3D-Printed PLA/PETG Sterilizable Static/Dynamic Modular External Finger Fracture Fixator.

Finger fractures are common and potentially disabling. Certain complex injuries, particularly with small fragments and/or open wounds, are not manageable with conventional orthopedic hardware. We created 3D-printed finger fracture fixators using polylactic acid (PLA) and polyethylene terephthalate glycol (PETG). We tested flexion stiffness and lateral stiffness, before and after sterilization for surgical use, and compared them against standard 316L surgical stainless-steel (SSS) wires. Because PLA and PETG are compromised at high temperatures, these were sterilized using hydrogen peroxide gas

Complex Open Pelvic Fracture in an 8-Year-Old Girl Treated With INFIX-A Case Study.

We describe the case of an 8-year old female patient with an open pelvic fracture after being run over by a bus. Open pelvic injuries in pediatric patients are very rare and are associated with high mortality rates and long-term morbidity. In this case, a multidisciplinary surgical approach is described. The injuries include a complex pelvic ring fracture, which was treated with an internal external fixator, together with severe urogenital and soft-tissue injury. The internal external fixator, a surgical technique involving a temporary internal fixation device, is well described in adults, but

A Novel Inexpensive Technique to Seal Negative Pressure Wound Therapy on External Fixation Devices.

Negative pressure wound therapy, a tool widely applied to treat lower limb traumas, is useful in reconstructive procedures. However, obtaining an airtight vacuum seal when using a negative pressure dressing around an external fixation device can be complicated and time-consuming. The plastic drape seldom adheres to screws, pins, or wires and, as such, the vacuum seal is jeopardized. In surgical departments, colostomy paste is widely used, readily available, and applied where pins and wires make contact with the plastic drape. It is a fast, practical, and inexpensive method of preventing air le

Osteonecrosis secondary to internal fixation.

Allergies to metals as a cause of failure of joint prostheses has historically been a controversial issue. Loosening of a prosthesis may be due to trauma, infection of the surrounding area, or a faulty implantation of the device. However, cases of loosening of the device do occur without any history of the above criteria. This leads one to consider other explanations for this phenomenon, such as sensitivity of the tissues to one of the metals in the implant. The following presentation demonstrates that metal allergies are a surgical concern when considering joint prostheses, or internal fixati

Surgical procedures and experimental outcomes of closed fractures in rodent models.

The closed fracture rat model, first described by Bonnarens and Einhorn, has been widely implemented in recent years to characterize various fracture phenotypes and evaluate treatment modalities. Slight modifications in the fixation depth, to reduce surgical error associated with movement/dislocation of the k-wire fixation, were previously described. Here, we describe this method which involves the creation of a medial parapatellar incision, dislocation of the patella, boring an 18 gauge hole through the center of the femur, delivery of an adjunct (if applicable), fixation of the k-wire in the

A modified technique for fixation of chronic instability of the distal tibiofibular syndesmosis using a wire and button.

Distal tibiofibular syndesmosis injury accounts for 1% to 11% of soft tissue injuries of the ankle. Some acute syndesmotic injuries will fail to heal effectively owing to inadequate treatment or misdiagnosis, eventually resulting in chronic instability, which can destroy the stability of the ankle joint. Various surgical techniques have been described for fixation of the syndesmosis. Among the existing methods, the suture button has the advantage of allowing for physiologic micromotion at the syndesmosis by maintaining the reduction and preventing the risk of screw breakage. However, the "rela

Intramedullary nailing in opening wedge high tibial osteotomy-in vitro test for validation of a method of fixation.

PURPOSE: Opening wedge high tibial osteotomy (HTO) as a treatment in unicompartimental osteoarthritis of the knee can significantly relieve pain and prevent or at least delay an early joint replacement. The fixation of the osteotomy has undergone development and refinements during the last years. The angle-stable plate fixator is currently one of the most commonly used plates in HTOs. The angular stable fixation between screws and the plate offers a high primary stability to retain the correction with early weight-bearing protocols. This surgical technique is performed as a standard of care an

New Device for the Treatment of Functional Ischemic Mitral Regurgitation: Proof of Concept in an In Vitro Model.

