BACKGROUND: Colloids are widely used in the replacement of fluid volume. However, doubts remain as to which colloid is best. Different colloids vary in their molecular weight and therefore in the length of time they remain in the circulatory system. Because of this, and their other characteristics, they may differ in their safety and efficacy.
OBJECTIVES: To compare the effects of different colloid solutions in patients thought to need volume replacement.
SEARCH METHODS: We searched the Cochrane Injuries Specialised Register (searched 1 December 2011), the Cochrane Central Register of Contro

Cardiopulmonary arrest refers to cessation of clinically detectable cardiac activity. In children, it usually results from progression of shock, respiratory failure or cardiac dysrhythmia. Early recognition and timely interventions in above group of patients is the key to prevent progression to cardiac arrest. The goal of resuscitation is to urgently re-establish oxygenation of vital organs by attention to Airway, Breathing and Circulation. Measures to restore airway patency include positioning, suctioning, continuous positive airway pressure, relieving a foreign-body airway obstruction and, e…

Delivering a new life into the world is truly a unique and proud moment, not just for the EMS providers who may do only one in their careers, but also for the parents, who probably did not think their new baby would be born outside the hospital.Due to the infrequency of delivering a baby in the prehospital environment, and even more rarely having to perform a major resuscitation on a newborn, perseverance in reviewing and preparing for delivering the newborn who requires ventilation cannot be overlooked, downplayed or pushed to the back of the training calendar. All EMS providers, services and…

Patients undergoing emergency surgery typically require resuscitation, either because they are hemorrhaging or because they are experiencing significant internal fluid shifts. Intravascular hypovolemia is common at the time of anesthesia induction and can lead to hemodynamic collapse if not promptly treated. Central pressure monitoring is associated with technical complications and does not improve outcomes in this population. Newer modalities are in use, but they lack validation. Fluid resuscitation is different in bleeding and septic patients. In the former group, it is advisable to maintain…

AIM: Perinatal asphyxia is a major contributor to the nearly 4 million neonatal deaths worldwide each year in resource-limited settings. Neonatal resuscitation, a proven method for preventing newborn deaths, is effective only when local caregivers have proper training and access to essential supplies. There are few published data describing neonatal resuscitation capacity in Nepal, where neonatal mortality rates are high. The goal of this study was to quantify neonatal resuscitation capacity at birthing sites in urban and rural Nepal.
METHODS: Seventeen birth centres ranging from tertiary car…

Adequate circulating volume to maintain mean arterial blood pressure above a critical value is necessary to reverse bradycardia by positive-pressure ventilation during resuscitation after asphyxia. A variety of circumstances can lead to visible or occult blood loss in the perinatal period; however, distinguishing hypovolemic shock from asphyxial shock can be difficult in the delivery room. Small, randomized, controlled trials support the usefulness of isotonic crystalloid rather than albumin-containing solutions for acute volume expansion; ready availability, lower cost, and lesser risk of inf…

BACKGROUND: Sudden cardiac arrest is a common mechanism of premature death in the community. Resuscitation is often possible, but no large study of resuscitation by doctors who practice there has been published.
METHODS: General practitioners (GPs) equipped with defibrillators reported 555 patients with cardiac arrest in whom they attempted resuscitation.
FINDINGS: Average age was 65.4, 75% male. Most arrests (49%) occurred at the patient's home but some (18%) occurred at or near the doctors' surgeries. Heart disease was responsible for 88% of the arrests: in these cases resuscitation to lea…

Resuscitation from circulatory and respiratory failure represent mainstays of emergency and critical care management. Importantly, no amount of resuscitative effort will be successful in promoting patient survival if the primary reason for the shock state is not identified and treated, independent of resuscitation. Having said that, aggressive resuscitation to normal functional levels of blood flow and organ perfusion pressure during the first 6 hours following the development of shock improves outcome both in patients with trauma or sepsis. However, clinical studies have demonstrated that res…

The administration of intravenous fluids is perhaps the most common treatment given in the intensive care unit. According to biologic rationale, ongoing fluid losses should be replaced to maintain fluid homeostasis and relative or absolute deficiencies in circulating blood volume should be prevented or rapidly corrected. There is agreement that insensible fluid losses and isotonic fluid losses should be replaced with a judicious mixture of water and crystalloid solutions. There is, however, a great deal of controversy on which fluids should be used during the acute resuscitation phase to deal …

Fluid deficits can arise as a result of a range of conditions, including haemorrhage, dehydration, vomiting and diarrhoea. Excessive fluid loss can have serious consequences for the patient and, if untreated, can lead to irreversible shock. The most common fluids used in fluid resuscitation are colloids and crystalloids, with research suggesting greater overall benefits for the use of the latter. Although it is vital to initiate fluid resuscitation early on after the patient has suffered trauma, there is some controversy as to the timing of this procedure--that is, whether it should be instiga…

The outcome of patients admitted to intensive care after a cardiac arrest was determined by reviewing intensive care unit records at four hospitals for 1993 and 1994. Of the 112 patients identified, 49 survived intensive care of whom 28 were discharged from hospital. In January 1996, 26 of the 28 patients could be traced; 22 of these were still alive. Seven factors were significantly different between survivors and nonsurvivors. At the cardiac arrest these were the number of direct current shocks (p < 0.05) and adrenaline doses (p < 0.01) given. In intensive care the factors were the presence

Basic life support (BLS) is a method of sustaining vital functions in a person who has collapsed and is unconscious, frequently with a cardiac arrest but sometimes with respiratory arrest, choking or other cause. BLS is a vital link in the 'chain of survival' for these critically ill people.

Suspended animation is defined as the therapeutic induction of a state of tolerance to temporary complete systemic ischemia, i.w., protection-preservation of the whole organism during prolonged circulatory arrest ( > or = 1 hr), followed by resuscitation to survival without brain damage. The objectives of suspended animation include: a) helping to save victims of temporarily uncontrollable (internal) traumatic (e.g., combat casualties) or nontraumatic (e.g., ruptured aortic aneurysm) exsanguination, without severe brain trauma, by enabling evacuation and resuscitative surgery during circulator…
