Case-area targeted interventions during a large-scale cholera epidemic: A prospective cohort study in Northeast Nigeria.

BACKGROUND: Cholera outbreaks are on the rise globally, with conflict-affected settings particularly at risk. Case-area targeted interventions (CATIs), a strategy whereby teams provide a package of interventions to case and neighboring households within a predefined "ring," are increasingly employed in cholera responses. However, evidence on their ability to attenuate incidence is limited. METHODS AND FINDINGS: We conducted a prospective observational cohort study in 3 conflict-affected states in Nigeria in 2021. Enumerators within rapid response teams observed CATI implementation during a ch

Characteristics of HIV Time-Space Alerts in the United States, 2018-2021.

BACKGROUND: HIV time-space cluster detection is routinely conducted by the Centers for Disease Control and Prevention and health departments to identify counties with elevated diagnoses compared with historical levels. These analyses, originally applied to all diagnoses and persons who inject drugs (PWID), generate "alerts" for review that may indicate clusters or outbreaks. The reoccurrence of alerts has not been previously described nationally. SETTING: Fifty US states and the District of Columbia. METHODS: Quarterly, during 2018-2021, the number of HIV infections diagnosed during the prec

Analysis of the epidemiological characteristics of pulmonary tuberculosis in Shijiazhuang, China 2010-2023.

BACKGROUND: Spatio-temporal analysis is a key epidemiological tool for monitoring disease transmission and identifying outbreak hotspots. However, the patterns of pulmonary tuberculosis (PTB) spread over time and space in Shijiazhuang remain poorly understood. This study aims to clarify the spatio-temporal dynamics of PTB transmission in this region. METHODS: We conducted a retrospective study using PTB surveillance data from 2010 to 2023, extracted from the national Tuberculosis Information Management System. Descriptive epidemiological analysis was conducted to assess the severity and distr

Positive psychological effects of space missions.

Being in space is a powerful experience that can have an enduring, positive impact on the psychological well-being of astronauts and cosmonauts. We sought to examine the frequency, intensity and distribution of such salutogenic experiences among persons who have flown in space, using a questionnaire we developed based on the scientific literature and first person accounts. All participants reported positive effects of being in space, but the degree of change varied widely, and some experiences were particularly common. Three of our five predicted attitude-behavior relationships were supported

Spatio-temporal analysis of smear-positive tuberculosis in the Sidama Zone, southern Ethiopia.

BACKGROUND: Tuberculosis (TB) is a disease of public health concern, with a varying distribution across settings depending on socio-economic status, HIV burden, availability and performance of the health system. Ethiopia is a country with a high burden of TB, with regional variations in TB case notification rates (CNRs). However, TB program reports are often compiled and reported at higher administrative units that do not show the burden at lower units, so there is limited information about the spatial distribution of the disease. We therefore aim to assess the spatial distribution and presenc

AIDS in men in the city of São Paulo, 1980-2012: spatial and space-time analysis.

OBJECTIVES: To identify spatial and space-time clusters with high incidence rates of AIDS in men living in the city of São Paulo since the first case of the disease in 1980. METHODS: HIV/AIDS notifications were obtained from the Notifiable Diseases Information System (57,440 men) between January 1980 and June 2012. The cases were geocoded by residence address; then analyses of purely spatial, space-time and spatial variation in temporal trends were performed for three sets of data: total cases of AIDS in men aged 13 years or older, men aged 50 years or older, and deaths from AIDS. RESULTS: I

A framework for investigating geographical variation in diseases, based on a study of Legionnaires' disease.

Demonstration of geographical variations in disease can yield powerful insight into the disease pathway, particularly for environmentally acquired conditions, but only if the many problems of data interpretation can be solved. This paper presents the framework, methods and principles guiding a study of the geographical epidemiology of Legionnaires' Disease in Scotland. A case-list was constructed and disease incidence rates were calculated by geographical area; these showed variation. Five categories of explanation for the variation were identified: short-term fluctuations of incidence in time

Geographical variations in the epidemiology of bacterial sexually transmitted infections in Manitoba, Canada.

