Reimagining stuttering therapy and outcomes through an acceptance and collaborative lens.

The purpose of this paper is to summarize a panel discussion at the 2024 World Stuttering and Cluttering Organization Congress that aimed to explore and reflect on the shifting landscape of stuttering therapy across the lifespan. The panel comprised of speech-language pathologists who have experience in stuttering therapy as clients, professionals, and researchers. The panel members reflect on the research, their professional experiences, and their own personal journey toward a more acceptance-based therapy and its outcomes. The paper concludes with the panel's reflections on the future of stu

What do speech-language pathologists do to support families' active involvement in early intervention? Exploring moment-to-moment interactions in sessions.

PURPOSE: There is growing emphasis on the importance of involving families in all aspects of the early speech-language pathology intervention process, however, both speech-language pathologists (SLPs) and families have reported that this can be challenging. To better understand how SLPs work together with families in intervention, this study aimed to (a) explore what SLPs do to support families to be actively involved in intervention, both inside and outside sessions, and (b) explore the perspectives of families regarding how SLPs supported their active involvement. METHOD: Video-reflexive et

Moving toward a systemic training model in a university program.

While the medical model continues to provide a basis for clinical service and the preparation of future speech-language pathologists, the instructor who teaches and uses systemic principles enriches the world view of students and prepares them for new roles being assumed by speech-language pathologists. Experiences of the author are described as she became aware of systemic principles and began teaching them to undergraduate and graduate students. Two formal strategies are discussed. These are linked to a Triadic Interaction Model and a Family-Based Treatment model. Students commented favorabl

Speech pathology prescribing in the outpatient setting: A review of requirements, considerations and barriers.

BACKGROUND: As health systems face increasing demands, non-medical prescribing is a workforce redesign strategy adopted within some services. Despite successful implementation in other professional groups, non-medical prescribing within speech pathology (SP) has not yet been described. AIMS: To provide a descriptive account of the development and planned implementation of two SP prescribing models. METHODS & PROCEDURES: The evolution of two SP-led prescribing models, including relevant training and credentialing, for use of (1) nystatin oral drops (100,000 units/mL); and (2) lidocaine (ligno

Learner training for phonetic transcription of typical and/or disordered speech: A scoping review.

BACKGROUND: Phonetic transcription is a core skill of speech and language therapists/pathologists (SLT/Ps) for clinical assessment of speech sounds and/or errors, and linguists for investigation of phonetic phenomena in various languages; hence, it is included in the curriculum of the corresponding degree programme. Many experts and course instructors have discussed their opinions on different aspects of phonetic transcription teaching and/or reported their own training programmes. However, no review has systematically summarized the types of expert recommendations and training methodology rep

An evaluation of the Aurora system as a flesh-point tracking tool for speech production research.

PURPOSE: Northern Digital Instruments (NDI; Waterloo, Ontario, Canada) manufactures a commercially available magnetometer device called Aurora that features real-time display of sensor position tracked in 3 dimensions. To test its potential for speech production research, data were collected to assess the measurement accuracy and reliability of the system. METHOD: First, sensors affixed at a known distance on a rigid ruler were moved systematically through the measurement space. Second, sensors attached to the speech articulators of a human participant were tracked during various speech tasks

The implications of RTI and EBP for SLPs: commentary on L. M. Justice.

PURPOSE: This commentary responds to Justice's article on response to intervention (RTI) and evidence-based practice (EBP) for reading instruction. The educational changes brought about by RTI and EBP provide an opportunity as well as a challenge for speech-language pathologists (SLPs) to make fundamental changes in service delivery. METHOD: In this article, I discuss how RTI will change who qualifies as reading disabled and who receives special reading instruction. I examine how RTI might change who qualifies and how they qualify for speech-language services. Finally, I consider what can be

Development of speech and language following bilateral frontal lesions.

Language and speech were studied in a young child with perinatally acquired bifrontal lesions. Bilateral frontal pathology seriously interfered with the development of intelligible speech and resulted in a persistent expressive aphasia. Analysis of the neuropsychological profile indicated impairments in intelligence and language comprehension. These deficits, however, were considered secondary to the profound speech programming disorder. The findings indicate that, despite the plasticity of the immature central nervous system, bilateral frontal injury sustained at an early age precludes the de

On the unity of children's phonological error patterns: distinguishing symptoms from the problem.

This article compares the claims of rule- and constraint-based accounts of three seemingly distinct error patterns, namely, Deaffrication, Consonant Harmony and Assibilation, in the sound system of a child with a phonological delay. It is argued that these error patterns are not separate problems, but rather are symptoms of a larger conspiracy to avoid word-initial coronal stops. The clinical implications of these findings are also considered.

A case of verbal auditory agnosia: missing the word ... missing the sound...

The neuropsychological nature of verbal auditory agnosia is not fully understood. This study aims to describe the language deficits and the remediation strategies used in a person with verbal auditory agnosia. In addition, it will address the theoretical issues concerning the nature of the phenomenon and the clinical implications in the management of people with this disorder.

Conceptualising "dose" in paediatric language interventions: Current findings and future directions.

Dose is a particularly important element of treatment for speech and language disorders, yet relatively little is known concerning how best to operationalise dose in clinical settings. This keynote provides an overview of dose as a theoretical and empirical concept and discusses recent findings that question whether "more is better" when treating children with language disorders. Given recent evidence showing that spaced treatments may result in optimal gains for children, I argue that low-frequency therapy sessions can be especially beneficial for children with language disorders when they ar

Educational provision for children with specific speech and language difficulties: perspectives of speech and language therapy service managers.

BACKGROUND: Children with specific speech and language difficulties (SSLD) pose a challenge to the education system, and to speech and language therapists who support them, as a result of their language needs and associated educational and social-behavioural difficulties. The development of inclusion raises questions regarding appropriate provision, whether the tradition of language units or full inclusion into mainstream schools. AIMS: To gather the views of speech and language therapy service managers in England and Wales regarding approaches to service delivery, terminology and decision-ma

How is dysphagia conceptualised, assessed, and managed with populations who are culturally diverse: A scoping review.

INTRODUCTION: Research demonstrates health disparities for culturally diverse populations who are minority groups within dominant cultural populations. Speech-language pathology practices for dysphagia are often derived from evidence drawn from Western contexts, but there is a need for culturally responsive practices to ensure optimal outcomes. In this scoping review we aimed to explore research on swallowing assessment and management for individuals with dysphagia who are culturally diverse, including how this is understood, the nature of practice, and what gaps exist. METHOD: This scoping r
