A case of thoracic degloving injury with flail chest.

A 75-year-old male patient suffered a chest degloving injury when he fell on his back and was run over by a small farm tractor he was pulling. At the time of patient admission, the paradoxical motion of the right chest wall was remarkable; and he had an open fracture of the right humerus, a dislocation of the right ankle, and a laceration of the right forearm. Chest computed tomography revealed fractures of the fifth to seventh ribs and detachment of both the right pectoralis major muscle and serratus anterior muscle from the chest wall, with a disconnected right thoracic cavity. Because the r

Injury of the thoracic aorta during laparoscopic esophagectomy.

We report an unusual complication of laparoscopic esophagectomy for carcinoma. During posterior transhiatal dissection, massive bleeding occurred from the left side of the supradiaphragmatic thoracic aorta. A small tear was clearly visible and temporary bleeding control by packing and pressure with a grasper through one of the trocars was effective. A median laparotomy was performed and the aorta was sutured using a standard laparoscopic needle holder. The esophagectomy was successfully completed through a right thoracic approach. Injury to the intrathoracic aorta may occur during transhiatal

Neurogenic thoracic outlet syndrome in whiplash injury.

A prospective study of 110 patients was carried out to determine the pathogenic significance of trauma to the upper body in the development of neural compressive irritation at the thoracic outlet. Twenty-nine patients were reviewed as cervical strain injuries (N group), 25 patients as probable neurogenic thoracic outlet syndrome (NTOS) (PT group), 39 patients as definite NTOS (T group), and 17 patients as NTOS associated with cervical disc disease (CD-T group). The time lapse between accident and diagnosis and the duration of treatment were significantly longer in T patients or CD-T patients t

Urban thoracic trauma: diagnosis and initial treatment of non-cardiac injuries in adults.

This comprehensive review aims to delineate the prevailing non-cardiac thoracic injuries occurring in urban environments following initial on-site treatment and subsequent admission to hospital emergency departments. Our study involved a rigorous search within the PubMed database, employing key phrases and their combinations, including "thoracic injury," "thoracic trauma," "haemothorax," "lung contusion," "traumatic pneumothorax," "rib fractures," and "flail chest." We focused on original research articles and reviews. Non-cardiac thoracic injuries exhibit a high prevalence, often affecting po…

Comparison of ultrasonography and computed tomography in the determination of traumatic thoracic injuries.

OBJECTIVE: In this study, the accuracy of bedside thoracic ultrasonography (TUSG) performed by emergency physicians with patients in the supine position was compared with that of thoracic computed tomography (TCT) for the determination of thoracic injuries due to trauma. METHODS: Patients who suffered the multiple traumas, whose thoracic trauma was identified on physical examination or TCT imaging were included in the study. TUSG was performed following a physical examination by the emergency physician who managed the trauma patient. Subcutaneous emphysema, pneumothorax, pulmonary contusions …

Functional outcomes of motor vehicle crash thoracic injuries in pediatric and adult occupants.

OBJECTIVE: Characterization of the severity of injury should account for both mortality and disability. The objective of this study was to develop a disability metric for thoracic injuries in motor vehicle crashes (MVCs) and compare the functional outcomes between the pediatric and adult populations. METHODS: Disability risk (DR) was quantified using Functional Independence Measure (FIM) scores within the National Trauma Data Bank for the most frequently occurring Abbreviated Injury Scale (AIS) 2-5 thoracic injuries. Occupants with thoracic injury were classified as disabled or not disabled b…

When and how should we manage thoracic aortic injuries in the modern era?

A best evidence topic in cardiovascular surgery was written according to a structured protocol. The question addressed was what are the optimum treatment modality and timing of intervention for blunt thoracic aortic injury (BTAI) in the modern era? Of the 697 papers found using the reported search, 14 (5 meta-analyses, 2 prospective and 7 retrospective studies) represented the best evidence to answer the clinical question. The author, journal, country, date of publication, patient group studied, study type, relevant outcomes, results and weakness of these papers are tabulated. All five meta-an

A systematic review and pooled analysis of CPR-associated cardiovascular and thoracic injuries.

