A 46-year-old man with excruciating shoulder pain.

A 46-year-old man with no significant medical history presented to his local emergency department complaining of excruciating right shoulder pain. The patient was in his usual state of excellent health until 4 days prior, when right shoulder pain developed while he was using a chainsaw to cut wood. The next day, flu-like symptoms developed with fevers, chills, and headache. An MRI revealed that the right pectoralis major was torn from its attachment to the acromion. His shoulder pain intensified despite treatment with hydrocodone and acetaminophen, and the flu-like symptoms progressed over the

Hemiplegic shoulder pain: defining the problem and its management.

PURPOSE: Hemiplegic shoulder pain can affect up to 70% of stroke patients and can have an adverse impact on rehabilitation outcomes. This article aims to review the literature on the suggested causes of hemiplegic shoulder pain and the therapeutic techniques that can be used to prevent or treat it. On the basis of this review, the components of an optimal management programme for hemiplegic shoulder pain are explored. METHOD: English language articles in the CINAHL and MEDLINE databases between 1990 and 2000 were reviewed. These were supplemented by citation tracking and manual searches. RES

Comparison of Shoulder Ultrasonographic Assessments between Polymyalgia Rheumatica and Frozen Shoulder in Patients with Bilateral Shoulder Pain: A Comparative Retrospective Study.

This study aimed to assess and compare the ultrasonographic (US) pathologic findings in patients with polymyalgia rheumatica (PMR) and bilateral frozen shoulder (FS). We included 19 patients with clinically diagnosed PMR and 19 patients with stage II bilateral FS. The US evaluation included the assessment of subacromial-subdeltoid (SASD) bursitis, long head of biceps (LHB) tenosynovitis, and posterior and inferior glenohumeral (GH) synovitis. Unilateral SASD bursitis was noted significantly more frequently in PMR patients than in bilateral FS patients (p = 0.001). There were no significant dif

Diaphragmatic hernia with isolated shoulder pain evoked by surfeit.

Bochdalek hernia in adult is extremely rare, so symptoms or indications of surgery are not reported enough. Here we report a case of small-sized Bochdalek hernia with isolated shoulder pain that resolved after surgical reduction of hernia. A 25-year-old man with diaphragmatic mass was referred to out-patient clinic. Chief complaint was an isolated left shoulder pain evoked by surfeit. Diaphragmatic hernia was suggested on chest computed tomography (CT) but it was not certain. So, video-assisted thoracic surgery (VATS) exploration was planned to clarify the diagnosis. The mass was proven to be

Clinical evaluation of nonarthritic shoulder pain: Diagnosis and treatment.

Shoulder pain and dysfunction is a complex problem frequently encountered by primary care physicians. Common nonarthritic conditions seen in the primary care setting include rotator cuff syndrome, impingement, posttraumatic stiffness, adhesive capsulitis, and instability. A thorough history and physical examination can aid in the diagnosis of many common shoulder complaints. Pain and instability are the most common shoulder complaints. Pain that is sharp or burning is commonly radicular in origin, whereas pain caused by tendinitis is often dull, diffuse, and aching. Instability is frequently f

Periscapular abscess: unusual cause of shoulder pain in children.

Periscapular abcess is an extremely rare clinical condition. Diagnosis and treatment can be delayed because the clinical picture often resembles septic arthritis of the shoulder. Early diagnosis and prompt surgical treatment are mandatory, as delayed treatment can be fatal due to sepsis. Previously, four cases of periscapular abcess were described in English literature. We describe two pediatric patients with periscapular abcess, who were treated surgically and healed without any complication.

Underlying pathology and associated factors of hemiplegic shoulder pain.

The prevalence of hemiplegic shoulder pain is approximately 22%-23% in the general population of stroke survivors and approximately 54%-55% among stroke patients in rehabilitation settings. Hemiplegic shoulder pain causes a reduced quality-of-life, poor functional recovery, depression, disturbed sleep, and prolonged hospitalization. Herein, we attempted to understand, based on a literature review and experts' opinion, the pathologic processes underlying hemiplegic shoulder pain and the major associated factors contributing to its development. The systematization of underlying pathologies was p

[Shoulder pain: assessment, diagnosis and treatment of common problems].

Shoulder pain is a common complaint seen at the primary physician's clinic, and it is the third most common musculoskeletal complaint after back and neck pain. Shoulder pain can have a wide range of etiologies: trauma caused from a sprain or a simple muscle strain to a large tear of one of the shoulder stabilizer muscles. Some shoulder pathologies can cause chronic pain and limitation in shoulder range of motion such as impingement syndrome, adhesive capsulitis, calcified tendonitis, cervical radiculopathy, glenohumeral osteoarthritis, and biceps tendonitis.
