[Olfactory dysfunction in Parkinson disease and REM sleep behavior disorder].

Olfactory dysfunction is an appealing biomarker for Parkinson disease (PD) because of the high prevalence (>90%) among PD patients and the relative ease of testing, as compared to other putative biomarkers such as neuroimaging of the dopamine system. Hyposmia develops during the early stages of PD and, therefore, may be one of the most sensitive markers for the early diagnosis of PD. Sniffin' Sticks, the University of Pennsylvania Smell Identification Test, and the Odor Stick Identification test for the Japanese, a short and simple nonlexical, olfactory identification test, can be useful for d

Myotonic dystrophy type 1, daytime sleepiness and REM sleep dysregulation.

Myotonic dystrophy type 1 (DM1), or Steinert's disease, is the most common adult-onset form of muscular dystrophy. DM1 also constitutes the neuromuscular condition with the most significant sleep disorders including excessive daytime sleepiness (EDS), central and obstructive sleep apneas, restless legs syndrome (RLS), periodic leg movements in wake (PLMW) and periodic leg movements in sleep (PLMS) as well as nocturnal and diurnal rapid eye movement (REM) sleep dysregulation. EDS is the most frequent non-muscular complaint in DM1, being present in about 70-80% of patients. Different phenotypes

Localization of the brainstem GABAergic neurons controlling paradoxical (REM) sleep.

Paradoxical sleep (PS) is a state characterized by cortical activation, rapid eye movements and muscle atonia. Fifty years after its discovery, the neuronal network responsible for the genesis of PS has been only partially identified. We recently proposed that GABAergic neurons would have a pivotal role in that network. To localize these GABAergic neurons, we combined immunohistochemical detection of Fos with non-radioactive in situ hybridization of GAD67 mRNA (GABA synthesis enzyme) in control rats, rats deprived of PS for 72 h and rats allowed to recover after such deprivation. Here we show

Sawtooth wave density analysis during REM sleep in normal volunteers.

OBJECTIVE: Sawtooth waves (STW) are a characteristic EEG feature of REM sleep but their source and function are unknown. We previously reported stereotypical properties of STW at stage REM onset, and alterations in bulbar postpolio syndrome. This study analyzes STW features throughout REM, in order to test the hypothesis that sawtooth wave activity may be predictable and have a consistent relationship across REM periods. METHODS: Twenty polysomnographic recordings were scored for occurrence, duration, and frequency of STWs. STW density was calculated based on the number of bursts/min REM and

Motor disturbance during REM sleep in group A xeroderma pigmentosum.

We investigated motor phenomena during rapid eye movement (REM) sleep in 13 patients with group A xeroderma pigmentosum aged from 11 to 39 months, and compared them with those obtained from 12 age-matched controls. At the time of sleep study, they had no abnormality on routine electrophysiological examinations. The amount of REM sleep and the incidence of motor phenomena during REM sleep in patients were similar to those in age-matched controls. However using the newly designated indices, we demonstrated disturbance on both the tonic motor inhibition occurring during the whole REM sleep period

Cortical asymmetry of REM sleep EEG following unilateral pontine hemorrhage.

A 24-year-old woman with a left pontine hematoma showed marked asymmetry in the EEG of REM sleep, suggesting that a unilateral pontine lesion is sufficient to disrupt normal REM sleep EEG in the ipsilateral hemisphere. Other REM sleep characteristics (rapid eye movements, muscle atonia) were unaffected by this lesion.

The importance of REM sleep fragmentation in the effects of stress on sleep: Perspectives from preclinical studies.

Psychological stress poses a risk for sleep disturbances. Importantly, trauma-exposed individuals who develop posttraumatic stress disorder (PTSD) frequently report insomnia and recurrent nightmares. Clinical studies have provided insight into the mechanisms of these sleep disturbances. We review polysomnographic findings in PTSD and identify analogous measures that have been made in animal models of PTSD. There is a rich empirical and theoretical literature on rapid eye movement sleep (REMS) substrates of insomnia and nightmares, with an emphasis on REMS fragmentation. For future investigatio

Deficiency of orexin signaling during sleep is involved in abnormal REM sleep architecture in narcolepsy.

Narcolepsy is a sleep disorder caused by deficiency of orexin signaling. However, the neural mechanisms by which deficient orexin signaling causes the abnormal rapid eye movement (REM) sleep characteristics of narcolepsy, such as cataplexy and frequent transitions to REM states, are not fully understood. Here, we determined the activity dynamics of orexin neurons during sleep that suppress the abnormal REM sleep architecture of narcolepsy. Orexin neurons were highly active during wakefulness, showed intermittent synchronous activity during non-REM (NREM) sleep, were quiescent prior to the tran

EEG sleep in depression and in remission and the REM sleep response to the cholinergic agonist RS 86.

A comparison of the sleep EEG patterns of patients with a major depressive disorder intraindividually between remitted and depressed state revealed an improvement of parameters of sleep continuity and a tendency for normalization of rapid eye movement (REM) latency and REM density in the former. Additional application of the cholinergic agonist RS 86 prior to sleep did not reveal a heightened sensitivity of the REM sleep system in the remitted sample. Whereas a group of presently ill depressives displayed a drastic reduction of REM latency, results of the remitted patients were comparable to h

REM Sleep Impairment May Underlie Sleep-Driven Modulations of Tinnitus in Sleep Intermittent Tinnitus Subjects: A Controlled Study.

