Sphenoid sinus mucoceles are exceedingly rare. Most have occurred in men between the ages of 30 and 40 years. We report a case of sphenoid sinus mucocele that was unusual not only for the age of the patient (13 yr), but for the massive size of the lesion (59 x 53 mm). The patient presented with a 20-day history of unilateral proptosis, decreased vision, diplopia, and photophobia. He also complained of severe bilateral nasal obstruction, nasal stuffiness, and a mucoid nasal discharge. Endoscopic decompression of the left sphenoid sinus was performed, and approximately 200 ml of thick, sterile m…

A 34-year-old woman was hospitalized for exploration of amenorrhea and galactorrhea. She had experienced primary sterility, diagnosed 11 years earlier. Endocrine tests demonstrated hyperprolactinemia (serum prolactin 594 ng/ml). Magnetic resonance imaging (MRI) showed a soft tissue mass located within the posterior part of the sphenoidal sinus. The pituitary gland appeared normal. Medical treatment with bromocriptine was unsuccessful. Transsphenoidal tumor resection was performed. Histology reported adenoma, which was consistent with prolactin-producing tumor demonstrated by immunohistochemica

BACKGROUND: Surgical treatment of CSF leakage in the sphenoid sinus is difficult for several reasons: 1. the close neighbourhood between the sphenoid sinus and the internal carotid artery, the cavernous sinus and cranial nerves (II, III, IV, VI), 2. a strong liquorrhoea caused by basal cisterns surrounding the sphenoid sinus, 3. it can be difficult to visualise completely the sphenoid sinus depending on the extent of pneumatisation.
MATERIALS AND METHODS: In this retrospective study 20 cases of duraplasty around the sphenoid sinus were evaluated. The approach, materials, operative techniques …

Skull base and intracranial cystic schwannomas are rare tumors. To our knowledge, a cystic schwannoma involving the sphenoid sinus has not been previously reported. The location and fluid-filled nature of the lesion resulted in an erroneous preliminary diagnosis of a destructive sphenoid sinus mucocele. Endoscopic biopsy and decompression established the correct diagnosis and reversed a progressive visual loss until definitive surgical removal of the lesion was accomplished. Diagnostic evaluation, histologic findings and management strategies are discussed.

In the period from 1980 through 1989, we treated 25 (20 postoperative and five primary) cases of mucoceles of the posterior ethmoidal sinus and/or sphenoidal sinus accompanied by visual disturbances. The postoperative mucocele developed 15 to 25 years after initial surgery. Manifestations of mucoceles include headache, ophthalmalgia, double vision, and exophthalmos in addition to a decrease in visual acuity. The degree of improvement in visual acuity after an operation depends on visual acuity before the operation, the mode of development of the mucocele, and the time from onset of the disease

In summary, an interesting case of an accessory parasphenoid sinus mucocele in a 10-year-old girl is reported. By choosing an endoscopic approach, hearing and balance were preserved, and an external sinus or neurosurgical procedure avoided. The surgery was done on an outpatient basis, with minimal postoperative discomfort and no complications. The mucocele was adequately drained and there has been no recurrence over the following 12 months.

Sphenoidal aspergillosis is an unusual cause of sella turcica enlargement. Pituitary abscess secondary to Aspergillus had been reported. In the present case, a woman with sphenoid sinus aspergillosis mimicked a pituitary tumor. This patient survived her infection with intact pituitary function following a transsphenoidal approach. No postoperative amphotericine-B and 5-fluorocytosine were necessary. CT scan revealed a mass occupying the sphenoid sinus extending to the sella turcica. Factors that should alert the clinican to the presence of a sphenoidal and pituitary abscess in a patient with s…

Although osteoma of the paranasal sinus is comparatively common, involvement of the sphenoidal sinus region is extremely rare. Lame reported that a total of twelve cases had been described in the European literature since 1800. Mikaelin, Kulczynski and Dolan, since then, described three cases. This paper deals with two surgical cases of sphenoidal sinus osteoma, reviewing literature on paranasal sinus osteoma and differential diagnosis in the sphenoidal sinus lesion. Case 1. A 20-year-old man was referred to us by an ophthalmologist, complaining of left visual disturbance (visual acuity: 0.02)

Paragangliomas, also known as glomus tumors, are unusual, generally benign tumors most commonly found in association with major vessels and cranial nerves. Only a small number of these tumors have been described arising in the paranasal sinuses, and none of these has been physiologically active. The following case report describes a metabolically active paraganglioma arising in the sphenoid sinus.

A 60 year old woman presented with headaches and a destructive lesion in the sella turcica. She refused treatment but returned seven years later with a spontaneous pneumocephalus and dementia. A mucocele of the sphenoid sinus was discovered. Her symptoms resolved after removal of the mucocele and obliteration of the sphenoid sinus.
