Rhinophyma is a disfiguring end-stage manifestation of acne rosacea. It is characterized by a painless hyperplasia of the sebaceous glands and connective tissues of the nose. Numerous surgical modalities-including scalpel surgery, dermabrasion, CO2 laser ablation, and electrocautery-have been reported with varying results. We describe here our experience in using sinus microdebrider to rapidly debulk and sculpt the tissues in cases of rhinophyma correction. The aim of the article is to present an update on the pathophysiology, clinical features and treatment of rhinophyma. Rhinophyma is thought to represent the most severe expression of acne rosacea for which surgery is the mainstay of treatment. The use of Versajet Hydrosurgery System has not been previously described for the treatment of this condition. We present six patients with moderate to severe rhinophyma ranging from 41 to 77 years of age who were treated successfully with this technique.

Rhinophyma is commonly treated by shaving the involved florid tissue with a scalpel. A technique using a modified electrocoagulation handset, which was used on 11 patients, is described. This technique allows the operator to sculpt the shape of the nose in a relatively bloodless field. The results in 10 of the 11 patients were good to excellent and the rate of complications was both small and avoidable.

Patients are aware when even minimal bulbosity appears secondary to acne rosacea. Many suffer psychic trauma as a result. Early surgical treatment of this entity eliminates disfiguring hypertrophy. We advocate early surgical treatment of even minimal rhinophyma.

A new instrument is suggested for surgical treatment of rhinophyma. It is an electrically driven dermatome that has been used beneficially in surgery on the palate. Its use offers several advantages over other instruments and methods currently used for the management of rhinophyma. It avoids the danger of shaving too deeply because the depth of each sweep is both predictable and constant. The size of the blade allows fine sculpturing even in narrow spaces.
