[Eosinophilic granuloma of the spine].

The contribution to the clinical picture of eosinophilic granuloma of the spine. This tumor-like, osteolytic bone lesion presents both diagnostic and therapeutic problems. Solitary eosinophilic granuloma of the spine is not common. Compared to the multiple manifestation of eosinophilic granuloma, which seldom spares the spine, vertebral involvement is rare in cases of solitary eosinophilic granuloma (about 10 p.c.). Spinal involvement of this disease is mainly characterized by an undramatic, uneventful clinical course, even in cases of extensive osteolytic bone defects, varying laboratory find…

The innervation of the lumbar spine.

The lumbar intervertebral discs are innervated posteriorly by the sinuvertebral nerves, but laterally by branches of the ventral rami and grey rami communicantes. The posterior longitudinal ligament is innervated by the sinuvertebral nerves and the anterior longitudinal ligament by branches of the grey rami. Lateral and intermediate branches of the lumbar dorsal rami supply the iliocostalis lumborum and longissimus thoracis, respectively. Medial branches supply the multifidus, intertransversarii mediales, interspinales, interspinous ligament, and the lumbar zygapophysial joints. The distributi

Tuberculosis of the spine: a review.

Spinal tuberculosis remains common in the "Third World" and is commoner in Britain and "developed" countries than many other disorders that receive much more attention. Though little new has been added to knowledge of its morbid anatomy, the observation that it is associated with a low bacterial population is emphasised. Controlled clinical trials in Korea, Hong Kong and Bulawayo have demonstrated the excellent results of outpatient ambulant chemotherapy, which are unimproved by other additional measures with the one exception of the skilled performance of radical resection of the focus with a

The spine in osteogenesis imperfecta.

A high incidence of spinal curvature, approaching 80 per cent in patients more than twelve years old, was found in a large series of patients with osteogenesis imperfecta. Curves showed early progression and a predictable pattern. In the age range between one and five years the incidence of scoliosis was 26 per cent; thereafter, it rose precipitously. The degree of curvature was greater in the older children. The Milwaukee brace was ineffective in the treatment of the scoliosis and the complications of its use were detrimental. In several patients it deformed the rib cage. It had little or no

Hydatid disease of the spine.

In 42 cases of hydatid diseases of the spine the preoperative diagnosis was as follows: hydatid disease--19, benign tumour--8, Tuberculosis--6, malignant disease--4, herniated disk--3, syringomyelia--1, infectious spondylitis--1. The site of the hydatid lesion in these patients was as follows: cervical--5, thoracic (T6-T12)--26, lumbosacral--11. Seven of these cases had previously been operated on for a hydatid cyst in other organs. We had eight recurrences of which five did not respond completely to surgical intervention.

Kyphosis in the paralytic spine.

The paralytic spine without supplementary support is unstable and susceptible to axial collapse. The biomechanical factors responsible for spinal stability should be considered in any treatment regimen for paralytic spines. If posterior fusion is elected as the treatment, kyphosis must be corrected so that the eccentric loading and kyphotic bending moment is very small. When this is done, posterior fusion alone is adequate to give spinal stability and increased function to the individual.

Surgical stabilisation of the spine.

It is important to assess spinal injuries, both in terms of cord damage and column damage, shortly after the trauma has occurred. A decision should then be made concerning the eventual stability of the bony vertebral column. If a lesion is unlikely to heal soundly by bone or if one is unable to obtain a reduction by conservative measures, one should consider surgical stabilisation of the damaged spine in the subacute phase. At the same time as one stabilISES THe spine with metallic fixation, bone grafts should be added for ultimate stability. If an unstable spine is encountered in the later st…

[The spine and horseback riding].

115 horseback riders have undergone clinical and radiologic examinations of the spine. A minimum of 10 years' riding was required. Depending on intensity of training and number of riding hours, the subjects were divided into three groups: roughriders, sport riders and hobby riders. It was striking that of the bare half of cases (51) suffering from lower back, only 8 (about 15%) had had medical treatment and actually been disabled. Moreover, 47% of the riders with lower back pain were painfree in the saddle, i.e. when riding. No causal connection was found between riding and a higher incidence …
