OBJECTIVES/HYPOTHESIS: 1) To determine the prevalence of dysphagia in children with laryngomalacia, 2) To ascertain whether severity of laryngomalacia influences the presence of swallowing dysfunction, and 3) To examine whether patients with medical comorbidities and laryngomalacia have a higher prevalence of swallowing dysfunction.
STUDY DESIGN: Retrospective cohort study.
METHODS: All patients seen in the aerodigestive center at our institution between January 2007 and December 2012 with the diagnosis of laryngomalacia were included. Swallowing function was assessed by symptoms, clinical s

OBJECTIVES: Patients hospitalized in the ICU can frequently develop swallowing disorders, resulting in an inability to effectively transfer food, liquids, and pills from their mouth to stomach. The complications of these disorders can be devastating, including aspiration, reintubation, pneumonia, and a prolonged hospital length of stay. As a result, critical care practitioners should understand the optimal diagnostic strategies, proposed mechanisms, and downstream complications of these ICU-acquired swallowing disorders.
DATA SOURCES: Database searches and a review of the relevant medical lit

This article provides an overview of nonsurgical and nonpharmacologic treatments for oropharyngeal dysphagia. The speech-language pathologist (SLP) is the primary member of the swallowing management team who will provide this type of dysphagia management. The primary focus of the SLP for dysphagia management is first to eliminate or reduce aspiration risk, as well as to improve or restore swallowing function. Ultimately, the management plan will depend on the physiologic underpinnings of the disorder and patient variables such as cognition, motivation, and ability to attend therapy sessions or

PURPOSE: Dysphagia (swallowing dysfunction) is a debilitating, depressing, and potentially life-threatening complication in cancer patients that is likely underreported. The present paper is aimed to review relevant dysphagia literature between 1990 and 2010 with a focus on assessment tools, prevalence, complications, and impact on quality of life in patients with a variety of different cancers, particularly in those treated with curative chemoradiation for head and neck cancer.
METHODS: The literature search was limited to the English language and included both MEDLINE/PubMed and EMBASE. The

In this paper, a mathematical modeling of the swallowing sound generation is presented. To evaluate the model, its application on swallowing disorder (dysphagia) diagnosis is discussed. As a starting point, a simple linear time invariant model is assumed to represent the pharyngeal wall and tissue excited by a train of impulses. The modeling is approached by two different assumptions. In one approach, it is assumed that the impulse train, representing the neural activities to trigger swallow, is the same for both groups of control and dysphagic, and it is the pharyngeal model that accounts for

It is difficult to teach students about the mechanism of swallowing. There are three phases of swallowing; oral phase, pharyngeal phase and esophageal phase. The bolus of food is propelled to back of mouth by the tongue and the swallowing reflex happens. After nasopharynx and mouth closure, the glottal closure occurs, then hyoid and larynx are lifted by the contractions of suprahyoid and thyrohyoid muscles. As for the epiglottis, it is compressed by the tongue and inclines downward. As the larynx is lifted upward and anteriorly, slight vacuum is caused in the lower pharynx and upper esophagus

OBJECTIVE: To clarify the pathophysiology of dysphagia by electrophysiological methods.
METHOD: Electrophysiological methods related to oropharyngeal swallowing were used to investigate 25 patients with cervical dystonia and 25 age matched normal volunteers.
RESULTS: Dysphagia was suspected in 36% of patients with cervical dystonia on the basis of clinical assessment. The incidence of dysphagia increased to 72% on electrophysiological evaluation of pharyngeal swallowing. Submental muscle electromyographic (EMG) and laryngeal relocation times were significantly prolonged and the triggering ti

We present the case of a 59-year-old man who sustained an esophageal perforation as a result of sword swallowing. An esophagogram established the diagnosis, and surgical repair was attempted. However, 19 days later, a persistent leak and deterioration of the patient's condition necessitated a transhiatal esophagectomy with a left cervical esophagogastrostomy. The patient recovered and has resumed his daily activities at the circus, with the exception of sword swallowing. This case report presents an unusual mechanism for a potentially lethal injury. Choking phobia is characterized by fear and avoidance of swallowing food, fluids or pills. It often appears in women after an episode of choking on food. We present the case of an adolescent beginning her phobia after a mononucleosis with severe dysphagia. Patient's solid restriction raised concerns about differential diagnosis with eating disorders. She was treated with psychoeducation, cognitive restructuring and graduated exposure in vivo, achieving a total recovery.

