Cervical stenosis in a professional rugby league football player: a case report.

BACKGROUND: This paper describes a case of C7 radiculopathy in a professional rugby league player after repeated cervical spine trauma. The report outlines the management of the patient following an acute cervical hyperflexion injury with chiropractic manipulation and soft tissue therapies. It also presents a change in approach to include distractive techniques on presentation of a neurological deficit following re-injury. The clinical outcomes, while good, were very dependent upon the athlete restricting himself from further trauma during games, which is a challenge for a professional athlete…

Age-related changes in the cervical spines of front-line rugby players.

Cervical spine trauma occurs frequently in front-line rugby players. To evaluate the accumulative effects of this trauma, magnetic resonance imaging scans of the cervical spine were performed on 47 rugby players and 40 age-matched control subjects. The aim of this study was to compare the changes in the cervical spine of players at different points in their careers. A study of cervical spine changes, including spinal curve, spinal constituents, posttraumatic deformities, and degenerative modifications, was completed by a study of cervical measurements. Front-line rugby players showed more earl…

Effects of cold-water immersion after rugby-specific training on endurance performance.

BACKGROUND: This study investigated whether whole-body cold-water immersion (CWI) following rugby-specific training influences endurance exercise performance 24 h later. METHODS: Eleven healthy male collegiate rugby players completed an incremental cycling test to determine peak oxygen uptake (V̇O<inf>2peak</inf>) and time to exhaustion at baseline (Pre). One week later, participants performed a standardized rugby-specific training session consisting of warm-up, skill-based passing, contact drills, individual training (i.e., conversion kicking and scrummaging), and a bronco endurance test (to…

Exceptional Case Reports of Professional Rugby Players with Diffuse Idiopathic Skeletal Hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is an exceptional finding in athletes but may increase spinal injury risk. We report 2 former elite rugby players who, during amateur play, sustained cervical hyperflexion injuries revealing probable DISH. Case 1: transient paraparesis from cervicothoracic ankylosis and cord compression, treated with posterior decompression. Case 2: transient monoparesis with C1-C2 instability and anterior osteophytes, managed by Magerl fixation. Spinal rigidity from DISH heightens fracture and neural injury risk in collision athletes. Magnetic resonance imaging …

Concussion-Related Cognitive and Lipid Changes in Retired UK Rugby Players Study.

Long-term effects of concussions, particularly in contact sport athletes, have been linked to changes in neuronal health. Lipid dysregulation has emerged as a potential contributor to neuronal injury and may serve as a measurable biomarker of brain pathology. This study investigated cognitive scores and serum lipid biomarkers in retired rugby players with a history of concussion to assess their association with concussion exposure. Serum levels of 24-hydroxycholesterol (24-HC), 25-HC, 27-HC, total triglycerides, and ceramide were compared between retired rugby players with a history of repeate…

Establishing reliable acceleration-speed profiles: Minimum data requirements in rugby union matches.

The aim of this study was to determine the minimum playing time in matches required to produce reliable acceleration - speed (AS) profiles from global navigation satellite system (GNSS) data. The playing time of 25 professional rugby union players was recorded using GNSS technology and segmented into four sections from 40, 80, 120 and 160 minutes of play. For each interval, the impact of integrating new match data on the AS profile was assessed, aiming to identify the saturation point where additional data did not induce significant changes in the profile outputs. A repeated measures ANOVA and…

Ketone Monoester Ingestion Alters Metabolism and Simulated Rugby Performance in Professional Players.

Ketone ingestion can alter metabolism but effects on exercise performance are unclear, particularly with regard to the impact on intermittent-intensity exercise and team-sport performance. Nine professional male rugby union players each completed two trials in a double-blind, randomized, crossover design. Participants ingested either 90 ± 9 g carbohydrate (CHO; 9% solution) or an energy matched solution containing 20 ± 2 g CHO (3% solution) and 590 mg/kg body mass β-hydroxybutyrate monoester (CHO + BHB-ME) before and during a simulated rugby union-specific match-play protocol, including repeat…

Can tackle height influence head injury assessment risk in elite rugby union?

OBJECTIVES: Tackle height laws are an area of controversy in rugby union. It is reported that the tackler is at most risk of a Head Injury Assessment (HIA). Therefore, the aim of this study was to use match video evidence of tackles in elite level rugby union to examine the effect of tackle heights on HIA risk for the tackler. DESIGN: Qualitative observational case-control study. METHODS: Each HIA (n=74) and control tackle (n=965) was categorised based on tackle direction (front- or side-on), tackle type (arm, shoulder or smother) and tackle height (upper trunk, mid-trunk, lower trunk, upper…

Subclavian Artery Pseudoaneurysm Formation 3 Months after a Game of Rugby Union.

Pseudoaneurysms of the subclavian artery remain a rare complication after fracture of the clavicle. We report a case of delayed diagnosis of a subclavian artery pseudoaneurysm after a closed fracture of the clavicle in a 15-year-old patient, 3 months after the original injury while playing rugby union. Despite several attendances to the Emergency Department with vague symptoms, the final diagnosis was confirmed by duplex ultrasound and Computed Tomography of the thorax. Surgical repair was indicated due to acute limb ischaemia from distal embolisation from a large pseudoaneurysm, with the pati…

Is return to professional rugby union likely after anterior cervical spinal surgery?

We have examined the outcome in 19 professional rugby union players who underwent anterior cervical discectomy and fusion between 1998 and 2003. Through a retrospective review of the medical records and telephone interviews of all 19 players, we have attempted to determine the likelihood of improvement, return to professional sport and the long-term consequences. We have also attempted to relate the probability of symptoms in the neck and radicular pain in the arm to the position of play. Neck and radicular pain were improved in 17 patients, with 13 returning to rugby, the majority by six mont…

Recurrent posterior thigh symptoms detrimental to performance in rugby union: predisposing factors.

Recurrent hamstring injury is a very common problem in rugby union, but has been largely ignored in the literature. It is concluded that a multifactorial aetiology may be present, and that these symptoms may be part of a continuum of symptoms that may lead to more serious injuries. Effective management needs to focus not just locally, but include proximal issues such as lumbo-pelvic stability and correcting lumbar spine dysfunction. The factors addressed in a prevention programme are likely to contribute to performance gains.
