A 19-year-old male presented with a three-day history of worsening epigastric pain, persistent vomiting, abdominal distension, and obstipation. His medical history included high-fall trauma in 2022, resulting in a parietal bone fracture, scalp swelling, and an epidural hematoma. Physical examination revealed epigastric tenderness, abdominal distension, and diminished bowel sounds. Imaging revealed a 7 cm left diaphragmatic defect with herniation of the stomach, colon, and mesentery into the thoracic cavity, confirming a diagnosis of gastric volvulus with a diaphragmatic hernia. An exploratory

UNLABELLED: Congenital diaphragmatic hernia is usually diagnosed prenatally and surgically repaired within the first few days of life. It is very rarely associated with acute gastric volvulus, an uncommon surgical emergency in children. A male infant with atypical presentation of acute gastric volvulus was diagnosed and treated by a swift-acting team across two medical centers. The patient presented with generalized abdominal distention and non-productive retching. Upper gastrointestinal series diagnosed acute gastric volvulus. During laparoscopic intervention, the volvulized stomach and the s

Boerhaave's syndrome is oesophageal rupture as a result of forceful vomiting or retching against a closed cricopharyngeus. We report the case of an 81-year-old man with oesophageal rupture secondary to volvulus of an intrathoracic stomach who underwent laparotomy with a repair of the oesophageal perforation and intrathoracic stomach, making a good recovery. This case demonstrates another life-threatening complication of an intrathoracic stomach and highlights the importance of an early diagnosis and the advantages of using a transabdominal approach in this situation.

A previously healthy 6-year-old girl suddenly developed severe abdominal pain and nausea. She was diagnosed with acute gastric volvulus, and a nasogastric tube was inserted to decompress the stomach. The volvulus did not reduce spontaneously; therefore, we performed endoscopic reduction on day 3 and were able to treat her successfully. We reviewed the Japanese literature on endoscopic reduction for gastric volvulus in children. Fifteen cases have been reported since 1994. There are no reports of perforation during the procedure. Patients whose general condition is stable and who have no severe

Gastric remnant volvulus following Roux-en-Y gastric bypass (RYGB) surgery is rare, with only two previously reported cases. Herein, we present the first case of gastric remnant volvulus following gastric sleeve conversion to RYGB in a 32-year-old female. Management for gastric remnant volvulus has not been clearly described in the literature due to the rarity of cases; however, previously documented cases of gastric remnant volvulus following RYGB were managed with gastropexy or resection of the gastric remnant. Due to anatomical limitations, gastropexy was not an option for our patient, and

INTRODUCTION: Gastric volvulus is torsion of the stomach and requires immediate treatment. The optimal treatment strategy for patients with gastric volvulus is not established, because of significant variations in the cause and clinical course of this condition.
PRESENTATION OF CASES: We describe our experience with six elderly patients with gastric volvulus caused by different conditions using various approaches. This includes two patients managed with endoscopic reduction, followed by endoscopic or laparoscopic gastropexy.
DISCUSSION: Endoscopy is a necessary first step to determine the op

BACKGROUND: Acute gastric volvulus associated with congenital diaphragmatic hernia is an uncommon disorder in infancy and a surgical emergency.
METHODS: We present a six-month female baby who presented with clinical features of intestinal obstruction. Ultrasonography of the abdomen revealed gastric volvulus. The baby underwent emergency laparotomy. Gastric volvulus with splenic herniation was encountered through a diaphragmatic hernia.
RESULTS: The defect was corrected, the stomach and spleen were mobilized into the normal anatomical position. The baby recovered well.
CONCLUSION: A high ind

INTRODUCTION: Gastric volvulus (GV) is defined as a rotation of the stomach along its short or long axis leading to variable degrees of gastric outlet obstruction. Rotation of the stomach >180° may cause closed loop obstruction and possible strangulation, which often causes acute abdominal pain. Strangulation and gangrene of the twisted stomach sometimes occurs, which demands immediate surgical intervention. We report a case of acute gastric volvulus due to a gastrointestinal stromal tumor (GIST), with multiple recurrences, that eventually required emergency gastrectomy.

