin

The hidden cost of health misinformation: distrust, treatment refusal, and the burden on healthcare professionals.

BACKGROUND: Health misinformation has become a persistent challenge in contemporary clinical practice, shaping patient beliefs, weakening trust in healthcare professionals, and contributing to the refusal of evidence-based treatments. In the context of the growing influence of digital platforms, frontline health professionals increasingly face consultations in which misinformation must be addressed alongside clinical care. METHODS: A qualitative descriptive design based on semi-structured interviews was administered through an online open-ended questionnaire. Healthcare professionals working

Non-response bias in estimates of HIV prevalence due to the mobility of absentees in national population-based surveys: a study of nine national surveys.

OBJECTIVES: To measure the bias in national estimates of HIV prevalence in population-based surveys caused by mobility and refusal to test. METHODS: Data from nine demographic and health surveys and AIDS indicator surveys were used. Non-responders were divided into three groups: (i) "refusals" who were interviewed but not tested; (ii) "refusals" who were present in the household but not interviewed or tested; and (iii) "absentees" who were absent from the household. Correction for HIV status was made for the non-responders using multiple imputation methods with logistic regression models base

Parental hesitancy and refusal toward routine childhood vaccination after the COVID-19 vaccination period: a cross-sectional study.

BACKGROUND: The COVID-19 pandemic intensified public debates on vaccines, amplifying concerns about safety, side effects, rapid development, and institutional trust. These concerns may be associated with current attitudes toward routine childhood immunization. This study assessed parental intentions regarding routine childhood vaccination among parents whose children had previously completed routine immunizations. METHODS: This cross-sectional analytical study included 320 parents of children aged 4-8 years who had completed routine childhood vaccinations and registered at Dokuz Eylul Univers

The potential for bias due to attrition in the National Exposure Registry: an examination of reasons for nonresponse, nonrespondent characteristics, and the response rate.

This study examined attrition in the Trichloroethylene (TCE) Subregistry of the National Exposure Registry (NER). The analyses focused on 3915 persons exposed to the chemical TCE through the drinking water in their home. Baseline data were compared for subgroups of the TCE Subregistry members who were eligible to participate in the first TCE Subregistry follow-up. Study members were grouped according to their participation status in the first follow-up: remainers (n = 3494) and losses (n = 421), and three subgroups of losses: refusals, unable to locate, and unable to contact.