BACKGROUND: Optimal surgical treatment of functional ischemic mitral regurgitation (FIMR) is still controversy. Due to the underlying pathophysiology, stand-alone ring annuloplasty is assumed with a high recurrence rate of mitral regurgitation, thus additional subvalvular repair techniques might improve the results. This in vitro study introduces a new device for subvalvular mitral valve repair. METHODS: We developed a new device for subvalvular mitral valve repair, consisting of two frames for papillary muscle (PM) attachment, which are connected with two holding bars serving for fixation of

Total Hip Arthroplasty in Failed Hip Fractures, a Challenge with Fruitful Outcome: A Case Series.

INTRODUCTION: Failed fracture fixation around the hip presents a major challenge in orthopedic surgery. Primary total hip arthroplasty (THA) is often required as a definitive salvage procedure following complications such as non-union, malunion, implant failure, or post-traumatic arthritis. OBJECTIVES: This study aims to evaluate the clinical outcomes, complications, and surgical strategies of primary THA performed as a salvage procedure following failed internal fixation of fractures around the hip, with emphasis on the use of adjuncts such as Prolene mesh, augmentation plates, cerclage wire

A straightforward technique for removal of the substernal bar after the Nuss operation.

Pectus excavatum is the most common chest wall deformity seen in the pediatric population. There have been a number of reports describing the operative correction of pectus excavatum, but scant literature is available describing removal of the substernal bar. This report describes a straightforward technique for removal of the substernal bar after the Nuss operation. This technique has been used in more than 110 patients without complications.

Can applied external fixators be sterilized for surgery? A prospective cohort study of orthopaedic trauma patients.

BACKGROUND: Temporary external fixators are often used to stabilize fractures when definitive fracture surgery must be delayed. Sometimes, external fixators are left in place during repeat operations, including definitive internal fixation of tibial pilon and tibial plateau fractures. It is unknown how well current surgical preparation sterilizes these devices, which become part of the surgical field. Our hypothesis was that our institution's standard surgical preparation creates a low rate of culture-positive environments on external fixators at the time of surgical skin incision. METHODS: W

Unstable Open Posterior Subtalar Dislocation Treated With a Ring External Fixator: A Case Report and Review of the Literature.

Traumatic dislocation of the subtalar joint is an infrequently occurring injury, with open true posterior dislocation an even rarer injury. We describe our treatment of a young motorcyclist who was brought into hospital after a road traffic accident, having sustained an open posterior subtalar dislocation. After initial reduction and resuscitation in the emergency department, he was taken to the operating theater for emergent wound debridement and external fixation of his ankle using a unilateral external fixator device. After 2 subsequent repeat debridements, this was changed to a ring extern

Lateral mass screw complications: analysis of 1662 screws.

STUDY DESIGN: Retrospective, consecutive patient series. OBJECTIVE: To quantify the risks and the complications associated with screw fixation devices of the cervical spine. SUMMARY OF BACKGROUND DATA: The usefulness of lateral mass internal fixation has been well documented in the clinical setting. However, there is a paucity of studies examining the complications associated with these devices in a degenerative clinical setting. METHODS: From 1999 to 2007, 225 consecutive patients underwent posterior cervical fixation using a screw-plate and polyaxial screw-rod implant systems. There were

Primary closure versus secondary healing of pin sites after the removal of provisional external fixator: a systematic review and meta-analysis.

BACKGROUND: External fixators are commonly used for managing complex fractures, but they are associated with the risk of pin site infections. Pin site management following external fixator removal can be performed using either primary closure or secondary healing. The relative efficacy and safety of these approaches, particularly in terms of infection rates, remain uncertain. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the infection rates of pin entry sites following the removal of the temporary external fixator, comparing primary closure and secondary healing method