Gastric volvulus is an uncommon condition in which a torsion of the stomach results in acute or chronic gastrointestinal symptoms. We report a case of an intrathoracic organo-axial gastric volvulus secondary to a hiatal hernia. The patient died after surgery. Even though it was a surgical emergency, the patient was admitted to a Medical Department because she was regularly examined by the Digestive Endoscopy Unit for a hiatal hernia. The Authors emphasize the contribution of endoscopy to the preoperative recognition of this condition.

Gastric volvulus is a rare and potentially life-threatening condition that usually presents acutely and requires immediate intervention via either endoscopic or surgical detorsion. Most often, it presents secondary to a hiatal hernia, with herniation and torsion of the stomach through the hiatus. Only a small subset of patients present with gastric volvulus after Nissen fundoplication for hiatal hernia repair. We report the case of an elderly patient with a previous hiatal hernia repair with Nissen fundoplication who presented with an intra-abdominal gastric volvulus that developed over the co

Intrathoracic gastric volvulus is a rare event. It occurs when the stomach undergoes organoaxial torsion in the chest, caused either by concomitant enlargement of the hiatus or by a diaphragmatic hernia. A delay in diagnosis and treatment can result in fatal complications such as gastric ischemia, perforation, and hemorrhage. We report a case of intrathoracic localization of an acute and incarcerated organoaxial gastric volvulus caused by a left-sided diaphragmatic hernia resulting from a diaphragmatic injury. The patient had undergone a left splenopancreatectomy 4 years earlier for non-Hodgki

Bilateral congenital eventration of the diaphragm almost uniformly presents in infancy with respiratory compromise and is associated with a high mortality rate. Delayed presentation of diaphragmatic eventration in older children and adults may be associated with acute gastric volvulus. Thus, any patient with abdominal pain, vomiting, or nonspecific gastrointestinal symptoms in association with abnormal diaphragmatic findings on chest x-ray should undergo further diagnostic workup with upper gastrointestinal series or computed tomography (CT) scan. Treatment of gastric volvulus requires immedia

A 42-year-old man was admitted to coronary care for assessment with severe retrosternal chest pain. Echocardiography showed significant external compression of the left atrium. A subsequent CT scan revealed him to have a large hiatus hernia, with most of his stomach herniating into his thorax causing left atrial compression and gastric volvulus. He subsequently underwent successful emergency decompression of the gastric volvulus and repair of his hiatus hernia.

A review of the world literature has revealed only 11 cases of gastric volvulus symptomatic in the first month of life. To those 11, this report adds two cases of intrathoracic organoaxial gastric volvulus that were observed in the first week of life and were managed operatively. Gastric volvulus should be considered in the differential diagnosis of newborn infants initially observed to have persisting regurgitation, vomiting, and respiratory distress. The diagnosis can be made with plain thoracoabdominal roentgenograms and confirmed by upper gastrointestinal contrast studies. Prompt surgical

Recurrent neo-left colonic volvulus is a rare complication following anterior resection. The conventional approach to treating recurrent volvulus is a large bowel resection with anastomosis or colostomy formation after successful endoscopic decompression. However, in elderly and comorbid patients, this can result in significant morbidity or mortality. Laparoscopic colopexy is a less invasive alternative that has not been previously reported for the treatment of neo-left colonic volvulus. We describe a case of an 86-year-old male who presented with recurrent neo-left colonic volvulus 10 years p

BACKGROUND AND OBJECTIVES: Acute and chronic gastric volvulus usually present with different symptoms and affect patients primarily after the fourth decade of life. Volvulus can be diagnosed by an upper gastrointestinal contrast study or by esophagogastroduodenoscopy. There are three types of gastric volvulus: 1) organoaxial (most common type); 2) mesenteroaxial; and 3) a combination of the two. If undetected or if a delay in diagnosis and treatment occurs, serious complications can develop.
METHODS: We present four cases of surgical repair of organoaxial volvulus consisting of laparoscopic r

BACKGROUND: The definition of gastroesophageal reflux disease is gastroesophageal reflux with associated symptoms or complications. Because parents or guardians perceive the disease's symptoms differently, it is difficult to determine how common it is. Although gastric volvulus and wrap migration are well-known complications following Nissen fundoplication, the herniation in our case was caused by a defect medial to the hiatus, which could potentially compromise the patient's life in the future.
CASE PRESENTATION: We present a rare complication post laparoscopic Nissen fundoplication, four ye
