We present a case of penile and scrotal oedema in a young man. Although oestradiol treatment and compression bandaging provided some relief from the symptoms, neither therapy was ideal. Triangular resection of the foreskin was therefore performed, although the oedema persisted. The aetiology of the condition remains unknown.

A 65-year-old man presented with a complaint of the swelling of his right scrotum. Ultrasonogram indicated a tumor in the right scrotum, but it was not apparent whether the tumor arose from the right testis or not. Right high orchiectomy was performed due to the suspicion of malignancy. The tumor arose from the right epididymis and spermatic cord. Histological examination revealed that the tumor was a fibrous pseudotumor in the scrotum.

A case of intrascrotal leiomyoma is reported. A 62-year-old male was admitted to our hospital with the chief complaint of a painless intrascrotal mass, which had been gradually increasing in size during the past 25 years. An 80-year-old man with a 7 year history of a slowly enlarging, asymptomatic scrotal nodule is presented. He had a negative history for sexually transmitted disease, trauma to the area, radiotherapy and chemical or arsenic exposure. The lesion was excised with a margin of 0.8 cm of normal skin. Examination of the specimen revealed a basal cell carcinoma.

Paracoccidioidomycosis is a systemic fungal infection caused by the thermally dimorphic fungus Paracoccidioides brasiliensis. The authors present a case of a 49-year-old man who developed verrucous lesions on nasal mucosa and on genital localization (inguinal and scrotal), without signs of systemic disease. Direct mycological examination, culture on Sabouraud glucose medium, and biopsy were positive for P. brasiliensis. The interest of this case is the genital localization which is uncommon in this infection. Moreover, there are few cases described in the literature.

A case of leiomyosarcoma of the scrotum is reported. A 44-year-old man referred to our hospital with the complaint of swelling of the left scrotal contents. The lesion was about 3 cm in diameter, isolated from testis, epididymis and spermatic cord. Ultrasonography revealed hypoechoic and slightly heterogenic area in the lesion. Surgical resection was easily done. Histological examination showed leiomyosarcoma. Adjuvant therapy was not performed. Three years after the resection, he is alive without any sign of recurrence or metastasis.

We report a rare case of Merkel cell carcinoma arising from the scrotal skin of an 84-year-old man. The tumor was treated by wide, local excision and radiation. The man died due to extensive metastatic disease 16 months after his surgery. The natural history, histology, and pathogenesis of Merkel cell carcinoma are discussed briefly.

Scrotal abnormalities are difficult to assess using clinical criteria alone. Ultrasound provides an accurate means of demonstrating the scrotal contents so that appropriate therapy may be instituted. In a retrospective study, 119 ultrasound examinations of 96 patients (aged 4 days to 23 years) have been compared with the clinical diagnosis, surgical/pathologic findings, and other imaging modalities. The application of an orderly diagnostic protocol to the conditions causing the acute scrotum will allow all cases to be classified rapidly, with either a presumptive diagnosis of torsion or other surgical conditions that require surgery or exclusion of torsion with confidence. It is imperative, however, in those cases in which the acute scrotum is not operated on that sufficiently long follow-up be done to prove that in each and every case the testis and scrotal contents return to normal, so that an occult neoplasm will not be innocently neglected.
