Factors associated with sleep disturbances in women undergoing treatment for early-stage breast cancer.

PURPOSE: The purpose of this study was to examine factors associated with sleep disturbance in women receiving adjuvant therapy for breast cancer. METHODS: This study employed a cross-sectional design using data collected at 3 months post-surgery from an ongoing longitudinal parent study. Participant data were divided into adjuvant treatment groups (chemotherapy, radiation, and aromatase inhibitors) and no adjuvant treatment groups. Symptoms were measured using patient self-report measures. Analysis of variance was used to assess between adjuvant treatment group differences in sleep disturban

A pilot staging system to predict persistent obstructive sleep apnea in children following adenotonsillectomy.

OBJECTIVES/HYPOTHESIS: A recent meta-analysis by Friedman et al. found that 66% of children have resolution of obstructive sleep apnea (OSA) after tonsillectomy and adenoidectomy (TA). However, accurate predictors of persistent OSA in children despite surgery are difficult to identify. We evaluate the utility of staging children with OSA using characteristics predictive of adult palatoplasty success. STUDY DESIGN: A retrospective analysis of children who underwent sleep studies before and after TA in a tertiary care center from 2008 through 2011. METHODS: Tonsil size (TS)/modified Mallampati

Discrepancy between subjective and objective sleep disturbances in early- and moderate-stage Alzheimer disease.

OBJECTIVE: Sleep disturbances such as nocturnal awakenings frequently occur in demented elderly persons and can contribute to depression, cognitive impairment, and caregiver burden. Recognizing sleep disturbances at an early stage of the disease progress is a first prerequisite of intervention and monitoring of progress. This study aimed to investigate the reliability of subjective sleep reports in early- and moderate-stage Alzheimer dementia (AD), by investigating whether they differ from matched healthy normal comparison groups with respect to the discrepancy of subjective and objective slee

Modelling population heterogeneity in sensitivity and specificity of a multi-stage screen for obstructive sleep apnoea.

We propose a model-based approach for modelling population heterogeneity in terms of sensitivity and specificity of multi-stage screening procedures that consists of multiple tests ordered according to criteria such as cost and invasiveness. It is assumed that a patient proceeds to the next test only if they test positive for the current test. An overall positive result occurs if a patient tests positive for all tests. A dropout occurs when a subject tests positive for a test but does not proceed to subsequent tests. Chinchilli proposed estimates of sensitivity and specificity based upon ratio

Sleep apnea syndrome and end-stage renal disease. Cure after renal transplantation.

We report two patients undergoing maintenance hemodialysis who presented with sleep apnea syndrome (SAS). The first patient is a 36-year-old man with a terminal Berger's glomerulopathy and associated obstructive sleep apnea syndrome (OSAS) (apnea-hypopnea index [AHI] = 80). He was receiving home hemodialysis and was treated by nasal continuous positive airway pressure (CPAP). After successful renal transplantation, his symptoms completely disappeared, and control polysomnography greatly improved (AHI = 9). The second patient had hypokalemic nephropathy with severe, uncontrolled hypertension an

Clinical Severity Staging System and Response to Hypoglossal Nerve Stimulation for Obstructive Sleep Apnea.

IMPORTANCE: While hypoglossal nerve stimulators (HGNS) have been shown to be associated with reduced apnea-hypopnea index (AHI) scores in many continuous positive airway pressure-intolerant patients with obstructive sleep apnea (OSA), it is not effective for all patients, and it is unclear who would benefit most. OBJECTIVE: To explore baseline factors associated with response to HGNS and create a prognostic stratification system. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included patients with OSA who underwent implant with an HGNS from April 2019 to October 2023 at

Wearable Sleep-Stage Classification: A Systematic Analysis of Signal Modalities, Representations, and Multimodal Fusion.

Accurate sleep staging is essential for characterizing sleep architecture, yet conventional polysomnography limits scalable and long-term sleep monitoring. Wearable sensors offer a practical alternative, but their performance depends on the physiological modalities used and how the signals are represented. This study investigates the effects of signal modality, feature representation, and sleep-stage granularity on subject-independent wearable sleep staging. Using DREAMT, we compare handcrafted features, short-time Fourier transform (STFT) representations, and their fusion across three-, four-

Automatic sleep staging and atonia detection in RBD using single-channel chin EMG.

