Machine learning based analysis of speech dimensions in functional oropharyngeal dysphagia.

BACKGROUND AND OBJECTIVE: The normal swallowing process requires a complex coordination of anatomical structures driven by sensory and cranial nerves. Alterations in such coordination cause swallowing malfunctions, namely dysphagia. The dysphagia screening methods are quite subjective and experience dependent. Bearing in mind that the swallowing process and speech production share some anatomical structures and mechanisms of neurological control, this work aims to evaluate the suitability of automatic speech processing and machine learning techniques for screening of functional dysphagia. Word recognition and the articulation index in older listeners with probable age-related auditory neuropathy.

This retrospective analysis of existing data was derived from 957 members of a population-based cohort who participated in a prior study of the prevalence of central auditory dysfunction. Word recognition scores (WRS) at three intensity levels were compared to predicted scores based on the Articulation Index (AI) and the Thornton-Raffin 95 percent critical differences. In 112 (11.7 percent) participants, one or more word recognition scores were significantly below the predicted score, which we consider a subtle sign of possible auditory neuropathy. In contrast, classic signs of retrocochlear d

Categorical speech perception deficits distinguish language and reading impairments in children.

We examined categorical speech perception in school-age children with developmental dyslexia or Specific Language Impairment (SLI), compared to age-matched and younger controls. Stimuli consisted of synthetic speech tokens in which place of articulation varied from 'b' to 'd'. Children were tested on categorization, categorization in noise, and discrimination. Phonological awareness skills were also assessed to examine whether these correlated with speech perception measures. We observed similarly good baseline categorization rates across all groups; however, when noise was added, the SLI grou

Neural Adaptation at Stimulus Onset and Speed of Neural Processing as Critical Contributors to Speech Comprehension Independent of Hearing Threshold or Age.

Background: It is assumed that speech comprehension deficits in background noise are caused by age-related or acquired hearing loss. Methods: We examined young, middle-aged, and older individuals with and without hearing threshold loss using pure-tone (PT) audiometry, short-pulsed distortion-product otoacoustic emissions (pDPOAEs), auditory brainstem responses (ABRs), auditory steady-state responses (ASSRs), speech comprehension (OLSA), and syllable discrimination in quiet and noise. Results: A noticeable decline of hearing sensitivity in extended high-frequency regions and its influence on lo

Nonauditory Functions in Low-performing Adult Cochlear Implant Users.

INTRODUCTION: Despite substantial benefits of cochlear implantation (CI) there is a high variability in speech recognition, the reasons for which are not fully understood. Especially the group of low-performing CI users is under-researched. Because of limited perceptual quality, top-down mechanisms play an important role in decoding the speech signal transmitted by the CI. Thereby, differences in cognitive functioning and linguistic skills may explain speech outcome in these CI subjects. MATERIAL AND METHODS: Fifteen post-lingually deaf CI recipients with a maximum speech perception of 30% in

Audiometric results following endolymphatic sac surgery.

Decompression and drainage of the endolymphatic sac is a surgical procedure which was developed to relieve vertigo and hopefully to maintain or improve hearing in patients with Meniere's disease. In this study, 16 medically diagnosed Meniere's disease patients who underwent endolymphatic sac surgery were followed audiometrically for a period of up to three years. Pre- and postoperative audiometric and other symptomatic findings were reported. Results indicated that in the greatest majority of cases hearing remained essentially unchanged or was improved. Poorer hearing was found in 26.3% of the

The Persian version of auditory word discrimination test (P-AWDT) for children: Development, validity, and reliability.

OBJECTIVE: Auditory discrimination of speech sounds is an important perceptual ability and a precursor to the acquisition of language. Auditory information is at least partially necessary for the acquisition and organization of phonological rules. There are few standardized behavioral tests to evaluate phonemic distinctive features in children with or without speech and language disorders. The main objective of the present study was the development, validity, and reliability of the Persian version of auditory word discrimination test (P-AWDT) for 4-8-year-old children. METHODS: A total of 120

Spectral peak picking improves tactile speech perception.

Individual differences in speech perception often arise from disparities in access to acoustic-phonetic cues, particularly among those with hearing loss. Haptic hearing aids, which convey speech information through the sense of touch, offer a complementary pathway to improve speech understanding. However, effectively transmitting critical speech features through vibrotactile stimulation remains challenging. To address this challenge, we introduce a tactile spectral peak picking (tSPP) approach, integrated into a vocoder-based audio-to-tactile conversion algorithm to enhance vibrotactile phoneme

Preoperative evaluation, surgical procedure, follow up and results of 150 cochlear implantations.

