Factors associated with alcohol-impaired driver crash deaths in the United States, 2018-2022.

INTRODUCTION: In the United States, the number of passenger vehicle drivers killed in crashes with blood alcohol concentrations (BACs) at or above 0.08% increased from 4,791 in 2019 to 5,540 in 2020 and remained elevated at 6,042 in 2022. This paper examines changes in alcohol policies, mental health factors, and law enforcement employment during 2018-2022 and their associations with alcohol-impaired driver deaths. METHOD: Panel regressions compared high-BAC (≥0.08%) driver deaths across states and months for all ages and ages 16-20. Predictors included state-level alcohol policy indicators (

State Strategies for Establishing Connections to Health Care for Justice-Involved Populations: The Central Role of Medicaid.

ISSUE: With many states expanding Medicaid eligibility, individuals leaving jail or prison are now often able to enroll in health coverage upon release. It is increasingly clear, however, that coverage alone is insufficient to address the often complex health and social needs of people who cycle between costly hospital and jail stays. GOALS: To identify emerging trends in the care delivery models that state Medicaid programs use for former inmates. METHODS: Literature review and interviews with state officials, plans, and providers. KEY FINDINGS: The care delivery models for individuals lea

Out-of-Pocket Costs for Advanced Imaging Across the US Private Insurance Marketplace.

PURPOSE: The aim of this study was to characterize out-of-pocket patient costs for advanced imaging across the US private insurance marketplace. METHODS: Using the 2017 CMS Health Insurance Marketplace Benefits and Cost Sharing Public Use File, which details coverage policies for qualified health plans on federally facilitated marketplaces, measures of out-of-pocket costs for advanced imaging and other essential health benefits were analyzed for all 18,429 plans. RESULTS: Independent of deductibles, 48.0% of plans required coinsurance (percentage fees) for advanced imaging, 9.7% required cop

Implementation of legislative requirements for emergency medical services in prepaid group practice organizations.

The Health Maintenance Organization Act of 1973, the Emergency Medical Services (EMS) Systems Act of 1973, and other laws are examined for their effects on the organization and management of emergency services in prepaid group practice plans (PPGP). The study was conducted in 1974-75 by the Group Health Association of America. The data were gathered through interviews with administrators and providers of seven PPGPs and with leaders of health planning agencies in the same communities, as well as through reviews of internal documents and a 1-month utilization survey of emergency and urgent care

Increased Public Accountability for Hospital Nonprofit Status: Potential Impacts on Residency Positions.

BACKGROUND: The Institute of Medicine recently called for greater graduate medical education (GME) accountability for meeting the workforce needs of the nation. The Affordable Care Act expanded community health needs assessment (CHNA) requirements for nonprofit and tax-exempt hospitals to include community assessment, intervention, and evaluation every 3 years but did not specify details about workforce. Texas receives relatively little federal GME funding but has used Medicaid waivers to support GME expansion. The objective of this article was to examine Texas CHNAs and regional health partne

Comparative analysis of contraceptive use in Punjab and Manipur: exploring beyond women's education and empowerment.

BACKGROUND: Women's education and empowerment are important predictors of contraceptive use across countries. However, two of the Indian states, namely, Punjab and Manipur, showed large variations in contraceptive use, despite the similar level of women's educational attainment and empowerment. Therefore, this paper attempts to understand variation in contraceptive use between these states, despite having similar level of educational attainment and empowerment among the married women. METHODS: This study primarily used cross-sectional data of the National Family Health Survey (NFHS) 2015-16 a

Dreams and nightmares: practical and ethical issues for patients and physicians using personal health records.

Electronic health records for patients, personal health records (PHRs), have become increasingly popular among policy makers and purchasers, but uptake among patients and physicians has been relatively slow. PHRs have varying uses that might make them more or less appealing to different stakeholders. The three core uses for PHRs - promoting communication, data use, and patient responsibility - each raises a set of potential practical and financial dilemmas. But some ethical concerns are also at play, some of which are rarely recognized as values-based barriers to the use of PHRs. Recognizing t

Reproductive health care delivery. Patterns in a changing market.

Providers of reproductive health services, including clinics and office-based physicians, face new challenges as the American health system progresses toward managed care. Although services for low-income women are often subsidized, the average out-of-pocket payments for reproductive health services are the same for women with incomes below and above 200% of the poverty level. Although many women, especially those classified as low income, use clinics, most say that they would prefer to receive care in a private physician's office and in a place where they can get general health care as well.

Problems of equity in US HIV integrated planning, 2015-2021: Enacting a bounded justice continuum.

The US HIV/AIDS response is notably worse than those of other high-income countries. The country's epidemic is marked by low viral suppression rates, high incidence, lacking coordination, and entrenched disparities along lines of sexuality, race/ethnicity, gender, class, and other factors. In 2010, the National HIV/AIDS Strategy for the United States (NHAS) was launched, centering an equity-oriented vision that prioritized marginalized groups. NHAS implementation required states to create HIV integrated plans to better coordinate services and meet populations' needs. We used Carol Bacchi's "Wh

The Added Burden of Personality Disorder on Subsidized Australian Health Service Utilization Among Women With Mental State Disorder.

This study aimed to investigate health service utilization among women with mental state disorder only (MSD-PD), mental state disorder plus personality disorder (MSD+PD), and controls in a population-based sample. Women (n = 635) from the Geelong Osteoporosis Study completed mental health assessments and were categorized into groups (MSD-PD, MSD+PD, controls). General practitioner (mental and non-mental health encounters) and specialized mental health service utilization was ascertained from data linkage to the Medicare Benefits Schedule, Australia (01/09/2008-31/12/2012). Negative binomial an

Evaluating public health responses to reintroduced smallpox via dynamic, socially structured, and spatially distributed metapopulation models.

The risk of smallpox reintroduction has motivated preparations in potential target countries. After reproducing the spatiotemporal pattern after the 1972 importation into Yugoslavia via coupled, biologically realistic systems of ordinary differential equations, we developed dynamic population models with current US age distributions and typical spatially distributed social structures. Surveillance and containment (S&C) coupled with vaccination of 95% of hospital-based health care workers (HCWs) within 2 days after the first diagnosis (estimated to be 18 days after aerosol release) were modeled

Creative Contributory Contests to Spur Innovation in Sexual Health: 2 Cases and a Guide for Implementation.

Sexual health campaigns are often designed "top-down" by public health experts, failing to engage key populations. Using the power of crowdsourcing to shape a "bottom-up" approach, this note describes 2 creative contributory contests to enhance sexual health campaigns. We provide guidance for designing creative contributory contests to improve HIV and other sexually transmitted disease testing.