Study ObjectivesThe diagnosis of rapid eye movement (REM) sleep behavior disorder (RBD) is of clinical interest, as there is a high rate of future conversion to Parkinson's disease and related neurodegenerative disorders. The clinical standard for diagnosis of RBD relies on human scoring of REM sleep without atonia (RSWA) via polysomnography, which is labor-intensive, costly, and unsuitable for long-term monitoring. Existing (semi-)automatic detection methods often depend on prior manual annotation of REM or suffer from low REM classification performance in this population. We here propose a f

Diagnostic accuracy of a two-stage model for detecting obstructive sleep apnoea in chronic tetraplegia.

BACKGROUND: Obstructive sleep apnoea (OSA) is highly prevalent in people with spinal cord injury (SCI). Polysomnography (PSG) is the gold-standard diagnostic test for OSA, however PSG is expensive and frequently inaccessible, especially in SCI. A two-stage model, incorporating a questionnaire followed by oximetry, has been found to accurately detect moderate to severe OSA (MS-OSA) in a non-disabled primary care population. This study investigated the accuracy of the two-stage model in chronic tetraplegia using both the original model and a modified version for tetraplegia. METHODS: An existin

Effect of Aromatase Inhibitor Therapy on Sleep and Activity Patterns in Early-stage Breast Cancer.

INTRODUCTION: Adherence to aromatase inhibitor (AI) therapy is poor, often because of treatment-emergent side effects, including musculoskeletal symptoms, fatigue, and insomnia. In the present analysis, we examined the sleep patterns and daytime function both objectively using actigraphy and subjectively using validated questionnaires in women initiating AI therapy. PATIENTS AND METHODS: Postmenopausal women with stage 0-III hormone receptor-positive breast cancer who were initiating AI therapy were eligible. The patients wore actigraphy devices for 10 consecutive days and completed questionn

Sleep-stage transitions during polysomnographic recordings as diagnostic features of type 1 narcolepsy.

OBJECTIVE: Type 1 narcolepsy/hypocretin deficiency is characterized by excessive daytime sleepiness, sleep fragmentation, and cataplexy. Short rapid eye movement (REM) latency (≤15 min) during nocturnal polysomnography (PSG) or during naps of the multiple sleep latency test (MSLT) defines a sleep-onset REM sleep period (SOREMP), a diagnostic hallmark. We hypothesized that abnormal sleep transitions other than SOREMPs can be identified in type 1 narcolepsy. METHODS: Sleep-stage transitions (one to 10 epochs to one to five epochs of any other stage) and bout length features (one to 10 epochs) w

The Relationship Between Volume Overload in End-Stage Renal Disease and Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is common, underdiagnosed, and undertreated among patients with end-stage renal disease (ESRD). As in all cases, pathogenesis of OSA is related to repeated upper airway (UA) occlusion or narrowing, but in ESRD, additional contributory factors likely include uremic destabilization of central respiratory control and anatomic changes in the UA related to fluid status. Pulmonary congestion is common in acute and chronic kidney failure and is a consequence of cardiomyopathy and fluid overload, two potentially reversible risk factors. Emerging evidence suggests that vol

Interexaminer agreement of Friedman tongue positions for staging of obstructive sleep apnea/hypopnea syndrome.

OBJECTIVE: To determine if strong interexaminer agreement exists in identifying Friedman tongue positions (FTPs) for staging of obstructive sleep apnea/hypopnea syndrome (OSAHS). STUDY DESIGN AND METHODS: Twenty-six digital video clips of oropharyngeal examinations were prepared to demonstrate FTPs I through IV. Specific parameters for each FTP were provided to 126 examiners (otolaryngology residents, attendings, and fellows). The video clips were then presented to the examiners to determine the FTP. Kappa statistical analysis was used to evaluate the degree of intergrader agreement or disagr
