Detection of a major risk factor for sudden death in an otherwise asymptomatic person often raises major difficulties in management, particularly where the only treatment available is invasive, such as the implantable defibrillator. Recent guidelines have described the appropriate use of this technology, but often difficulty remains. This is particularly the case where the condition is newly recognised and its natural history not yet extensively described. A 63 year old man, whose condition was diagnosed as Brugada syndrome, in whom this problem is illustrated is described.

The physician must screen patients for risk of exposure to the human immunodeficiency virus (HIV). This requires direct and specific questioning about blood product exposure, sexual practices and use of intravenous drugs. If risk factors are uncovered, patients must then be counseled on how to minimize their risk. The screening process is potentially uncomfortable for both the physician and the patient. However, intensive educational efforts, including behavioral changes, are necessary to reduce the number of HIV-infected individuals.

OBJECTIVE: The aim of the study was to estimate the maximum incidence of agranulocytosis which clozapine would have caused between 2006 and 2010 had there been no blood monitoring system; and to determine the number of clozapine-associated cases of agranulocytosis and related deaths recorded between 1993 and 2011.
METHOD: Records associating clozapine use with white blood cell deficiency (WBCD), in the Therapeutic Goods Administration's Case Line Listing of adverse drug reactions, were examined. The figure of 11,000 was used as the population on clozapine each year from 2006-2010.