BACKGROUND: The cochlear implantation is among the most important achievements of medicine and biotechnology in the last 20 years, because it allows individuals who had never heard or had lost their hearing to perceive sound and improve their quality of life. Selection criteria for candidates are strict and are evaluated in each individual by a scientific committee specially trained for implantations which includes Ear Nose and Throat (ENT) surgeon, audiologist, psychiatrist and speech therapist. PATIENTS AND METHODS: In our department, the first cochlear implantation was performed in 1995. D

[Presbyacusis].

Presbyacusis is senescence in the auditory system. Currently, the etiology of presbyacusis is thought to be mainly due to environmental noise and arterial sclerosis. Cumulative effects of social noise influence the auditory function gradually. Sclerotic change of arteries in the brain occurs along the total auditory pathway. Histopathological findings reveal characteristic changes in all parts of the auditory pathway in elderly persons. Alterations include thickening of the ear drum, sclerotic change of the ossicles joint, decreasing number of hair cells, degeneration of the eighth nerve and o

Listener-assessed intelligibility of a hearing aid self-adaptive noise filter.

A principal complaint of hearing aid users is the inability to understand speech in the presence of competing background noise. Efforts at reducing the adverse effects of external noise on amplification systems have centered on manually adjusted or low frequency activated high-pass filtering; however, there has been recent interest in the potential of self-adaptive noise reduction systems. In this study we investigate the performance of normals and three groups of sensorineural loss subjects under five noise conditions with a self-adaptive noise filter incorporated in a conventional hearing ai

Speech discrimination in patients with Bell's palsy and a paralysed stapedius muscle.

The contraction of the stapedius muscle results in a frequency dependant attenuation of sound through the middle ear. Idiopathic facial nerve paralysis (Bell's palsy) usually results in a paralysis of the stapedius muscle. This prospective study included 119 patients presenting with Bell's palsy over an 18-month period. After applying exclusion criteria, 80 patients with normal pure-tone audiograms underwent speech audiometry. This was performed on presentation and after recovery of the facial nerve palsy. Fifty-six patients (70%) with an absent stapedius reflex showed marked 'roll-over' from

Re-reconsidering the role of temporal order in spoken word recognition.

Spoken word recognition models incorporate the temporal unfolding of word information by assuming that positional match constrains lexical activation. Recent findings challenge the linearity constraint. In the visual world paradigm, Toscano, Anderson, and McMurray observed that listeners preferentially viewed a picture of a target word's anadrome competitor (e.g., competitor bus for target sub) compared with phonologically unrelated distractors (e.g., well) or competitors sharing an overlapping vowel (e.g., sun). Toscano et al. concluded that spoken word recognition relies on coarse grain spec

The benefits of hearing aids and closed captioning for television viewing by older adults with hearing loss.

OBJECTIVE: Although watching television is a common leisure activity of older adults, the ability to understand televised speech may be compromised by age-related hearing loss. Two potential assistive devices for improving television viewing are hearing aids (HAs) and closed captioning (CC), but their use and benefit by older adults with hearing loss are unknown. The primary purpose of this initial investigation was to determine if older hearing-impaired adults show improvements in understanding televised speech with the use of these two assistive devices (HAs and CC) compared with conditions

Contribution of consonant versus vowel information to sentence intelligibility for young normal-hearing and elderly hearing-impaired listeners.

The purpose of this study was to examine the contribution of information provided by vowels versus consonants to sentence intelligibility in young normal-hearing (YNH) and typical elderly hearing-impaired (EHI) listeners. Sentences were presented in three conditions, unaltered or with either the vowels or the consonants replaced with speech shaped noise. Sentences from male and female talkers in the TIMIT database were selected. Baseline performance was established at a 70 dB SPL level using YNH listeners. Subsequently EHI and YNH participants listened at 95 dB SPL. Participants listened to ea

Hearing complaints of patients with King-Kopetzky syndrome (obscure auditory dysfunction).

The hearing complaints in patients with King-Kopetzky syndrome were studied by use of the open-ended hearing problem questionnaire. The main findings are that complaints were commonly focused on the categories of 'live speech' and 'electronic speech' difficulties in patients with King-Kopetzky syndrome, particularly the auditory difficulties of speech in noise and group conversation. However, they reported problems with warning signals significantly less frequently than a control 'audiological rehabilitation' group and the patients studied by Barcham and Stephens (1980). In addition, main psyc
