[Shoulder arthroplasty complications: case report].

BACKGROUND: Infection and instability are the complications of prosthetic shoulder surgery of more difficult management. Instability builds up to half of the cases, while the fungal infection accounts for less than 1% of all and can cause severe damage to bone and soft tissue. METHODS: In this case clinical fungal infection by Candida parapsilosis is presented in a shoulder hemiarthroplasty indicated by a fracture of the proximal humerus. He received surgical treatment in two-stages and administration of fluconazole for eight weeks prior to the second surgical time. In revision surgery was pl

Functional Outcomes and Complications After Oncologic Reconstruction of the Proximal Humerus.

BACKGROUND: No consensus exists on the best method of articular reconstruction in patients who require proximal humerus resection for the management of primary bone sarcomas, soft-tissue sarcomas extending into the bone, benign and locally aggressive primary bone tumors, and metastatic disease. METHODS: We identified patients from two institutions who underwent wide resection of the proximal humerus along with oncologic reconstruction using osteoarticular allografts (OAs), endoprostheses, or allograft-prosthesis composites. We prospectively collected functional outcomes and retrospectively as

Shoulder arthroplasty in the young patient.

We reviewed the performance of 22 shoulder implant arthroplasties in 19 patients age 50 or younger at surgery. Patients were evaluated by telephone interview, written questionnaire, and radiographic examination. The average age at surgery was 38.6 years, and the average follow-up was 5.6 years. Patients were grouped by disease process (rheumatoid arthritis [RA], avascular necrosis [AVN], trauma, and hemophilic arthropathy [HA]) and type of prosthesis (total vs. hemiarthroplasty). The highest scores (best shoulder function) were seen in patients with hemiarthroplasty for HA. Patients with hemia

Shoulder arthroplasty for the treatment of rotator cuff insufficiency.

Irreparable rotator cuff tendon tears result from chronic tears, failed cuff repairs, and fracture sequelae and occur in patients with rheumatoid arthritis. The management of patients with cuff tear arthropathy can be challenging. When pain is severe and function is poor, surgical options include hemiarthroplasty, bipolar arthroplasty, extended head arthroplasty, arthroplasty with tendon transfer, reverse shoulder arthroplasty, and fusion. A review of the literature shows good pain relief with hemiarthroplasty in carefully selected patients; however, the reverse prosthesis has been found to be

Dynamic balance of persons with unilateral upper limb absence when responding to a walking disturbance.

BACKGROUND: A high prevalence of falls has been reported in individuals with upper limb absence (ULA). This prevalence is increased in upper limb prosthesis users. It is possible that ULA and prosthesis use may alter recovery mechanisms in response to a perturbation. RESEARCH QUESTION: The purpose of this study was to investigate the reactive response of individuals with unilateral transradial ULA to perturbations during walking compared to control participants, and to determine the effect of prosthesis use on perturbation response strategies and resultant dynamics. METHODS: 10 upper limb pr

Use of the subscapularis preserving technique in anatomic total shoulder arthroplasty.

Subscapularis tenotomy for total shoulder arthroplasty has been the standard approach for shoulder surgeons that utilize the deltopectoral approach. The risk of subscapularis insufficiency after this approach has been well documented. In order to avoid subscapularis complications, Lafosse reported a technique for total shoulder arthroplasty that utilizes a trans-deltoid approach through the rotator interval that yielded satisfactory clinical outcomes. However, he also reported concerns about humeral head undersizing and inadequate osteophyte resection. We present an alternative subscapularis p

Inverted-bearing reverse total shoulder arthroplasty: scapular notching does not affect clinical outcomes and complications at up to 7 years of follow-up.

BACKGROUND: Scapular notching is a common complication of reverse total shoulder arthroplasty (RTSA). Inverted-bearing RTSA (IB-RTSA) systems, with polyethylene (PE) glenospheres and metal or ceramic humeral liners, reduce notching and PE wear compared with traditional Grammont prosthesis designs. However, whether notching after IB-RTSA influences clinical outcomes or complications remains unknown. Therefore, we evaluated the influence of notching on midterm clinical outcomes and complication rates after IB-RTSA. METHODS: In our prospective multicenter, observational study, patients underwent

Humeral Head Reconstruction in Anatomic Shoulder Arthroplasty: How to Assess It, How to Avoid Overstuffing, and Whether It Matters.

➢ Humeral head anatomy affects the tension and mechanics of the glenohumeral joint. Thus, aiming for anatomic reconstruction can help to avoid negative consequences of component malpositioning (such as "overstuffing") on soft-tissue tension and impingement-free range of motion.➢ The most common method to assess humeral head reconstruction is comparing the prosthetic humeral articular surface with the "perfect circle" incorporating the lateral cortex of the greater tuberosity, the medial greater tuberosity, and the medial calcar at the anatomic neck. Although this method is quick and helpful in

The Influence of Component Design and Positioning on Soft-Tissue Tensioning and Complications in Reverse Total Shoulder Arthroplasty: A Review.

» Reverse total shoulder arthroplasty was designed to function in the rotator cuff deficient shoulder by adjusting the glenohumeral center of rotation (COR) to maximize deltoid function.» Adjustments in the COR ultimately lead to changes in resting tension of the deltoid and remaining rotator cuff, which can affect implant stability and risk of stress fracture.» Soft-tissue balance and complication profiles can be affected by humeral component (version, neck shaft angle, and inlay vs. onlay) and glenoid component (sagittal placement, version, inclination, and lateralization) design and applica

Reverse shoulder arthroplasty implant design and configuration has a significant effect on conjoint tendon impingement.

BACKGROUND: Anterior shoulder pain after reverse shoulder arthroplasty (RSA) is not uncommon and may be due to humeral impingement against the conjoint tendon during internal rotation (IR). It is unknown what effect different implant designs and configurations have on conjoint tendon impingement. The purpose of this study was to investigate the influence of several RSA implant design parameters on conjoint tendon impingement during IR. METHODS: Twelve upper extremity cadavers were dissected to visualize and digitize the path of the coracobrachialis using a tracking system. This data were tran

An analysis of variability and terminology inconsistencies in the amount and type of constraint in polyethylene bearing liners across reverse shoulder arthroplasty implant systems.

BACKGROUND: This study aims to evaluate the amount and type of constraint between different reverse total shoulder arthroplasty (rTSA) systems to address a current gap in uniform nomenclature and help guide surgical decision-making in rTSA. METHODS: Data were collected on commercially available polyethylene liners for rTSA implant manufacturers in the United States. Constraint ratios were calculated using liner depth and glenosphere radius, and for selected designs, lip heights were used. All available sizes and diameters were analyzed, and data were cross-referenced with published literature

Central-peg radiolucency progression of an all-polyethylene glenoid with hybrid fixation in anatomic total shoulder arthroplasty is associated with clinical failure and reoperation.

BACKGROUND: Glenoid component loosening is a common cause of failure after anatomic total shoulder arthroplasty. Prior studies of all-polyethylene glenoid implants with hybrid fixation did not show early glenoid radiolucency to be clinically significant. The purpose of this study was to determine the clinical significance of progression of radiolucency around the central peg of the glenoid component. METHODS: We identified 73 shoulders that underwent primary anatomic total shoulder arthroplasty between January 1995 and May 2015 for osteoarthritis with an all-polyethylene pegged glenoid, with
