Indian childhood cirrhosis of the liver continues to be a major cause of morbidity and mortality of children living in the Indian sub-continent. The present paper deals with the morphological changes in various stages of the disease process. Ballooning and feathery degeneration of hepatocytes, complete absence of fatty change, frequent occurrence of intracytoplasmic hyaline bodies, dissection of pseudolobules by fibrous septa and fibrosis around single cell were notable features. Sixteen patients with bird-fancier's lung were screened for evidence of coeliac disease by assessing their clinical features, red-bloodcell or serum folate levels, and serum for reticulin antibodies. This test is based on the observation that wheat protein binds selectively to connective tissue fibres in cryostat sections of mammalian tissues. In a child with immune thrombocytopenic purpura (ITP) the findings of circulating reticulin antibodies and IgA and IgG gliadin antibodies suggested the diagnosis of coeliac disease. This was verified by small intestinal biopsy. In spite of a gluten-free diet the thrombocytopenia persisted. The association of ITP and coeliac disease was previously described in adults but to our knowledge this is the first report of the coexistence of ITP and coeliac disease in a child.