OBJECTIVE: The incidence of thoracic injuries resulting from cardiopulmonary resuscitation (CPR) is not well characterized. We describe a case in which a CPR-associated atrial rupture was identified with ultrasound and successfully managed in the intensive care unit with a bedside thoracotomy and atrial repair. We then describe a systematic review with pooled data analysis of CPR-associated cardiovascular, pulmonary, pleural, and thoracic wall injuries. DATA SOURCES: PubMed, Scopus, EMBASE, and Web of Science were searched to identify relevant published studies. Unpublished studies were ident…

Analysis of cardiac and pulmonary injuries resulting from an experimental penetrating thoracic injury.

BACKGROUND: This study was planned to analyze the internal and external anatomical findings of cardiac injuries and the presence of accompanying pulmonary injuries in intentionally inflicted thoracic injuries to swine models. METHODS: We inflicted a penetrating heart injury in six suis domesticus female swine models. Two cardiac injuries, one on the left paratracheal of fourth intercostal space (ICS) and the other on the right side were inflicted on each model by the same researcher using a 20-cm long scalpel. All animals were then sacrificed for morphological evaluation. RESULTS: After stri…

Thoracic injuries to contained and restrained occupants in single-vehicle pure rollover crashes.

Around one in three contained and restrained seriously injured occupants in single-vehicle pure rollover crashes receive a serious injury to the thorax. With dynamic rollover test protocols currently under development, there is a need to understand the nature and cause of serious thoracic injuries incurred in rollover events. This will allow decisions to be made with regards to adoption of a suitable crash test dummy and appropriate thoracic injury criteria in such protocols. Valid rollover occupant protection test protocols will lead to vehicle improvements that will reduce the high trauma bu…

[Forensic expertise of thoracic aorta, heart and pericardial injuries in car-occupant fatalities].

INTRODUCTION: Forensic expertise has not specified with certainty any specific injury among fatally injured frontal car-occupants in frontal car collisions. OBJECTIVE: To establish if blunt injuries of thoracic aorta, heart, and pericardium could be used as predictors where the fatally injured car-occupant was at the moment of car-collision. METHODS: A retrospective autopsy study was performed. The subjects were fatally injured car-drivers, front-seat and rear-seat passengers in head-on car collisions. In each of them we analyzed the injuries of thoracic aorta, heart and pericardium. RESULT

[Value of imaging procedures in the diagnosis and therapy of severe thoracic injuries].

Plain chest radiographs allow the diagnosis of most intrathoracic injuries. However, they are only momentary pictures and give no information on the respiratory function. A tension pneumothorax, rib fractures and subcutaneous emphysema should be diagnosed clinically before radiographs are taken. Computed tomography is helpful in evaluation of intrapulmonary lesions, hemothorax, rupture of the diaphragm and dislocation of the heart. Further diagnostic tools include aortography in suspected aortic rupture, sonography in cardiac injuries and hemopericardium, bronchoscopy in suspected bronchial or…

[Osteosynthesis of severe injuries of the thoracic and lumbar spine with internal fixation].

From a posterior approach long Schanz screws are inserted through the pedicles into the bodies of the two vertebrae just adjacent to the lesion and connected by the threaded Fixateur-interne-rods. By tightening the nuts the Schanz screws are fixed in all directions. The advantages of the F.I.-system are: excellent reposition by the long lever-arm of the Schanz screws, immobilization of only two segments avoiding the iatrogenic loss of mobility and lumbar lordosis, stability against flexion forces better than with Harrington distraction rods, additional rotational stability.

[Interest of prolonged peridural anaesthesia in the treatment of thoracic injuries (author's transl)].

Prolonged peridural analgesia relieves the pain and thanks to immediate return of ventilatory function avoids most complications of poor prognosis in mobile sternum. After a historical and pharmacological survey, the authors describe a standard technic, the course and the necessary supervision. The study of 29 cases of mobile sternum of variable gravity, treated in this way, permitted the authors to determine the indications and contra-indications and the role of associated treatments: physiotherapy, surgical fixation, respiratory assistance. The rare failures may benefit from another therapeu

Management of moderate blunt thoracic aortic injuries in patients with solid organ injury.

OBJECTIVE: Blunt thoracic aortic injuries (BTAIs) are the second leading cause of trauma-related deaths in the United States. Using the Harborview grading system, BTAIs can be classified as minimal, moderate or severe. Patients with BTAIs often present with multiple concomitant injuries, including solid organ injuries (SOIs), which can influence treatment decisions. Although moderate BTAIs can undergo semi-elective repair, the optimal management of moderate BTAIs with associated SOIs is unknown. The aim of this study was to analyze our experience with patients presenting with concomitant moder