(1) Background: Poor sleep and fragmented sleep are associated with several chronic conditions. Tinnitus is an auditory symptom that often negatively combines with poor sleep and has been associated with sleep impairment and sleep apnea. The relationship between tinnitus psychoacoustic characteristics and sleep is still poorly explored, notably for a particular subgroup of patients, for whom the perceived loudness of their tinnitus is highly modulated by sleep. (2) Methods: For this observational prospective study, 30 subjects with tinnitus were recruited, including 15 "sleep intermittent tinn

REM sleep proportion: an independent predictor of olfactory dysfunction in obstructive sleep apnea hypopnea syndrome.

OBJECTIVE: This study aimed to investigate the association between olfactory dysfunction and sleep architecture parameters in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS), identify independent factors influencing olfactory function, and offer novel insights into the multisystemic impact of OSAHS. METHODS: Employing a cross-sectional retrospective design, data from 198 patients diagnosed with OSAHS via polysomnography (PSG) from December 1, 2020, to June 30, 2024, were analyzed. Olfactory performance was assessed using the Sniffin' Sticks test kit. Sleep structure parameters

REM sleep fragmentation and arousability distinguish chronic insomnia subtypes and reflect subjective-objective sleep discrepancy.

STUDY OBJECTIVES: The objective was to characterize rapid eye movement (REM) sleep structure in chronic insomnia patients with objective polysomnography (PSG)-defined macrostructural abnormalities (CIN-AP) or without such abnormalities (CIN-NP), and to determine how these subtypes differ from each other and from good sleepers (GS). METHODS: We analyzed 1049 participants: 141 GS, 214 CIN-NP, and 694 CIN-AP. CIN was defined by ICSD-3 criteria. Automated algorithms quantified sleep stages, arousals, and REM features including percentage and duration (total, phasic, tonic), number of arousals and

Polysomnographic biomarkers for persisting (isolated) REM sleep without atonia in patients with obstructive sleep apnea.

BACKGROUND: The relationship between obstructive sleep apnea (OSA) and REM sleep without atonia (RSWA) is complex and not fully understood. While RSWA may prevent upper airway collapse during REM sleep, it can also mimic REM sleep behavior disorder (RBD) and be a potential biomarker for neurodegeneration. This study aims to identify the prevalence, characteristics, and predictors of persistent isolated RSWA (iRSWA) following positive airway pressure (PAP) therapy in OSA patients. METHODS: A retrospective analysis was conducted on 297 patients with both OSA and RSWA who underwent polysomnograp

Periodic Leg Movements during Sleep Associated with REM Sleep Behavior Disorder: A Machine Learning Study.

Most patients with idiopathic REM sleep behavior disorder (iRBD) present peculiar repetitive leg jerks during sleep in their clinical spectrum, called periodic leg movements (PLMS). The clinical differentiation of iRBD patients with and without PLMS is challenging, without polysomnographic confirmation. The aim of this study is to develop a new Machine Learning (ML) approach to distinguish between iRBD phenotypes. Heart rate variability (HRV) data were acquired from forty-two consecutive iRBD patients (23 with PLMS and 19 without PLMS). All participants underwent video-polysomnography to confi

Peculiar respiratory response observed during sleep-onset REM sleep of an infant with Ondine's curse.

We treated an infant with congenital central hypoventilation syndrome ("Ondine's curse"). She was cyanotic and given ventilatory support at the first hour after birth. An investigation of sleep state and respiration performed at the age of 3 months led to this diagnosis. Hypoventilation persisted in all sleep stages, with the most severely reduced tidal volumes occurring during delta-wave sleep (stages 3 and 4). In addition, severe secondary reduction in tidal volumes occurred in sleep-onset REM sleep. This phenomenon was absent in non sleep-onset REM sleep. At 4 months of age, her respiratory

Exogenous Ketosis Improves Sleep Efficiency and Counteracts the Decline in REM Sleep after Strenuous Exercise.

INTRODUCTION: Available evidence indicates that ketone bodies may improve sleep quality. Therefore, we determined whether ketone ester (KE) intake could counteract sleep disruptions induced by strenuous exercise. METHODS: Ten well-trained cyclists with good sleep quality participated in a randomized crossover design consisting of two experimental sessions each involving a morning endurance training and an evening high-intensity interval training ending 1 h before sleep, after which polysomnography was performed overnight. Postexercise and 30 min before sleeping time, subjects received either

Correlates of sleep-onset REM periods during the Multiple Sleep Latency Test in community adults.

The diagnosis of narcolepsy without documented cataplexy is based on the observation of two or more sleep-onset REM periods (SOREMPs) during the Multiple Sleep Latency Test (MSLT). We report on the prevalence and correlates of SOREMPs in the community-based Wisconsin Sleep Cohort Study. MSLTs were conducted following nocturnal polysomnography (NPSG) and daily sleep diaries in 289 males and 267 females (age 35-70, 97% Caucasians). Multiple SOREMPs were observed in 13.1% of males and 5.6% of females. An MSLT mean sleep latency < or =8 min and > or =2 SOREMPs (diagnostic of narcolepsy) was observ