Feasible epidemiological approaches are required to make a better assessment of the stage of an epidemic and to monitor its transition through various phases. Application of the Lorenz curve and Gini coefficient to summarise the inequality in STD incidence rates between jurisdictions in Manitoba, Canada, was found to provide useful insights into the concentration of these epidemics over time and thus their transition through epidemic phases. Further exploration of the statistical properties of these and other indices of inequality and their potential application to STD epidemiology is warrante

[Subacute sclerosing panencephalitis (SSPE) in Poland 1987-1989. Stage IV of the epidemiological studies].

In the years 1987-1989 111 patients of SSPE who react positively to laboratory tests were found in Poland. The incidence rate of the disease was in 1987-1.30; in 1980-0.98 and in 1989-0.66 per million inhabitants (in the years 1984-1986 the incidence of SSPE was respectively 1.22; 1.05 and 0.9 per million). The authors observed a shift in the incidence peak of SSPE from the age group 8-9 years in 1977-1983 and 10-12 years in 1984-1986 to the age group 13-14 years in 1987-1989. Among 111 SSPE cases only 6 children (5.4) had been vaccinated against measles while the coverage of measles vaccinati

The statistical analysis of multiple binary measurements.

Epidemiologic studies often compare several groups of subjects for the presence or absence of a specified biological trait, where each subject in a group contributes two or more observations to the analysis. Examples occur in ophthalmologic studies, where each subject contributes observations on each of two eyes, and dental studies, where observations on each of several teeth may be contributed. Application of the standard Pearson chi-square test to such data is not valid, since the resulting sample observations are not statistically independent. In this paper we show how simple adjustments ca

Classification of organic brain syndromes by cluster analysis.

Cluster analysis was carried out on a sample of 92 patients with behavior disorders caused by degenerative, vascular, (alcohol) toxic, and other diseases of the brain. Rating variables of the AMDP system concerning mental state, social behavior, need for special care, sleep disorders, autonomic, physical, and neurologic symptoms were used in the absence of severe degrees of disordered consciousness such as stupor, coma, delirium tremens, and gross cerebral lesions. Results suggested the existence of four major groups of global cognitive impairment combined with neurasthenia and irritability in

Neural evidence of switch processes during semantic and phonetic foraging in human memory.

Humans may retrieve words from memory by exploring and exploiting in "semantic space" similar to how nonhuman animals forage for resources in physical space. This has been studied using the verbal fluency test (VFT), in which participants generate words belonging to a semantic or phonetic category in a limited time. People produce bursts of related items during VFT, referred to as "clustering" and "switching." The strategic foraging model posits that cognitive search behavior is guided by a monitoring process which detects relevant declines in performance and then triggers the searcher to seek

Sentinel phenotype for rubella embryopathy: time-space distribution in Brazil.

The dyad comprising eye anomalies and congenital heart defects in the same newborn has been proposed as the best sentinel phenotype for the early detection of rubella embryopathy. Time-space birth prevalence distributions of the eye-heart dyad were described in 36 Brazilian hospitals from the Latin-American Collaborative Study of Congenital Anomalies - ECLAMC network, for the period 1994-2008. Seventy dyad cases observed among 554,531 births showed seasonal variation (Χ² = 5.84; p < 0.05), suggesting an environmental etiology, with an increase in cases in October-March and acrophase in Decembe

[Typhoid and paratyphoid fever in Yunnan province: distributional patterns and the related meteorological factors].

OBJECTIVE: To characterize the spatial distribution of typhoid and paratyphoid fever (TPF) in Yunnan province, China and to determine the effectiveness of meteorological factors on the epidemics of TPF. METHODS: Data of reported TPF cases in Yunnan province (2001 - 2007) from the China Information System for Diseases Control and Prevention was applied to GIS-based spatial analyses to detect their spatial distribution and clustering of TPF incidence at the county level. Panel data analysis was used to identify the relationships between the TPF incidence and meteorological factors including mon

Re-assessment of mitigation strategies for deliberate releases of anthrax using a real-time outbreak characterization tool.

Responding rapidly and appropriately to a covert anthrax release is an important public health challenge. A methodology to assist the geographical targeting of such a response has recently been published; as have a number of independent studies that investigate mitigation strategies. Here, we review and combine some of these published techniques to more realistically assess how key aspects of the public health response might impact on the outcomes of a bioterrorist attack. We combine a within-host mathematical model with our spatial back-calculation method to investigate the effects of a numbe
