A 64-year-old male with a 40-year history of dermatologic disorders is presented. The patient carries diagnoses of neurodermatitis and suspected myosis fungoides on which has been superimposed a factitial dermatitis requiring increasingly extended hospitalizations. Given a very disturbed home life and personality style, the patient showed no incentive for change, and his family and caregivers became increasingly frustrated and distraught. An operant behavioral approach proved helpful to staff in standardizing care and setting realistic expectations. This led to a reversal of a downward course

In this study we conducted a resurvey at 33 months of elderly general medical clinic outpatients previously classified as depressed or not using the Zung Self-Rating Depression Scale. Emphasis on the process and content of the emotional aspects of rehabilitation are presented as a process interwoven with a patient's physical rehabilitation. The impact of trauma and subsequent rehabilitation involves for patients the resolution of these stages: shock, denial, depressive reaction, reaction against independence, and adaptation. Factors which facilitate and impede the negotiation of these steps on both the part of the patient and attending staff are examined from the prospective of the psychiatrist as a member of the medical rehabilitation team.

The hypochondriacal patient presents numerous frustrations for the physician. The typical hypochondriac is a middle-aged person dominated by a preoccupation with bodily symptoms. Bereavement, serious life failures, and severe blows to self-esteem put patients at high risk for somatization. Hypochondrisis has been cited as a symptom of masked depression, early paranoid schizophrenia, and dementia and/or delirium. A careful diagnostic work-up with attention to any correctable psychiatric illness sets the stage for regularly scheduled, long-term supportive therapy.

The HIV-associated lipodystrophy syndrome is a new disease entity and a result of the otherwise successful antiretroviral therapy. It consists of metabolic disorders which may in part lead to an increased cardiovascular risk or hyperlactatemia resulting in increased morbidity and mortality. The change in body shape is often disfiguring and diminishes the quality of life of the affected individual substantially. The present paper reviews the current options and limits to treat the various components of the syndrome.

In the former western part of Germany, in 1989 about three million people claimed social costs because of urinary incontinence. 80 percent of them were women. The psychic situation of these women is mainly determined by social consequences of their incontinence. Behavioural changes to avoid incontinence lead to social isolation. A coherence between incontinence or urgency and agony or depression is acknowledged. Especially in elderly people, who have to change their well-known environment due to nursing problems, incontinence is a major problem, which often deteriorates with the new situation.

AIDS has a unique set of characteristics that makes group psychotherapy more complex than with other populations: (1) the threat of an early death, (2) a highly variable course of illness, and (3) stigma related to the illness and to the preexisting lifestyles of most patients. Neurasthenia is a diagnostic entity included in ICD-9 which is used in daily clinical practice in China and the Soviet Union, as well as some other Asian countries, so far as I know.
