Words and Worms: The Nosology of Strongyloidiasis.

Strongyloidiasis impacts millions of people on several continents. Although chronic infection is often subclinical, select forms of immune dysregulation predispose patients to severe clinical manifestations resulting from acceleration of the nematode's autoinfective cycle. The terms "hyperinfection syndrome" and "disseminated strongyloidiasis" commonly refer to these disease states; however, contemporary usage is not uniform. We herein review the origins of terms related to the nosology of strongyloidiasis and highlight important mechanisms of disease pathogenesis that make untreated strongylo…

Strongyloidiasis Infection As the Cause of Pancreatitis.

Strongyloidiasis is endemic to socioeconomically disadvantaged tropics and subtropics, primarily as an asymptomatic carriage. It is seen in developed nations among the underprivileged. We describe a case of an Asian immigrant with eosinophilia, common bile duct stricture, cholangitis, and pancreatitis with Strongyloides spp. found on duodenal biopsy in the setting of steroid exposure.

Methotrexate exposure and risk of strongyloidiasis.

OBJECTIVE: Rheumatologic disease patients receiving immunomodulating drugs such as methotrexate (MTX) have increased infection rates. Strongyloides, a global endemic intestinal parasite, can cause significant or fatal disease in immunocompromised patients. The risk of serious Strongyloides infection with MTX dosed for rheumatologic disease is unknown. METHODS: We performed a systematic literature review searching EMBASE, Medline and Web of Science databases. All studies reporting humans exposed to MTX and tested for Strongyloides were reviewed. Exclusion criteria were bone marrow transplantat

Strongyloidiasis: A Disease of Socioeconomic Disadvantage.

Strongyloidiasis is a disease caused by soil transmitted helminths of the Strongyloides genus. Currently, it is predominately described as a neglected tropical disease. However, this description is misleading as it focuses on the geographical location of the disease and not the primary consideration, which is the socioeconomic conditions and poor infrastructure found within endemic regions. This classification may result in misdiagnosis and mistreatment by physicians, but more importantly, it influences how the disease is fundamentally viewed. Strongyloidiasis must be first and foremost consid…

Strongyloidiasis in the immunocompetent: an overlooked infection.

We examined the survivin expression pattern by immunohistochemistry in 43 fibroadenomas and 153 ductal carcinomas of the breast. The subcellular localization of survivin and the intensity of immunoreaction were assessed. We analyzed the differences of survivin expression between fibroadenomas and carcinomas. We also correlated the survivin expression pattern in carcinomas with other clinicomorphological parameters such as the age of patients, the grade and size of primary tumor as well as the lymph node metastasis. Overall, survivin was detected in 107/153 carcinomas (69.9%) and in 26/43 fibro

A Fatal Case of Strongyloidiasis in Pregnancy.

BACKGROUND: Strongyloides stercoralis is a common human parasite worldwide and has been associated with severe infection in immunosuppressed patients. High mortality rates have accompanied this severe disseminated infection. There is a scarcity of literature surrounding severe Strongyloides infection in pregnancy. CASE: A 30-year-old primigravid Haitian woman at 25 weeks of gestation presented with acute abdominal pain and an abnormal fetal heart tracing. Mild anemia and eosinophilia were laboratory abnormalities on admission. She received corticosteroids for the fetus and subsequently develo

Update on strongyloidiasis in the immunocompromised host.

Immunocompromised persons are the most vulnerable population at risk for developing life-threatening clinical syndromes associated with strongyloidiasis, such as hyperinfection syndrome (HS) or dissemination. This review focuses on describing Strongyloides infection in the immunocompromised host, including immune response against this infection, analyzing the cases with HS published during the past 4 years in the United States, and describing the most sensitive diagnostic tools and the most effective treatment for each clinical syndrome. Strongyloidiasis is becoming an important parasitic dise…

Disseminated strongyloidiasis in a immunocompromised host.

Strongyloidiasis in an immunocompromised patient has the potential to be life threatening. We describe a boy who was on steroids for acute demyelinating myelitis and receiving antibiotics for E. coli UTI and meningitis. He developed anasarca, malabsorption, malnutrition and left ventricular failure. Duodenal biopsy revealed abundant rhabditiform larvae of Strongyloides stercoralis. The diagnosis went unsuspected and proved fatal. This emphasizes the need to have a high index of suspicion and early intervention for S. stercoralis in immunosuppressed persons who present with refractory gastroint…

Seroepidemiology of strongyloidiasis in the Peruvian Amazon.

A stool and serosurvey for Strongyloides stercoralis was conducted in a community in the Peruvian Amazon region. Strongyloidiasis stercoralis was identified in the stool of 69 (8.7%) of 792 participants. Six hundred nine sera were tested using by an enzyme-linked immunosorbent assay (ELISA), which had a sensitivity of 92% and a specificity of 94%; 442 (72%) were positive. In multivariable logistic regression models, having S. stercoralis in stool was associated with hookworm in the same specimen (odds ratio [OR] = 4.44, 95% confidence interval [CI] = 2.02-9.79), occasionally or never wearing s…
