Massive facial keloid precipitated by chronic suppuration and mechanical skin traction: A case report.

Keloids are benign fibroproliferative tumours with a high recurrence rate of 20%-100%, therefore, multimodal treatment is recommended. We report the case of an exceptionally large facial keloid precipitated by a vicious cycle of chronic inflammation and mechanical skin traction and discuss the management challenges in a low resource setting. A 67-year-old man presented with a 10-year history of a facial keloid that rapidly enlarged to 2,800 g in 2 years causing difficulties eating, speaking, dressing, head movements and breathing. He had multiple other smaller keloids, hypertension, HIV, and a

Towards improved outcome in children treated surgically for spontaneous intracranial suppuration in South Wales.

INTRODUCTION: Spontaneous central nervous system (CNS) infections in children are rare. Treatment involves surgical intervention and antibiotic therapy. We describe a single centre experience of managing this condition in South Wales. METHODS: We performed a retrospective review of surgically managed cases in our unit for patients under 18 years of age between 2008 and 2018. Data were collected regarding aetiology, location, microbiology examination, treatment and outcomes. RESULTS: Twenty-six patients were identified of which 25 case notes were available. Fifteen were male and 10 were femal

The prevalence of inverse recurrent suppuration: a population-based study of possible hidradenitis suppurativa.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory disease involving inverse recurrent suppuration (IRS). The epidemiology of the disease is not well described, with previous studies reporting prevalence estimates from 0·00033% to 4%. OBJECTIVE: To determine the prevalence of IRS in a large population-based survey. METHODS: Data were obtained from a general cross-sectional population study in Denmark. A validated self-administered questionnaire was used to identify IRS. Persons with the combination of outbreaks of 'boils' during the last 6 months in predefined areas and a mi…

[Verneuil's disease in the differential diagnosis of suppuration of the ano-perineo-buttock region].

Verneuil's disease or hidradenitis suppurativa is not frequent. Nevertheless since VERNEUIL's classic description in 1854, many cases have been reported. It is a disease of the apocrine sweat glands, that may arise on each of their localizations: axilla, breast areolae, umbilicus, perineum, groin, buttocks. The chronic suppurative disease of these glands give rise to multiple fistulous constantly infected tracts. The typical appearance usually allows a clinical diagnosis. Other disease may nevertheless look like hidradenitis suppurativa. We present two cases and discuss the differential diagno

Deep learning enables accurate diagnosis of acute cholecystitis and prediction of suppuration using noncontrast CT.

To develop a deep learning model using abdominal noncontrast computed tomography (CT) for diagnosing acute cholecystitis (AC) and predicting progression to acute suppurative cholecystitis (ASC). A total of 641 patients from three medical centers were retrospectively analyzed. Deep learning models were constructed for AC diagnosis and ASC prediction. Model interpretability was assessed using gradient-weighted class activation mapping (Grad-CAM) and t-distributed stochastic neighbor embedding. Performance was compared with radiomics models and radiologist assessments. The deep learning model ach

Giant Echinococcosis of the Liver with Suppuration: A Case Report and Review of the Literature.

Purpose: Cystic echinococcosis (CE) is a common, complex parasitic disease that constitutes a major public health concern. CE demonstrates high endemicity in areas where dogs are used for herding or where animal husbandry practices involve close contact with livestock. It can clinically manifest with a variety of signs and symptoms, such as cholangitis, jaundice, pancreatitis, external biliary fistula, inferior vena cava obstruction, portal hypertension, and superinfection. The latter can notably be related to suppuration, either by rupture or bacteremia. The aim of this study is to report our…

External fistulous wound with Pseudomonas aeruginosa infection and massive bleeding following rupture of pulmonary suppuration.

We present a 54-year old man with a pulmonary infectious cavity continuing to a cutaneous fistula. Before he was admitted to our hospital, he had undergone open-window surgery for a left thoracic empyema due to the rupture of pulmonary suppuration of the left upper lobe. He had then undergone thoracoplasty with the plombage of the cavity using left pectoralis major muscle. However, this procedure had failed and the external fistulous wound remained infected by Pseudomonas aeruginosa and occasional massive bleeding from the cavity occurred. He underwent en bloc left upper lobectomy for the exte…

Acute suppuration of the pancreatic duct associated with pancreatic ductal obstruction due to pancreas carcinoma.

Acute obstructive suppurative cholangitis is a well-known clinical entity; however, acute suppuration of the pancreatic duct in the setting of pancreatic ductal obstruction is an uncommon pancreatic disorder. We report a case of acute suppuration of the pancreatic duct without either a concomitant pancreatic abscess or an infected pseudocyst, presenting as acute relapsing pancreatitis. In this case, the underlying cause of suppuration of the pancreatic duct was pancreatic ductal obstruction and chronic pancreatitis secondary to pancreas head carcinoma along with infection of Escherichia coli. …

[The capacities of autoleukocyte-labelled scintigraphy in the detection of foci of inflammation and suppuration].

The diagnostics of occult inflammation foci up to day remains the major and complicated problem in almost each clinical discipline. Whole-body scintigraphy and SPECT studies with 99mTc-autoleucocytes show silent foci of inflammation and infection. 68 patients (24 females and 44 males) aged 14-59 years with non-localized inflammatory diseases were evaluated. Each patient underwent routine clinical and laboratory investigation. But because of the inadequacy of other diagnostic procedures, the Tc-99m-WBC scans may be of great clinical importance. For the determining of the SPECT of thorax were pe

Subcorneal pustular dermatosis (Sneddon-Wilkinson disease) with amicrobial lymph node suppuration and aseptic spleen abscesses.

Subcorneal pustular dermatosis (SPD) or Sneddon-Wilkinson disease is a skin eruption in which concomitant systemic involvement has not been reported to our knowledge. We describe a patient suffering from SPD of 6 years duration with amicrobial inguinal lymph node suppuration and aseptic spleen abscesses followed by pyoderma gangrenosum.