Swallowing disorders are common, especially in the elderly, and may cause dehydration, weight loss, aspiration pneumonia and airway obstruction. These disorders may affect the oral preparatory, oral propulsive, pharyngeal and/or esophageal phases of swallowing. Impaired swallowing, or dysphagia, may occur because of a wide variety of structural or functional conditions, including stroke, cancer, neurologic disease and gastroesophageal reflux disease. A thorough history and a careful physical examination are important in the diagnosis and treatment of swallowing disorders. The physical examinat

Early diagnosis and treatment of feeding and swallowing problems may improve a child's health status and quality of life and reduce health care costs. Pediatric nurse practitioners have a salient role in the early diagnosis of children at risk for feeding and swallowing problems. Increased knowledge about the normal swallowing process, children at risk for feeding and swallowing disorders, and associated symptoms enhances early diagnosis and treatment. The child's general health history and physical evaluation, along with additional diagnostic tests, provide the basis for the formulation of an

The aim of this study was to look at whether full vocal cord adduction is achieved in 45 normal subjects when asked to hold their breath, as in the supraglottic safe swallow technique. Fiberoptic nasendoscopy was used to assess vocal cord adduction. Results show that 57.7% of normal subjects do not fully close their vocal cords for the complete duration of breath holding. This finding has implications for the efficacy of teaching methods of the supraglottic safe swallow technique.

Congenital anomalies of the head and neck and congenital and acquired neuromuscular disorders in children are frequently associated with dysfunctional feeding and swallowing. Determination of the affected phase of swallowing and appropriate guidelines for feeding depend on detailed clinical assessment at the bedside and radiographic evaluation by videofluoroscopy. Fiberoptic and rigid endoscopic examination of the airway are useful in the diagnosis of subtle structural abnormalities that are not appreciated on physical and radiologic examination. Once a diagnosis is established, repeating the

Melatonin is being touted as the latest cure-all. But in their eagerness to explain to the public the possible health benefits of this natural hormone, the mass media as well as some scientists have misrepresented the scientific data.

Swallowing or aspiration of a foreign body is a complication that may arise from any procedure in the oral cavity performed without the use of a rubber dam. Two cases of swallowed endodontic instruments and possible complications are presented. Emphasis is given in the discussion on the early diagnosis and treatment and primarily on prevention with the use of rubber dam, an absolute essential during endodontic treatment.

Swallowing and aspiration of dental foreign objects is often reported in the literature. Swallowing seems to be more common than aspiration, and is observed most often in the elderly. Too often the size and configuration of these objects compounds their impaction and removal. A 67-year-old man swallowed a unilateral removable partial denture which became lodged in his mid-oesophagus. The denture was located by radiographic examination of his chest and removal by a rigid oesophagoscopy was performed under general anaesthetic.

We examined the effect of continuous swallowing on breathing pattern and ventilation in 7 adult subjects. Repetitive swallowing was induced by oral infusion of water at a variable rate of 40, 60, 80 or 100 ml/min, while the subject breathed through the nose. The number of swallows increased from a mean of 5.2 (+/- 2.7 SD) swallows/min during the control period to 9.2 +/- 2.0 to 13.7 +/- 2.9 swallows/min during infusion of 40 and 100 ml/min, respectively. The duration of interruption of breathing was bolus volume-dependent, increasing from 0.55 +/- 0.09 sec with a mean bolus volume of 4.6 +/- 1…
