Spontaneous atraumatic splenic rupture secondary to infectious mononucleosis.

PRESENTATION: A healthy 23-year-old man developed sore throat and fever with positive EBV Infectious Mononucleosis test, followed by abdomin … tics and vaccinations. He was well at three-month follow-up. DISCUSSION: EBV-associated atraumatic splenic rupture is rare yet life-threatening. CT imaging confirms injury and bleeding. Management depends on physiology; unstable patients require prompt surgical management, whereas stable cases may merit spleen-preserving strategies and close monitoring.

Systemic Lupus Erythematosus Complicated by Spontaneous Splenic Rupture.

Spontaneous splenic rupture (SSR) is an uncommon but life-threatening surgical emergency. When it occurs as a complication of systemic lupus erythematosus (SLE), the diagnostic and therapeutic strategy becomes particularly complex. Published literature on SSR in the setting of SLE is sparse, and there is no clear consensus on the optimal management of such a surgical catastrophe. What is the optimal treatment strategy for patients with SSR complicated by SLE? This paper addresses these questions by providing an in-depth analysis of a representative case of SLE complicated by SSR that we succes…

Spontaneous splenic rupture, an unusual presentation of tuberculosis.

INTRODUCTION: Spontaneous splenic rupture from tuberculosis (TB) is a very unusual presentation within the wide range of presentations of this infectious disease. CLINICAL CASE: A 40-year-old male with a diagnosis of human immunodeficiency virus, begins with fever and pain in the left hypochondrium. A computed tomography scan was performed, showing probable splenic abscesses; suddenly, it begins with hemodynamic deterioration, exacerbation of pain, a surgical exploration was performed, showing spontaneous splenic rupture. Microscopic study of the spleen shows the presence of Mycobacterium tub…

Spontaneous Splenic Artery Pseudoaneurysm Rupture Causing Hemorrhagic Shock.

Splenic artery pseudoaneurysm (SAP) is an uncommon etiology of acute abdominal pain, requiring a high degree of clinical suspicion to diagnose in a timely manner. There are currently no reports of spontaneous SAP ruptures in the emergency medicine literature. We report a case of a man who presented with acute abdominal pain secondary to an SAP. A computed tomography angiography scan of the abdomen revealed a ruptured SAP with hemoperitoneum. He successfully underwent emergency laparotomy and surgical ligation of his SAP with splenectomy. SAP rupture remains an under-recognized etiology of abdo

Spontaneous Splenic Rupture in an Ill Returned Traveller.

UNLABELLED: Spontaneous splenic rupture is a known, but rare and possibly fatal, complication of different infectious diseases. We present a case of a 38-year-old male patient who presented with fever, icterus and spontaneous splenic rupture after a visit to Vietnam and discuss the differential diagnosis of splenic rupture in ill returned travellers. LEARNING POINTS: Spontaneous splenic rupture is a rare complication of several tropical diseases, including Salmonella infection.Atypical presentations of Salmonella infection should always be considered in febrile ill returned travellers.

Atraumatic splenic rupture due to covid-19 infection.

BACKGROUND: Covid-19 is a novel disease caused by the severe acute respiratory coronavirus (SARS-CoV2). We discuss a gentleman who presented with an atraumatic rupture of the spleen secondary to this infection. BRIEF SUMMARY OF PRESENTATION: A 57-year-old service engineer was brought into the emergency department after having collapsed at home. RT-PCR was positive for covid-19 infection. CT scan showed evidence of haemoperitoneum and splenic rupture. He underwent splenic artery embolisation and required ventilatory and circulatory support on ITU. He made a full recovery and was discharged hom…

Delayed Splenic Rupture: A Rare Complication of Snakebites.

Bites by venomous snakes can cause fatal systemic and coagulation disorders. Rare complications, such as compartment syndrome and hemoperitoneum, may also require surgical intervention. Here, we describe our experience with an unusual case of snakebite-induced delayed splenic rupture. A 54-y-old male with no specific medical history visited a local hospital for a bite by an unidentified snake. He had been bitten on the left thumb and was administered antivenom. He was discharged from the local hospital after 3 d when his symptoms had improved. However, he revisited our emergency medical center…

Spontaneous rupture of a non-parasitic splenic cyst.

Rupture of a non-parasitic splenic cyst is a rare but possibly dangerous complication with 21 cases described so far. We present a 46-year-old woman who presented with acute abdominal pain and was diagnosed with a spontaneous ruptured splenic cyst that was successfully treated by laparoscopic splenectomy. Histological examination showed characteristics corresponding with a non-parasitic congenital cyst that had lost its epithelial lining. Several treatment options can be considered for splenic cysts, depending on size and location. In case of rupture, the clinical condition of the patient shou

Atraumatic splenic rupture in a peritoneal dialysis patient.

Splenic hemorrhage is a potentially life-threatening complication usually occurring after blunt trauma to the abdomen. Atraumatic splenic rupture (ASR) is an uncommon condition, and mostly results from pathology affecting the spleen, such as tumor infiltration or infection. Here, we report a case of atraumatic rupture of a normal spleen in a patient undergoing peritoneal dialysis, and review similar cases in the literature. The case involved a 58-year-old man with nephrotic syndrome who had been undergoing peritoneal dialysis for 1 year. He presented to the hospital with abdominal pain, nausea…

Splenic Hematoma Mimicking Rupture Uterus: A Diagnostic Dilemma.

Spontaneous splenic hematoma occurring during pregnancy is a rare entity. It often occurs in preexisting pathology of the spleen such as thalassemia or infectious etiologies such as malaria, typhoid, dengue, or infectious mononucleosis but most commonly after a trauma. The occurrence of splenic hematoma during pregnancy without any underlying cause is rare. Here, we present such a case and the diagnostic dilemma associated with it.

[Mantle cell lymphoma presenting with spontaneous splenic rupture].

A 48-year-old man was transferred to our emergency room because of sudden-onset epigastric pain and nausea. Abdominal contrast-enhanced computed tomography (CT) showed splenomegaly with splenic infarction and intra-abdominal bleeding, suggestive of splenic rupture. An emergent open splenectomy was performed. His spleen was markedly swollen and showed continuous bleeding due to a laceration. On histopathological examination, his spleen was filled with abnormal tumor cells. He was diagnosed as having mantle cell lymphoma based on the findings of immunohistochemical and cytogenetic analyses of th…

[Secondary Splenic Rupture after Initially Inconspicuous CAT Scan].

BACKGROUND: Splenic injuries occur in 1-5 % of blunt abdominal trauma cases. After initial haemorrhagic compression, secondary delayed spleen rupture can occur with a latency of one day to a month or longer. Mortality is then up to 15 %. The spleen injury is almost always recognisable on CT or ultrasound. CASE HISTORY: In one case from our clinic, secondary splenic rupture occurred in a patient after discharge from hospitalisation, even though the initial CT and ultrasound were unremarkable. The patient survived, and underwent emergent splenectomy 8 days after the trauma. An expert review of …

Spontaneous Splenic Rupture in Vascular Ehlers-Danlos Syndrome.

Vascular Ehlers-Danlos Syndrome (VEDS) is a rare autosomal dominant collagen vascular disorder. Different from other Ehler-Danlos Syndrome subtypes, VEDS has poor prognosis due to severe fragility of connective tissues and association with life-threatening vascular and gastrointestinal complications. Spontaneous splenic rupture is a rare but hazardous complication related to this syndrome. To date, only 2 cases have been reported in the literature. Here we present another case of this uncommon complication, occurring in a 54-year-old woman in clinical follow-up for VEDS who presented with sudd…
