[Challenges and clinical strategies for endodontic microsurgery in the posterior maxillary region].

The posterior maxillary region presents substantial challenges in endodontic microsurgery owing to its unique anatomical position and intricate root canal anatomy. Three factors contribute to this complexity: first, the posterior location and limited surgical field, compounded by prominent buccal and palatal contours, severely limit visual clarity and operative maneuverability. Second, the complex root canal anatomy‒particularly isthmuses and the second mesiobuccal canal‒increases the difficulties and risks associated with root-end resection and retrograde obturation. Third, the intimate proxi

The pH changes of four different root canal sealers after mixing at various time intervals in vitro.

The purpose of this study was to compare the surface pH level of four different type sealers after mixing at various time intervals in vitro. After cleaning and shaping root canals in 50 recently extracted, single-rooted human teeth, they were divided into six groups. Each of four groups was obturated with Apexit, N2, Sealapex, and Grossman sealers. The remaining groups served as controls. In group 1 nothing was added to the vial of saline solution. In group 2 canals had been instrumented but not obturated with sealer. The pH level of saline solutions was determined at intervals of 1 hour, 4 h

Effects of chlorhexidine gluconate as an endodontic irrigant on the apical seal: short-term results.

The aim of this study was to ascertain whether chlorhexidine gluconate (0.12%), used as an endodontic irrigating solution would affect the apical seal obtained when using three different root canal cements. One hundred extracted human single-canal teeth were divided into 9 experimental groups of 10 teeth each and a positive and negative group of 5 teeth each. The teeth were decoronated at the level of the cementoenamel junction, then accessed, instrumented to a Master Apical File #50, irrigated with either sterile saline, 5.25% NaOCl or 0.12% chlorhexidine gluconate, and dried using paper poin

Effect of Retreatment Protocols on the Push-Out Bond Strength of Hydraulic Calcium Silicate Cement in Open-Apex Teeth.

OBJECTIVES: During retreatment, complete removal of previous obturation materials is essential, as remnants may compromise adhesion. This study evaluated the effect of four retreatment protocols on the push-out bond strength of RetroMTA (BioMTA, Seoul, Korea), a fast-setting calcium silicate-based material, used as an apical barrier in teeth with open apices that had been previously obturated with gutta-percha and AH26 sealer. MATERIALS AND METHODS: Sixty extracted single-rooted human teeth were instrumented and obturated using lateral condensation and then stored in phosphate-buffered saline

Short-Term Efficacy of Vitapex Paste in Single-Visit Root Canal Treatment for Acute Irreversible Pulpitis: A Retrospective Cohort Study.

BACKGROUND: The selection of root canal obturation material is critical for the successful management of acute irreversible pulpitis. Vitapex, an iodoform-calcium hydroxide-based paste, possesses recognized antimicrobial and biocompatible properties. However, comprehensive evidence comparing its efficacy in single-visit root canal treatment against the conventional gutta-percha/zinc oxide-eugenol (ZOE) sealer combination remains relatively limited. OBJECTIVE: To compare the clinical efficacy of Vitapex paste versus gutta-percha/ZOE sealer in single-visit root canal treatment for acute irrever…

Conservative Management of Class 4 Invasive Cervical Root Resorption Using Calcium-enriched Mixture Cement.

Class 4 invasive cervical root resorption (ICRR) presents a treatment dilemma in endodontics. The widely accepted treatment options for a class 4 ICRR are to leave these teeth untreated for as long as they are asymptomatic or extraction. This report presents a conservative approach for the management of class 4 ICRR. A 28-year-old woman was referred for root canal treatment of tooth #26. The patient had a history of orthodontic treatment. Radiographic evaluation showed class 4 ICRR that had perforated the root canal space, a radiolucent crestal bony defect, and a periapical lesion. Clinically,

Cone-Beam Computed Tomography Assessment of Root Canal Morphological Variations in Maxillary Second Premolars.

Background and objective A comprehensive understanding of root canal morphology and its anatomical variations is critical for the success of root canal treatment (RCT). Unrecognized variations may lead to incomplete cleaning, shaping, and obturation, potentially resulting in treatment failure. This study aimed to evaluate the frequency and distribution of root canal configuration types in maxillary second premolars (Mx2P) and their association with gender using cone-beam computed tomography (CBCT). Materials and methods This retrospective cross-sectional study analyzed 384 CBCT scans of fully

Diving Deep: Exploring Dual Palatal Canals in the Upper Second Molar.

Dental anatomy exhibits considerable variation with the presence of additional canals being a common occurrence. The upper second molar typically presents with three canals and three roots; however, variations such as the existence of an extra canal or a root can pose challenges during endodontic treatment. Maxillary molar is characterized by an additional canal located within the palatal root, often exhibiting complex configurations and variations in morphology. Access refinement is critical to gaining adequate visibility and facilitating instrumentation. Meticulous exploration of the pulp ch

Comparative assessment of retreatment systems for the removal of root canal filling materials using cone-beam computed tomography: An in vitro study.

INTRODUCTION: Successful endodontic retreatment depends on effective removal of existing root canal filling materials to enable adequate disinfection. Persistent intraradicular microorganisms are a primary cause of endodontic failure, underscoring the importance of thorough obturation removal. Although several nickel-titanium (NiTi) rotary retreatment systems have been introduced to improve efficiency, their performance may vary depending on the type of endodontic sealer used. The present study compared the efficacy of four rotary retreatment file systems in removing filling materials obturate

Factors affecting the periapical healing process of endodontically treated teeth.

Tissue repair is an essential process that reestablishes tissue integrity and regular function. Nevertheless, different therapeutic factors and clinical conditions may interfere in this process of periapical healing. This review aims to discuss the important therapeutic factors associated with the clinical protocol used during root canal treatment and to highlight the systemic conditions associated with the periapical healing process of endodontically treated teeth. The antibacterial strategies indicated in the conventional treatment of an inflamed and infected pulp and the modulation of the h

Biological perspectives on the non-surgical endodontic management of periradicular pathosis.

Differential radiographic criteria are unreliable for the histological diagnosis of periradicular lesions of pulpal origin. It should not, therefore, be assumed that large lesions or lesions with an opaque border are cystic. Even if cystic, or if strands of epithelium are present, the literature supports repair following the removal of the source of the pathosis. In most cases, the aetiological factors are oral contamination through the root canal or degenerating pulpal tissue. Therefore, the mere surgical removal of lesions of pulpal origin without removing the causes through proper root cana
